• ci(production-line-guard): the F11.10 line guard never runs — dispatch-only since the Nov 4 sweep, and main already fails it (directory.rs 2,610 lines, ungrandfathered)

• ci(opencode-config-guard): the metered-provider guard never runs — the Nov 4 sweep removed its pull_request and push triggers, and ci-policy still assigns it PR + main tiers

• update-nixpkgs-inputs: a requested ref that matches no inputs exits 0 — a typo'd ref (or --help) silently updates nothing while callers proceed as if it worked

• dispatch: fleet repo list names the nix repo by its pre-rename name — dezren39/darwin-nix is not allowlisted, has no queue/dir mapping, and its OIDC callers are rejected; only the redirected dezren39/nix spelling passes

• opencode-acp-adapter: --server value is never validated — a trailing --server silently talks to the default serve, --server= / a flag-like value loop forever on 'Failed to parse URL', and a typo'd flag is silently ignored
