Problem

Agents that drive T3 Code from outside through /mcp (ChatGPT, cron-woken bots, Grok Bot agents) start a thread and then their own turn ends. To learn that the thread finished, failed, or is waiting on someone, they either hold t3_thread_wait open (up to an hour, with no keepalive, fragile through tunnels) or poll. The workaround people actually run is a watcher outside T3 Code that wakes the agent for every thread, including ones it never started.

Discussions: #19534 (MCP Events in T3's MCP server), #6933 (outbound completion webhook). Related: #15975 (Home watches threads for its own agent), #18490 (MCP protocol negotiation).

Change

An outside agent can now ask T3 Code to call it back for a specific thread:

  • t3_thread_watch { action: "watch" | "unwatch", threadId?, delivery: { url, secret | secretRef } } returns { threadId, watching, wasWatching }, like watch_pull_request. Unwatch needs only the URL.
  • t3_thread_launch takes an optional watch. ThreadWatchReactor.watchLaunch registers it before the thread exists, so its first run is never missed, and a launch that fails or is interrupted before creating its thread ends the watch.
  • T3 Code POSTs to the URL when the watched thread's run completes or fails, when it asks a question (including request_secret), when it waits for an approval, and once more with ended when the thread is settled, archived or deleted. A run that ends as its thread settles is sent before ended.

Design

  • ThreadWatchReactor sits beside PullRequestWatchReactor and works like AgentAwarenessRelay: it subscribes to domain events with the relay's filter, plus request_secret turn items (they show as pending input but that filter skips them), and coalesces per thread with the relay's worker.
  • It decides from the thread shell with the phases mobile alerts use (resolveThreadAwarenessPhaseV2, now exported). The kinds and keys follow Home's watches in feat(home): add Home, an agent that manages threads across your machines #15975: completed, failed, question, approval, ended, with attention keyed by run and request so a second question in one run is its own event.
  • Registrations and what each watch was last told live in memory, as in the relay. Only the registration and its secret are stored, in thread_watches (migration 068), so the secret never reaches the event log or the read model.
  • A watch starts from the thread as it is when it is made; a watch made before its thread exists compares with nothing. After a restart each watch starts again from its thread as it is then. Settled, archived and subagent threads cannot be watched.
  • Deliveries are forked, so a slow endpoint never delays another thread or orchestration. Failed requests are retried three times with exponential backoff from 2 s, like HeldHooksWaker, keeping the same webhook-id. A watch that is unwatched or replaced drops its pending retries, and registration changes take turns so a late 410 cannot delete a newer watch's row.

Request

POST <url>
content-type: application/json
webhook-id: evt_<32 hex> (one per change, same on retries)
webhook-timestamp: <unix seconds>
webhook-signature: v1,<base64 HMAC-SHA256(key, id.timestamp.body)>
x-mcp-subscription-id: tw_<32 hex> (one per thread and URL)

{"eventId":"evt_…","name":"thread.watch","timestamp":"<ISO>",
 "data":{"environmentId","threadId","title",
         "kind":"completed|failed|question|approval|ended","runId"},
 "cursor":null}

The body follows the MCP Events webhook shape and Standard Webhooks signing, so an off-the-shelf verifier works and receivers stay unchanged if thread.watch is later exposed through events/subscribe. That needs MCP 2026-07-28 and custom JSON-RPC methods that McpServer in effect 4.0.5 cannot register yet (#18490 covers 2025-11-25).

Security

  • The secret is a client-supplied whsec_ secret (base64 of 24 to 64 bytes), as MCP Events requires, or a secretRef from request_secret so it never enters an agent transcript.
  • https only (http only for localhost), no credentials or fragment in the URL, redirects not followed, request tracing off so URLs stay out of spans. Bodies carry status only, never thread content.
  • The tool goes through McpToolAccess.writesThreads: read-only clients are refused and the caller's mode limits apply. Results never list other endpoints, and unwatching needs the URL.

Known limits

Shared with desktop and mobile alerts: the shell shows only the latest run, so a completion immediately followed by a queued run is reported with the next completion; a run someone stops sends nothing; changes made while the server is down are not replayed. Auto-settling an inactive thread ends its watches, as it ends pull request watches.

Scope and approval

One problem: outside agents cannot hear that a thread they started needs them. This is a new feature, and #19534 has no maintainer reply yet, so I'm opening it to make that discussion concrete. I'm happy to reshape or close it if you want a different direction.

Decisions I left for you:

  • A watch can target any thread the caller may write to, not only threads it launched. Support MCP Events in T3's MCP server #19534 asks which it should be; narrowing it is a small change.
  • Delivery runs on the environment, not the relay, so a machine that is asleep sends nothing and changes made while it is down are not replayed.
  • Watches are not tied to the MCP client that made them, so revoking a client does not end its watches (the user guide says so). Binding them to the client would need an ownership and revocation lookup the server does not have today.
  • Endpoints are not filtered by resolved address. Callers already need thread-write access, the body is fixed by the server, and responses are never returned to the caller. The checks in htmlRender/publicProxy.ts could be extracted if you want them.

#22554 also adds migration 068; whichever lands second renumbers.

No UI changes beyond the label every MCP tool needs in t3McpToolPresentation (the work log shows "Updated thread watches"), which core.test.ts requires.

Verification

Run on the PR head (d9a75ab428) against main 569bd029ea:

vp test run apps/server/src/orchestration-v2/threadWatch.test.ts \
  apps/server/src/orchestration-v2/ThreadWatchReactor.test.ts \
  apps/server/src/mcp/toolkits/core.test.ts apps/server/src/mcp/toolkits/project/handlers.test.ts \
  apps/server/src/persistence/Migrations.test.ts apps/server/src/orchestration-v2/runtimeLayer.test.ts \
  apps/server/src/mcp/McpHttpServer.test.ts apps/server/src/relay/AgentAwarenessRelay.test.ts \
  apps/server/src/mcp/OrchestratorMcpToolkit.integration.test.ts \
  packages/shared/src/t3McpToolPresentation.test.ts packages/client-runtime/src/t3ToolSummary.test.ts \
  packages/client-runtime/src/work-log/presentation.test.ts

Result: 12 files, 185 tests passed. Typecheck clean for apps/server, packages/shared and packages/client-runtime; lint and format clean on the changed files.

The new tests verify request signatures independently of the sender and cover:

  • threadWatch.test.ts: each change is reported once; a second question in one run gets its own event id; the Standard Webhooks reference vector; secret format bounds.
  • ThreadWatchReactor.test.ts: one request per change from the watch on; a change made while a watch is being registered is still sent; a launch is watched before its thread exists, and a launch that fails or is interrupted ends the watch; restart baseline; domain events including a request_secret wait; settling sends what changed, then ended, and a thread watched again can end again with a new event id; 410 ends the watch; retries keep the event id and unwatching drops them; a late 410 leaves a replacement watch; a hanging endpoint does not delay another thread; refusals; one watch per URL with a secretRef.

Each guard was checked by breaking it: removing the stale-retry check, the guarded delete, the request_secret filter, the startup baseline, the end after ended, the 410 handling, the ended-thread refusal, the first-run handling for watches made before their thread, the per-request event id, the post-registration check (and running it for a not-yet-launched thread), the launch cleanup, reporting a completion before ended, the time in the ended id, or the forked delivery each fails the intended test.

Not checked: an end-to-end run against a live server and a real receiver, and knip, which ran out of memory under my local resource limits.

Agents that drive T3 Code through /mcp end their turn before the threads they
start, and today learn that a thread finished or is waiting only by holding
t3_thread_wait open or polling. Add t3_thread_watch (watch/unwatch) and a
watch option on t3_thread_launch: T3 Code POSTs a Standard Webhooks request in
the MCP Events body shape when a watched thread's run completes or fails, asks
a question, waits for an approval, or ends. A reactor beside the awareness
relay decides from the same phases mobile alerts use.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR introduces a substantial production thread-watch workflow with persistent signing secrets, outbound signed webhooks, retries, lifecycle handling, and new MCP surface area. Its runtime and security-sensitive integration require human review, and an unresolved race can lose notifications during watch registration.

Needs human approval because:

Watching reads the thread's baseline, then consumes the secretRef and stores
the row before the watch is live, so a run that ended in between was never
sent if no later event followed. Check the thread once the watch is live,
except for a watch made before its thread exists, which has no baseline to
miss and would otherwise end itself.

Merge Risk: 🟡 Moderate · up to 08a36

Agents may miss a completion or failure notification when a thread ends in the same observation, and receivers may discard an ended notification as a duplicate. Resolve or explicitly accept these gaps before merging.

Security architecture risk: 🟡 Moderate · up to d9a75

Thread-write access now permits server-side callbacks, including to local services, without separate destination authorization. Authentication, signed payloads, manual redirects, and bounded retries reduce exposure, but do not constrain the initial destination. Callback ownership and restart behavior also warrant explicit operational consideration.

Retained concerns

  • Medium · security · inferred: The new callback path delegates server-network POST authority to thread-write callers, including approval-required callers. They can select localhost HTTP endpoints or unrestricted HTTPS destinations and induce delivery through thread transitions, without a separate destination approval. Redirect blocking and HMAC signing do not constrain the initial target. This creates a blind server-side request-forgery and local-service interaction risk; actual downstream impact depends on reachable services accepting the fixed callback request.
Security review details

Security Blast Radius

  • inferred — The independently attackable scope is the hosting environment's reachable callback destinations and metadata for registered threads. Loopback HTTP is directly permitted; private-network HTTPS reachability depends on routing and normal TLS validation. No cross-environment compromise, arbitrary response disclosure, or cloud/IAM privilege gain was established.

Security Findings and Attack Paths

  • inferred — An authorized thread-write caller can choose a local callback URL and valid signing key, register a watch, and induce a qualifying thread transition. The server then POSTs fixed JSON to that destination without a separate network-authority check. This is a constrained blind-request path, not evidence of arbitrary request bodies or successful exploitation of an internal service.

Trust Boundaries and Controls

  • observed — Registration rejects read-only clients and checks live thread callers and target mode ceilings. Delivery rejects URL credentials and fragments, disables redirects and URL tracing, signs the payload, and bounds request duration. These controls protect caller access and delivery integrity, but do not authorize the initial network destination.
  • observed — Watch ownership is intentionally thread/URL based rather than client based. Registrations survive client revocation and restart, so credential revocation alone does not stop metadata callbacks. Documentation explicitly discloses this policy; it is not treated as an unintended bypass.

Resilience and Maintainability Implications

  • observed — Retries retain the event ID and recheck registration identity. Unwatch suppresses subsequent retries but does not cancel a request that already passed the identity check. Delivery state is not durable, so callback receipt must not be used as the sole security or approval-state authority.

Hardening Proposals

  • proposed — Separate callback destination authority from thread-write access. Consider environment-approved destinations or privileged opt-in for private and loopback targets, with effective-address enforcement at connection time. Preserve manual redirects, TLS verification, and fixed payloads.
  • proposed — Make environment-owned watches discoverable and revocable during incident response, and clarify that unwatch cannot recall an already-started request. Receivers should verify signatures and freshness, deduplicate event IDs, and re-read authoritative thread state before taking privileged actions.

Explanation

The title "feat(server): tell outside agents when a watched thread needs them" matches the main change: a new thread watch feature (t3_thread_watch tool, ThreadWatchReactor) that sends signed webhook callbacks to outside agents when a watched thread completes, fails, asks a question, waits for approval, or ends. It follows the conventional commit format the repository template asks for. It is specific enough to convey the primary change, though it does not mention the launch-time watch option or the migration.

Full details: Description check

Explanation

The description follows the repository template closely. It has Problem, Change, Scope and approval, and Verification sections. The Problem section states the issue and cites related discussions. The Change section describes the new tool, the launch option, and the webhook behavior, with a request format example. The Scope and approval section names the single problem, states that the feature is new, links #19534 without a maintainer reply, and lists decisions left to reviewers. The Verification section gives the exact test command, the results (12 files, 185 tests), typecheck and lint status, the mutation checks, and what was not checked (live end-to-end run and knip). The description does not link a triaged bug issue or maintainer approval comment, which the template asks for on new features. It also cites an approval exemption path that does not apply to this feature. The template's agent attribution line is present. Overall the description is mostly complete, and the gap in maintainer approval is a process concern that the reviewer should confirm.

…aunch watches in the reactor

- A run that completes or fails as its thread is settled was reported only as
ended, so the completion was lost; this is how an agent that settles its own
thread ends. The rules now report what changed, then ended, and the reactor
sends them in order before ending the watch.
- An ended event's id now includes when it was seen, so a thread watched again
after unsettling can end again without receivers dropping it as a repeat.
- The launch lifecycle moves out of the MCP handler into
ThreadWatchReactor.watchLaunch, which registers the watch and runs the launch
as one step and, when the launch fails or is interrupted, checks the thread
so a watch for a thread that never came to be ends.
- Import effect/http modules from their subpaths, and drop comments that
  restated the code.
