feat: support native TypeScript 7 LSP - #582

Use tsc --lsp by default for JavaScript and TypeScript. Support pull diagnostics, server refresh requests, and transient request retries. Wait for language server initialization and surface actionable errors when the server exits, rejects initialization, or times out. Cover the new behavior with fake-server and MCP integration tests.
Install TypeScript 7 in the sandbox and root development environment, replacing the separate typescript-language-server package.
