Title gen doesn't fire on /goal's first prompt: the trigger runs (inbox promotion → SessionRunner title fiber), but SessionTitle.generate reads its input from SessionHistory.firstUserMessage, which filters on type = 'user' — and synthetic inbox items persist as type: 'synthetic'. So the title silently no-ops (if (!firstUser) return) until a real user message exists in the session. Fix is probably falling back to a synthetic message for title input when no user message exists yet.
