Claude kept its process after authentication failed, so logging in again could leave an existing thread using stale credentials. A turn that fails authentication now marks that process for replacement. The next user turn starts a fresh process and resumes the same conversation; ordinary server errors and recognized HTTP 403 permission errors continue reusing the process. Sign-in guidance now tells the user to send the message again.

Continues @robertnisipeanu's work in #18558. Main was merged while preserving the contributor's commits. The takeover fixed a missing guard in ordinary process reuse after main's live-model refactor. It also corrected the recorded replay expectation and retires old replay iterators when the next query opens, preserving trailing interrupt frames while keeping replacement frames with the new query.

Verification:

  • Acceptance tests: 294 before → 295 after, adding one 403 row to the existing parameterized case. Main and the initial merged implementation fail authentication replacement; ordinary server-error reuse passes. The final adapter/ClaudeHome suites pass all 294 cases (289 + 5).
  • All 34 existing Claude replay scenarios pass, including recovery, resume and interrupt cases. Total expanded verification: 328 distinct passing cases. The 403 case fails before the permission exclusion; removing the authentication reuse guards still breaks the recovery replay. Before/after proof.
  • Scoped lint, formatting and server typecheck pass. Two existing unused-declaration warnings remain; typecheck emits Effect suggestions only. One existing foreground-subagent case timed out in the initial baseline and passed subsequent full runs; its cause remains undiagnosed.
  • Two parallel adversarial review rounds: round one found the 403 classification issue and four replay interrupt failures; both are fixed. Round two found no code blockers; Claude independently reran all 328 passing cases.

The tests execute the real adapter with injected SDK sessions and a recorded transcript. No fresh live expired-login or credential-refresh flow was exercised; the contributor's historical live CLI evidence was not repeated. No screenshot demonstrates this process-cache behavior. macOS, Windows, iOS, Android, Electron desktop and remote/relay connections were not tested live.

The existing background-work guard can delay replacement until shells/subagents finish or the user presses Stop. Continuation turns drain already-produced output; the next actual user prompt replaces the signed-out process. Configured API-key changes remain separate from subscription login recovery.

Implementation and coordination: GPT-6.1 Sol through Codex in T3 Code. Tests: GPT-6 Astra through Codex. Reviews: Claude Opus 5.5 through Claude Code and GPT-6 Sol through Codex.

The Claude CLI reads its login once per process, and a thread keeps one
process alive between turns. After a turn fails to authenticate, mark
that process signed out so the next turn replaces it with a fresh one
that resumes the same conversation. The sign-in error tells the user to
send the message again instead of starting a new thread.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This fix changes production authentication handling by replacing stale Claude processes and resuming conversations after re-login, rather than only changing tests or copy. The behavior is narrowly tested, but its credential and process-lifecycle implications warrant human review.

Walkthrough

After a Claude authentication failure, the adapter marks the live query signed out and opens a new process for the next turn. The new process resumes the existing native session. Subscription-login guidance now tells users to send the message again.

The adapter marks a live query signed out after an authentication failure and does not reuse that process for the next turn. Tests verify that a fresh process resumes the native session, while a server error leaves the existing process in use.

Merge Risk: 🟡 Moderate · up to 3456c

A result-only authentication failure could leave the same thread using an expired process after login. Handle that result before merging unless this remaining recovery gap is accepted.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 3456c

Recovery preserves the existing conversation and access settings. No introduced privilege expansion was identified, but ownership guarantees during overlapping recovery attempts are not fully established.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • observed — The changed recovery branch acts on the adapter-owned live process for the existing native conversation. Replacement receives the same application thread and provider-session identifiers, configured instance environment and workspace inputs; the change adds no authority-bearing input.

Trust Boundaries and Controls

  • observed — Replacement uses the existing runtime-policy derivation, MCP tool configuration, permission mode and approval callbacks. Incoming frames from a no-longer-current query are rejected, and stream-exit cleanup checks query-object ownership before clearing or finalizing replacement state. These controls predate the PR and remain in place.

Resilience and Maintainability Implications

  • inferred — Ordinary queued starts have meaningful serialization and stale-attempt controls: queue progression rejects blocking runs, and startup conditionally advances the current starting attempt before reaching the provider. These reduce the concurrent-replacement concern, but do not establish complete exclusion of overlapping distinct recovery starts. The adapter's active-turn guard alone is not an atomic reservation around opening and installing a query.

Hardening Proposals

  • proposed — Establish an explicit single-owner or idempotent replacement guarantee for overlapping stale and recovery starts, including interrupted opening. This would close the remaining process-ownership proof gap; it is not an observed PR vulnerability.

Explanation

Issue #18554 requires same-thread recovery after a successful Claude re-login. The diff marks a live query signedOut when a turn has status failed and an authentication failure message. openQuery excludes signed-out queries from reuse. The replacement path opens a new process while preserving the native thread, as verified by the regression test with resume set to WAKE_NATIVE_SESSION. The test also verifies that server_error reuses the process. The signed-in guidance now tells the user to send the message again.

Full details: Out of Scope Changes check

Explanation

The changes remain within issue #18554. The adapter flag and reuse condition implement process replacement. The parametrized test covers authentication failure and non-authentication failure behavior. The Claude sign-in message and its test update the user guidance for the reported recovery flow. No unrelated files or behavior changes appear in the diff.

Explanation

The description clearly explains the problem, implementation, verification results, limitations, and agent usage. However, it does not provide the required Scope and approval information, and it does not follow the template section headings. It references PR #18558 but does not link the triaged issue or document explicit maintainer approval or an exemption rationale.

Resolution

Use the required Problem, Change, Scope and approval, and Verification headings. Add a link to the triaged issue, such as #18554, or document the explicit maintainer approval and scope. If no prior issue or approval exists, explain why this focused fix qualifies for the exemption.

Taking over this one as maintainer for batch claude-03. I will preserve your commits, merge current main, and run separate minimal-test, implementation, and adversarial-review passes before pushing. No merge planned.
