Create missing parent directories when cache opens a filesystem :dbname path. Callers can point at a nested cache location without repeating directory setup; SQLite itself creates only the database file.

New filesystem databases on POSIX systems are created with mode 0600 so cached API/provider data is not exposed by an ordinary 022 umask. Existing files retain their permissions. SQLite URI paths (file:...) and in-memory databases retain their existing behavior.

Validation: bb test passes (57 tests, 316 assertions), including creating and using a database under missing nested directories and checking owner-only POSIX permissions. bb lint passes with no errors or warnings.
