fix(server): a restart no longer resends imported history to a session that already has it - #15495
A stopped, failed, or restarted first turn could leave imported history marked delivered to the native session, yet the next message sent it again because no V2 run had completed. The duplicate handoff could consume the remaining context allowance and prevent the thread from continuing.
Skip a new legacy handoff when delivery records show that the destination native session already received it. Preserve delivery to replacement sessions and retry uncertain or undelivered history. This changes provider-independent server orchestration; there is no client or wire-contract change.
Rebuilt from Carter4242’s #15495. The takeover replaces the predicate test matrix and synthetic cancellation with behavior checks through Stop, startup recovery, and existing provider-switch fixtures.
Verification: 75 original focused tests → 78 retained tests across Orchestrator.migration.test.ts (5→4), LegacyV1Cutover.integration.test.ts (2→6), and ProviderSwitch.integration.test.ts (68 unchanged). Removing the production fix makes four regressions fail: Stop, startup recovery, and both switch-return paths produce duplicate handoffs. Replacement-session, pending-delivery, and undelivered-history cases preserve delivery. The rebuilt implementation passed 121 focused tests, server typecheck, scoped lint, formatting, and git diff --check; lint retains two existing warnings. Two independent full-diff review rounds found no correctness blockers; the first round’s duplicate retry fallback was removed. The final revision also passes all 78 touched-suite tests, server typecheck, scoped lint and formatting.
The integration checks drive real orchestration, effect-worker milestones, and SQLite persistence using replay provider adapters. No real provider CLI/model session or client UI flow was exercised. macOS, Windows, iOS, Android, Electron desktop, and relay/tunnel connections were not verified. The reviewers also identified a possible pre-existing rollback/delivery invalidation issue; rollback after native delivery was not verified and is outside this fix. Screenshots are not applicable to this server-only behavior.

Macroscope's review found this PR approvable — The change prevents imported history from being resent when persisted records show it was already delivered to the same provider session. New sessions and imports without completed delivery evidence retain the existing preparation path.

The orchestrator now checks for an existing delivered legacy-import handoff before preparing another. Integration tests cover legacy cutover recovery scenarios and provider-switch failures.

Summary
Define and apply handoff coverage apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/Orchestrator.migration.test.ts
Adds a check for a matching delivered manual_context handoff in the standard and provider-switch legacy-import paths. Updates the migration test description.
Test legacy cutover recovery apps/server/src/orchestration-v2/legacy/LegacyV1Cutover.integration.test.ts
Extends the test adapter and boot helper. Adds scenarios for stop, restart, replacement, pending injection, and undelivered history.
Test recovery after provider-switch failures apps/server/src/orchestration-v2/testkit/ProviderSwitch.integration.test.ts
Waits for the failed runs and verifies that a later Codex run reuses the native thread without the imported transcript.
History-recovery behavior has no established blocking defect. Document the untraced helper or give it a named span; the change is otherwise mergeable with that follow-up.
Security Architecture Review
Security architecture risk:🔵 Low · up to 21092
The change narrowly prevents repeated history delivery without demonstrating broader access or increased privileges. Recovery retains conservative handling of uncertain delivery, but session-identity and concurrent recovery assumptions are not fully established.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The demonstrated effect is scoped to history delivery for an imported application thread and its selected provider thread. The reviewed change adjusts handoff eligibility rather than adding provider capabilities, tool authority, or a new authentication boundary.
Trust Boundaries and Controls
observed — The new predicate binds delivered coverage to both logical provider-thread identity and current native-thread identity. The unit test rejects coverage recorded for a different native thread, providing counterevidence to suppressing import delivery after an identity change.
Resilience and Maintainability Implications
observed — Delivery persists a pending marker before injection and records injected status only after success. Inline completion is recorded after turn start returns. A pending delivery on the current native thread triggers fresh-session recovery rather than being treated as completed coverage.
observed — Inspected dispatch paths use per-thread locking and command receipts to serialize local planning and avoid replaying an already processed command. These controls do not establish cross-node serialization.

## Summary
A follow-up on a long Grok thread failed before the prompt was sent:
`ContextHandoffBudgetError` ("Insufficient context allowance for the
provider handoff"). T3 replayed the pre-compact transcript as a
`manual_context` handoff and counted every stored byte as a token
against the 128k fallback, so the budget was 0. The same native session
still had room in the Grok CLI (about 59k of 256k after compact).
- Skip a v1 import handoff when one was already delivered into this
native thread.
- On the same native thread, drop handoffs already covered by a settled
delivery, and drop handoffs that target a run at or before a completed
`/compact`.
- Stop replaying missed failed or interrupted runs from before that
compact, and stop counting those items toward occupancy.
A new native session (resume fallback, provider switch) still gets the
full handoff and the hard budget check. The next message on an existing
session is sent to the provider.
Upstream [pingdotgg#15495](pingdotgg#15495) only
stops preparing another legacy handoff. It does not drop handoffs
already stored, and it does not change the occupancy estimate. It is
still open.

…n that already has it
An imported thread sent its imported history with every message until a V2
run completed. A Stop or a server restart cancels the run after the history
already reached the native thread, so each later message added another legacy
handoff. Check the delivery records of earlier legacy handoffs to the same
provider thread instead.
Document the untraced span choice or use a named Effect.fn.
legacyImportCovered is a service operation that performs a projection read (getThreadRecords). It uses Effect.fnUntraced, and no comment explains why. The guideline asks for a named span by default. It also asks for the reason to be written next to any deliberate fnUntraced use. The sibling helpers in this file carry such a comment.
Two options:
Use a named span: Effect.fn("Orchestrator.legacyImportCovered")(function* (...) { ... }).
Keep fnUntraced and add the same reason comment the other helpers use.
As per coding guidelines: "Reusable functions use Effect.fn("Service.operation")... Use Effect.fnUntraced deliberately... Explain the reason beside the function."
