fix(server): keep follow-up tasks pending until nested work finishes - #21877
A follow-up sent to a delegated task finished as soon as its run ended, even while a nested delegate_task from that run was still running. The parent received the interim reply as the task result. When the nested result later woke the child, the continuation run had no task ID, so its final reply never reached the parent.
Change
Follow-up publication, task_status and delegatedTaskResultPending now use the readiness check the original task already uses (delegatedTaskProgress), scoped to the follow-up's runs and the nested tasks they started. Like the original task, a follow-up also waits for provider background work on the child thread. A follow-up that fails or is cancelled before it starts still settles at once.
A nested-result wake now runs under the task ID of the follow-up that started the nested work, while that follow-up has no result. The wake continues the same unfinished follow-up. A new task would leave the first one reporting the interim reply. Wakes for the original task stay untagged and keep their existing routing, because the original task already waits on the whole thread.
A follow-up's wake steers only into a run of that follow-up. If another follow-up is running, the wake queues behind it, so the nested result does not end up in the other follow-up's reply. Follow-up intake and dispatch locking are unchanged.
Real run of the issue's steps on an isolated Linux dev server, with a Codex GPT-6.1-Sol writer and nested checker. A script drove the server over RPC instead of the browser UI, and the Codex agents called T3's MCP tools.
On main at f34ba437a5, the follow-up published the interim reply while the checker ran. The continuation produced the final reply without a task ID, and no result reached the parent.
With the fix at b539214a59, task_status reported waiting_for_children while the checker ran. The final reply reached the parent once, and the original task's result stayed unchanged. The last commit, d267a51ff6, changes only the check for follow-ups that never started, and the automated tests below cover it.
Not run: other providers, a Stop during nested work, and the browser UI. Only automated tests cover a nested result that arrives while another follow-up is running, and a follow-up whose start fails.
With apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/mcp/OrchestratorMcpService.ts reverted to 066c8fc0cd, 3 tests fail with assertion errors: "OrchestratorMcpService > retries terminal acknowledgement with a fresh command id (follow-up: true)", "subagent follow-ups > delivers a nested follow-up independently of the original task", "subagent follow-ups > delivers a nested-active follow-up independently of the original task".
Separate tests cover a follow-up whose queued start fails (it settles as failed) and one still queued (it stays working). Both also pass on main. They keep the new readiness checks from blocking these cases.
vp fmt --check and vp lint report no errors and no new warnings on the 4 changed TS files.
Typecheck passes for t3.

A provider-native child task can remain active while a later follow-up publishes early, potentially delivering an incomplete result to the parent. Fix the shared run-scoping before merging.
Security architecture risk: Low · up to c1d24
The change keeps nested work attached to the correct pending task and prevents its result from steering an unrelated follow-up. No new access boundary or privilege expansion was established. Concurrent cancellation and rollback termination remain incompletely validated.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The inspected ownership change affects a delegated child thread, its nested work and its pending task in the parent thread. It changes attribution and timing of provider continuation work; the inspected changes do not establish a new credential, sandbox or cross-tenant authority path.
Trust Boundaries and Controls
observed — Delegated-completion dispatch still requires a server-created queued message and validates the durable delivery’s open disposition, generation and message identity. Wake text is derived from validated task IDs rather than accepting the supplied message text. Public task-status invocation still requires a thread scope.
Resilience and Maintainability Implications
inferred — The parent eligibility lookup is not atomic with child continuation dispatch: server-created wakes take the child lock, while user and MCP follow-up intake also take the parent lock. Existing cancellation separately stops the child and its delegated work, so the lookup race alone does not demonstrate a new cancellation bypass. Its complete interaction with concurrent disposal remains unproven.

Explanation
[#21837] The PR keeps follow-up tasks pending while scoped nested work remains active. It assigns the follow-up task ID to the nested-result continuation and routes the wake to that follow-up. The original task result remains unchanged. The expanded tests cover nested work, a competing follow-up, final delivery, pre-start failure, and queued work. The reported regression tests fail when the implementation is reverted.

The changed production files implement [#21837] follow-up readiness, task status reporting, nested-result ownership, and wake routing. The changed tests exercise these lifecycle cases. The test session changes support the delegated-completion tests. No unrelated production behavior or unrelated file change is demonstrated.

Thanks for this. While looking into #16603 (its 2026-10-20 report is a follow-up sent with t3_thread_send whose child then started a native Claude background subagent), I read this PR against main. I think it covers that report when the native subagent starts in the follow-up's own run: the Claude adapter writes the provider_native row with runId set to that run, so the runId-scoped delegatedTaskProgress should hold the task until the subagent ends.
Two small gaps from reading the diff at 3dda05ac50 (I have not run it):
The new follow-up tests use app-owned nested tasks only. A case with a Claude-style provider_native row (running, same runId as the follow-up run), then subagent.updated to completed, would pin the reported shape and check that the result is delivered once.
The filter taskRuns.some((run) => run.id === subagent.runId) drops a native subagent that started in an earlier run of the same child thread and is still running when the follow-up ends. In that case only the provider's background roster would still hold the task, and only if the provider reports it there.
Happy to send these as a small follow-up after this lands, or you may prefer to fold them in here.

Thanks for checking this against #16603. I read both points in source at 3dda05ac50 and did not run anything new.
Right. The follow-up tests use app-owned nested tasks only. The one provider_native row in DelegatedCompletionDelivery.test.ts predates this PR and belongs to an original task.
Right. The filter keeps every run of the follow-up's own task, so it drops a native subagent started under the original task or another follow-up on the same child thread. The roster will not cover that for Claude: the adapter lists only local_bash tasks there. On main a follow-up waits for no nested work at all, so this PR narrows the gap without closing that case.
Julius's verification of this head is still pending, so I'd like to keep it unchanged. A follow-up from you for both after this lands would be welcome.
Checked against this PR's head 771e855fc8: with it applied, a follow-up that starts a background command (run_in_background) is now held while the command runs, but it still completes with its interim reply once the command's wake run starts.
To help repro that, two focused tests for DelegatedCompletionDelivery.test.ts, with results on main and on this PR, are in #21837 (comment). Happy to adjust them if they'd be useful here.

apps/server/src/orchestration-v2/Orchestrator.ts:13507-13535 Major · Functional Correctness · Heavy lift
Provider-native subagents bound to an earlier run are excluded from the follow-up publication gate.
The task-scoped filter keeps only subagents whose runId belongs to the follow-up's own runs. A provider-native subagent bound to an earlier run of the same child thread is dropped from the check. A later terminal follow-up can then publish its result while that native work is still active.
The same filter appears in two places:
finalizeAppOwnedSubagentTask, which decides publication (Orchestrator.ts:12097-12108).
delegatedTaskResultPending, used by MCP status reads (Orchestrator.ts:13518-13528).
A fix limited to delegatedTaskResultPending does not change publication. The publication gate has its own copy of the filter. The MCP task_status path also builds its own run-scoped subagent list (OrchestratorMcpService.ts:1477-1505, per prior inspection). The same change is needed at each site. Count non-terminal provider-native subagents of the child thread, not only those in the task's runs. The change must also settle or exclude stale rows of ended runs. Otherwise the gate can block publication indefinitely.
