Review scope and approval: This PR targets main. Review the complete contribution, including the inherited panel registry, device/file/preview/terminal/PR panel refactor and Pi contribution-status contracts, server/provider wiring and web/mobile UI. Maintainer agreement to the plugin subsystem, its process lifecycle and administrative authorization boundary remains outstanding; the current Macroscope assessment requires human approval. This repair does not supply that approval.

Current repair

Published as cc34f24.

  • Fixed the PluginCatalog.ts formatting failure and the mobile status strip's stale AppText import reported by current-head CI.
  • Carried the compatibility repair already tested in the adjacent stack layers: replaced eight removed react-test-renderer suites with React DOM interactions, moved twelve DOM suites into the browser project, removed three manual memoizations, and corrected the terminal test mock's accidental thenable. Device-open completion retains its promise boundary and scope guard.
  • Addressed the filename-digest finding: directory entries are read as raw bytes and rejected before lookup if their UTF-8 decoding cannot round-trip. A raw Linux filename and a valid name containing can no longer alias during digesting. Valid Unicode names remain supported.

Focused verification

An exact-head source archive with existing dependencies was used; no dependency installs, worktree switches or shared checkout changes.

  • Six plugin suites (pluginSource, PluginCatalog, PluginSupervisor, pluginIpcFraming, PluginCatalogRpc, pluginHostChild): 51 passed, 2 Linux-only cases skipped on macOS.
  • Twelve web suites with React Compiler enabled under a temporary jsdom configuration: 50 passed. Mobile contribution-status presentation: 8 passed. Focused React Compiler coverage: 13 passed.
  • Server, web and mobile tsc-rs --noEmit: exit 0. Changed-file lint and format checks pass; lint retains one inherited array-index-key warning in a test stub. Diff whitespace checks pass.
  • The identical digest implementation was previously exercised on Linux for #16048: changing a raw invalid-byte filename's contents left the old digest unchanged; the repaired walk rejected invalid file and directory names and detected changes to a valid Unicode filename. This was a direct production-walk diagnostic, not a Linux Vitest or full plugin-process run.
  • Limits: local Chromium configuration cannot load because the existing dependencies lack @vitest/browser-playwright. The parent also had a Chromium keyboard-focus failure; it has not been shown fixed by these DOM checks. Current-head CI passes Lint, Build and Typecheck, Test, Rust, Release Smoke and all eight server shards, but Test Web and aggregate Check fail. The Chromium log reports only repeated Vitest mock-factory error wrappers; FilesSidePanel.browser.test.tsx and PullRequestSidePanel.browser.test.tsx abort before running tests. The log does not identify the underlying exception or offending mock. A focused reproduction in an equipped Chromium environment remains necessary; the local jsdom passes do not establish browser readiness.
  • Fresh independent review was skipped because the recovery instruction prohibits delegates. The older tests, RPC trace and reviewer attribution below retain their original revisions; they were not rerun as integrated client/process proof for this repair. No new client screenshots or captures were made.

Problem

Users have asked for a way to extend T3 Code with their own code: Ideas #6714 (extension system), #6837 (Pi-style extension API) and #6977 (local app integration SDK). Today a T3 server has no safe place to run such code. Running it inside the server would let one bad plugin stall or crash every thread. Running it without an explicit, byte-exact consent step would let a plugin directory change under the user without them noticing.

This PR adds that place and nothing user-visible yet: a host that runs each consented local plugin in its own supervised child process, and a catalogue an administrator manages over RPC.

This is part 1 of a pair. Nothing in this PR can run a plugin: the host starts a process only when something invokes a plugin, and the first caller of the catalogue's invoke is the next PR in the stack (plugin event delivery). The management UI (web, desktop, mobile) comes in a later stacked PR; until then the catalogue is reachable only through the RPCs below. We recommend reviewing this PR together with the event delivery PR, which shows a plugin launched, an event delivered and its acknowledgement.

Why this qualifies

This is the proposal route in CONTRIBUTING. It needs a maintainer to agree to a plugin system at all, on #6714 / #6837. No maintainer has agreed to this direction yet. We opened it so you can judge the trust boundary in real code before any UI depends on it. It stacks on the Pi status PRs only for ordering; it uses nothing from them. Later stacked PRs add the things plugins can do (events, tools, settings, actions, views) and the management UI. These PRs remain open pending maintainer direction. Previous logical layer: #16046.

Fix

  • Contracts (plugin.ts, pluginCatalog.ts): the t3-plugin.json manifest (id, name, version, apiVersion: 1, entry a relative .js/.mjs path, capabilities, proposedApi), the host state (idle, starting, running, backoff, quarantined, incompatible), installation records with source digest and consent, and an optional plugins environment capability. Clients decode the host state forward-compatibly and treat an unknown value as unknown, never as safe.
  • Supervisor (PluginSupervisor.ts, pluginHostChild.ts): one child process per enabled plugin, started on first use, never eagerly. The child is this same T3 install re-run with a hidden __plugin-host subcommand (dispatched in bin.ts before the CLI graph loads). IPC is NDJSON over fd 3 with a hard per-line byte cap on both sides and Effect Schema decoding on the server. Limits: V8 heap cap (256 MB), minimal environment, activation and per-call timeouts, a concurrency cap, SIGKILL after a cancel grace, capped restart backoff, then quarantine until an explicit resume. Disable revokes the registration at once and returns only after the process has exited; a disable retried after its caller was interrupted waits for that same stop. A plugin whose entry uses top-level await is marked incompatible up front instead of crash-looping.
  • Catalogue (PluginCatalog.ts, pluginSource.ts, migration 064 plugin_installations): an administrator adds an absolute plugin directory. The server reads the manifest and digests every file in the directory (sha256 over sorted relative paths and bytes, hidden files included, symlinks and oversize trees refused). Nothing runs until the administrator consents to that exact digest and enables the installation. The bytes are checked again on refresh, consent, enable, server start and before any call that would start a fresh process. Any change disables the installation and asks for consent again; it is never re-enabled automatically.
  • RPCs (plugins.list, plugins.subscribe, plugins.add, plugins.refresh, plugins.consent, plugins.enable, plugins.disable, plugins.remove, plugins.resume): registered in the RPC group's scope middleware like every other method, with no per-handler checks. Reads need orchestration read. Every mutation needs access:write, the administrative scope that already manages pairing links and sessions; standard pairings never carry it.

Launchers. The child reuses main's resolveSelfInvocation, so there is one spawn path, not one per launcher. It covers the three ways T3 ships a server today: npx t3 (Node, ^22.16 || ^23.11 || >=24.10 per the server's engines), the desktop app (the server runs under Electron's binary, so the child gets ELECTRON_RUN_AS_NODE=1), and the single-executable CLI archive (build:exe, which takes the heap flag through NODE_OPTIONS because it accepts no Node flags on its command line). Every launcher loads the entry with require, so a plugin behaves the same everywhere. No launcher-specific loader was added.

Only what another module imports is exported. The manifest and IPC schemas, the host's error classes and the plugin API's supporting interfaces stay private to their modules; their types are exported only where a caller uses them.

The complete contribution against main includes the inherited panel host and Pi status work as well as the plugin host: 108 files, +11,725/−676 at this repair. The plugin host is the concern introduced by this layer; GitHub currently compares the full contribution against main.

Security

Current review follow-up: CodeRabbit remains paused at 0cd1118, so its retained review does not cover this head. Its storage-failure concern is supported by current source: PluginCatalog.revoke persists the disabled record before unregistering, and a failed save returns an error while leaving the existing registration and process intact. This is the explicit save-first transaction behavior, not a successful-revocation claim or demonstrated execution of changed bytes. Whether runtime authority should instead be revoked when persistence fails remains a maintainer decision; no injected storage-failure test or lifecycle change was made in this follow-up. The current Chromium Test Web/aggregate Check blocker remains as described above.

  • Trust boundary. Plugins are trusted code that runs as the server's OS user. The child process is an availability boundary, not a sandbox: it contains crashes, hangs, V8 heap growth and message floods, not disk, network or subprocess access. The heap cap does not bound external (Buffer) memory or processes the plugin starts. Only an administrator can add, consent to or enable a plugin, and only for a directory on the server's own machine.
  • Digest consent. Consent binds to the exact bytes the administrator was shown. plugins.consent takes the digest the user saw and fails if the bytes have changed since. A digest describes the directory at the moment it was read. The directory's owner can still change it between a check and the load, and code loaded from outside the directory is not covered. The digest pins what the user agreed to run; it is not an integrity boundary against the directory's owner.
  • Scopes. Standard pairings (including remote and tunnel clients paired normally) can list and watch the catalogue but cannot change what runs. Tests below call every mutation through the real scope middleware and show a standard client is refused before the handler runs.
  • Containment. The child gets only PATH, HOME, temp and locale variables. Server credentials and provider keys in the server's environment are not passed. Its working directory is the plugin directory, and its entry must stay inside that directory after symlinks are resolved.

Evidence

Environment: macOS arm64; this PR on top of the previous PR in this stack.

How to exercise it: no client calls these RPCs yet, so review goes through the RPCs. With an isolated dev server (vp run dev) and an administrative session:

  1. plugins.add { directory } on a directory containing t3-plugin.json and its entry. The installation appears with its digest and status needs-consent. A standard-scope client gets EnvironmentAuthorizationError { requiredScope: "access:write" }.
  2. plugins.consent { installationId, digest }, then plugins.enable. plugins.subscribe emits a fresh snapshot after each change, and the host state is idle: nothing has started.
  3. Edit a file in the directory and call plugins.refresh. The installation is disabled and needs consent again.
  4. plugins.disable and plugins.remove. The directory is left untouched.

No RPC in this PR invokes a plugin handler; the capability PRs above add those callers. Process launch, crash, restart backoff, quarantine, resume and kill-on-disable are therefore proven with real child processes in the tests, not through a client.

Live RPC trace (run on an earlier revision of this PR with an identical patch, before the stack was restacked; isolated server on a fresh home, macOS 26 arm64, Node 24; an administrative session from t3 auth session issue and a standard client paired with t3 auth pairing create; plugin fixtures are two directories with a manifest and a one-line main.js):

Parent (Pi status mobile PR) This PR
server.getConfig → capabilities.plugins absent true
plugins.list / plugins.add / plugins.enable Die: Unknown request tag: plugins.list (same for every plugins method) served
Standard client n/a list and subscribe allowed; add, refresh, consent, enable, disable, resume, remove each refused with EnvironmentAuthorizationError requiredScope=access:write
Administrative client n/a every method allowed

What the trace shows, in order:

  1. plugins.add returns needs-consent with the digest. Adding it again, a relative path, enabling before consent, and consenting to a different digest are each refused with a typed reason (already-added, invalid-directory, consent-required, source-changed).
  2. consent → disabled; enable → enabled, generation 1, host idle. No plugin process exists (0 children of the server).
  3. A byte is appended to the plugin's main.js while it is enabled; plugins.refresh turns it into needs-consent (new digest, disabled). enable is refused until consent is given to the new digest.
  4. disable → enable moves to a new generation. plugins.subscribe emitted a fresh snapshot after every change.
  5. A second plugin is enabled, then the server is stopped and that plugin's file is edited while it is down. After restart, the unchanged plugin is enabled again lazily (host idle, 0 processes); the changed one is needs-consent and enable is refused.
  6. remove (refused for the standard client) forgets both records and leaves both directories on disk.

Remote pass (vp run dev --share, same isolated home; a scripted RPC client, not a browser, since no client calls these RPCs yet, reached the server only through the tailnet HTTPS origin via the dev proxy): the server's startup pairing token exchanged for administrative scopes (… access:read access:write relay:write) and a standard pairing token for standard scopes. The remote administrator listed, disabled, enabled, added, consented, enabled, refreshed and removed. The remote standard client could list and subscribe, and all seven mutations were refused with requiredScope=access:write.

Nothing in this PR starts a plugin process, so the trace ends at idle. The event delivery PR's trace shows the same host launching a plugin through production wiring, a crash and restart, quarantine and resume, and an interrupted plugins.disable whose retry waits for the process to exit.

Historical checks at 569feb96e7, re-run 2026-10-05 (CI=true, all exit 0):

  • vp test run on 10 focused files (contracts manifest and catalogue schemas, IPC framing, source digest, supervisor with real child processes, catalogue with real SQLite and children, plugin RPC scopes through the real middleware, RPC scope table, migration manifests): 10 files, 69 tests pass. Recorded on an earlier revision with an identical patch: with this PR's production files reverted to its parent, 9 of the 10 files fail: seven cannot load (missing plugin modules and contracts) and the two migration suites fail without the plugin installation migration. The unchanged RpcAuthorization.test.ts passes on both.
  • A disable retried after its first caller was interrupted stays pending while the old process is alive and returns only after it exits (supervisor with a real child, catalogue with a held stop). Both tests fail on the version of this PR without the fix.
  • The supervisor and catalogue tests wait on state events, snapshots and process exits, and advance time with TestClock. None sleep.
  • PluginCatalogRpc.test.ts serves the nine methods through WsRpcGroup and rpcScopeAuthorizationLayer. A standard client can list and subscribe; all seven mutations are refused with requiredScope: access:write and their handlers never run. An administrative client reaches every handler. A relay-only client cannot read. Registering a mutation at orchestration read makes the test fail.
  • vp run --filter typecheck for @t3tools/contracts and t3; vp lint --report-unused-disable-directives and vp fmt --check on the touched files; vp run knip:check; web build; vp run build:desktop; node scripts/release-smoke.ts. All pass.

Surfaces

  • Entry points: none in this PR. RPC only; the management UI arrives in a later stacked PR. No settings, command palette or keybinding yet.
  • Clients: web, desktop and mobile do not call these methods yet. Old clients never call them; new clients must check the plugins capability.
  • Providers: not applicable. Plugins do not touch provider sessions in this PR. Codex, Claude, Cursor, Grok, OpenCode, Antigravity and Pi are unchanged.
  • Contracts: new schemas, nine RPCs and an optional plugins capability. New client with an old server: no capability, no calls. Old client with a new server: never calls the methods.
  • Reverse states: add ↔ remove, enable ↔ disable, consent is revoked by any byte change, quarantine and incompatibility are cleared by plugins.resume. Remove forgets the record and never deletes the directory.
  • Connection modes: the methods use the normal WebSocket RPC path and scope check, so local, direct remote, relay and tunnel behave the same. Directory paths always refer to the server's machine, not the client's.
  • Docs: no user doc yet, because users cannot reach the feature. The management UI PR adds the user guide.

Not verified

  • No handler invocation at this head: no RPC in this PR invokes plugins. Here, launch, crash, restart and kill are covered only by tests with real child processes; the event delivery PR shows them in a live server.
  • Launchers were run on macOS arm64 only (Node, Electron run-as-node, single executable). Windows (fd 3 as a socket) and Linux were not run.
  • The remote pass used the tailnet (--share) only. No relay or T3 Connect tunnel run; the methods add no transport.
  • The interrupted-disable retry cannot be reproduced over RPC at this head, because no process ever starts here. It is covered by tests with a real child, and over RPC in the event delivery PR's trace.
  • No client in this PR calls the plugin RPCs, so there is no client-side capability gate yet. It is implemented and tested with its first client; no old-server run was done here.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to 0cd11

The main permission and thread-isolation controls are preserved, but plugins execute with the server account’s authority. A storage failure can prevent runtime revocation, and approval checks do not make writable plugin files immutable.

Retained concerns

  • Medium · security · inferred: Runtime revocation depends on a successful database write. If saving the disabled record fails during disable or detected source-change handling, unregistering is not reached, leaving the previous registration and any running child intact. An already-running child remains eligible for invocation without another digest check. The operation reports failure rather than falsely reporting success, and the executing code may still be the previously approved version; nevertheless, storage failure can prevent withdrawal of its server-user authority.
Security review details

Security Blast Radius

  • inferred — A running plugin’s effective scope is the server OS account, not its associated thread or declared capability list. Consequently, exposure can include account-accessible workspaces and stored credentials. Restricting inherited environment variables reduces direct inheritance but does not provide filesystem or network isolation; wider tenant and deployment exposure was not established.

Security Findings and Attack Paths

  • inferred — The supported failure path is conditional: an installation is registered, an administrator requests revocation or reinspection detects changed source, and saving disabled state fails. Because unregistering follows that write, any existing child can retain authority. This does not establish that altered bytes execute, that the failed operation reports success, or that an unprivileged remote caller can activate a plugin.

Trust Boundaries and Controls

  • observed — The newly introduced code-execution management boundary requires administrative access-write rather than ordinary environment-maintenance authority. The connection’s session scopes are checked centrally before its handlers run.
  • observed — The panel host carries a scoped thread identity. Annotation sends compare the originating thread key with the latest committed sender and drop mismatches. The terminal-panel extraction retains ChatView-owned callbacks, scoped session lookup, and the existing terminal-operation permission gate.

Resilience and Maintainability Implications

  • observed — The process boundary supplies failure containment through heap and IPC limits, activation and call deadlines, cancellation grace, restart backoff, and quarantine. These controls protect availability but are explicitly not a security sandbox for server data.

Hardening Proposals

  • proposed — Separate immediate runtime revocation from durable persistence recovery, so a failed write can block further execution while still exposing the unresolved durable-state error.
  • proposed — If approval is intended to guarantee the exact bytes subsequently executed, use a verified immutable installation snapshot. This would strengthen the current trusted-writable-directory model rather than repair a demonstrated sandbox escape.

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR introduces a substantial production plugin runtime with persistence, authentication, supervised child processes, new RPCs, and default-enabled cross-client status behavior, alongside a broad panel refactor. Its runtime and operational blast radius, including an unresolved shutdown cleanup concern, requires human review.

59-70: 📐 Maintainability & Code Quality | 🔵 Trivial | ⚡ Quick win

Replace the standalone ContributionStatusStoreShape interface with ContributionStatusStore["Service"].

The service module defines a separate ContributionStatusStoreShape interface. It then passes that interface to Context.Reference. The repository rule requires the service interface to be inline in the tag and referenced as Foo["Service"]. These consumers use the shape name today:

  • PiAdapterV2Options.statusStore
  • subscriptionStream
  • the openSource annotation
  • makeGatedStatusStore and serveStatus in the tests

Move the interface members into the Context.Reference<...> type argument. Then replace each ContributionStatusStoreShape reference with ContributionStatusStore.ContributionStatusStore["Service"].

-export interface ContributionStatusStoreShape {

-  readonly subscribe: Effect.Effect<
-    SnapshotSubscription<ContributionStatusSnapshot>,
-    never,

-}
+export class ContributionStatusStore extends Context.Reference<{

+    SnapshotSubscription<ContributionStatusSnapshot>,
+    never,
+    Scope.Scope

+}>("t3/contributions/ContributionStatusStore", { defaultValue: () => ({ /* … */ }) }) {}

As per coding guidelines: "Interface. No standalone FooShape; name the type Foo["Service"]."

113-124: 📐 Maintainability & Code Quality | 🔵 Trivial | ⚡ Quick win

Keep the underlying filesystem error as cause on PluginSourceError.

The catch callback turns any read failure into a fixed reason and then discards the original error. A failure other than ELOOP (for example EACCES or EMFILE) becomes "a file could not be read." with no trace of the real cause. The catalog then persists that text as problem, and an operator cannot find out why the inspection failed.

Add an optional cause: Schema.optional(Schema.Defect) field to PluginSourceError, and pass the caught error into it. Keep reason bounded, as it is now.

♻️ Proposed change
 class PluginSourceError extends Schema.TaggedError<PluginSourceError>()("PluginSourceError", {
   directory: Schema.String,
   reason: Schema.String,
+  cause: Schema.optional(Schema.Defect),
 }) {
@@
     catch: (cause) =>
       new PluginSourceError({
         directory,
         reason: ...,
+        ...(cause instanceof Refusal ? {} : { cause }),
       }),

As per coding guidelines: "An error that wraps a failure keeps the immediate underlying error as cause" (docs/internals/effect-services.md).

171-187: 🩺 Stability & Availability | 🔵 Trivial | 💤 Low value

Keep one malformed host line from crashing the child.

receive calls JSON.parse without a guard. The decoder calls receive synchronously from the socket data handler. A SyntaxError therefore escapes as an uncaught exception, and the child exits with no Failed or ActivationFailed message. The server bounds and encodes what it sends, so this happens only if the stream is corrupt. Even then, the child should end deliberately, as the overflow path does. Catch the parse error and exit with status 1.

Proposed fix

+    try {

🟡 Medium plugins/PluginSupervisor.ts:844

Supervisor shutdown leaves the plugin's IPC and stderr streams open when a subprocess inherits stderr: disable waits only for child.exited, then Scope.close(fibers) interrupts the reader before its 250 ms drain timer destroys the streams. Because onExit has already removed the child from children, the fallback cleanup misses it. Ensure the reader destroys its streams on interruption, or wait for draining to finish before closing fibers.
