Problem

A Muse thread can lose every T3 tool after the server restarts or its Muse host dies while work is queued. Muse Code 1.4.12, and the 1.4.13 canary, do not start the MCP servers passed in session/resume when the resumed session replays queued work, such as a queued user turn or a background-terminal delivery. That host never gets mcp__t3_code__* tools, and MSP cannot load them later. With mode: "optional" nothing fails visibly. The agent just can't delegate.

Change

The resume ack shows it: session.activeTurnId names the replayed turn. When T3 resumed with MCP config and the ack names an active turn, register() closes that host and resumes on a fresh one, as failure recovery does (bump hostEpoch under eventPermit, then launchHost()). An idle resume starts MCP normally. After five relaunches it logs a warning and continues without T3 tools.

A second resume on the same host does not start MCP, so it takes a new host. The cost is that each relaunch cancels one replayed item, up to five. In the run below, the queued user message stayed in Muse's history and the next run answered it. A queued steer is cancelled the same way, even one the server already counted as delivered before a restart.

Muse leases a session to one live host, so the new host can resume only once the old one is gone. Each host now owns a child scope, and the relaunch closes the replaying host's scope, which runs the existing release: close() bounded at 13 seconds, then the scope unit's reap. A replayed background command can inherit Muse's pipes and keep close() pending after Muse exits. Unbounded, that would hang ensureThread, and interruptTurn behind it, for good. The bound now applies with or without a scope unit. The SDK has sent SIGKILL by 12 seconds, so the lease is free and session teardown can't hang either. The release now checks the host epoch, which the relaunch bumps first, so closing the replaying host leaves the session open.

One gap is left. A host that already failed has a stale epoch, so its release does not set closed when the session closes. The session's finalizer does, after that host's close() returns or hits the 13-second bound. For up to 13 seconds a call into the closing session still sees it open. Setting closed when teardown starts would remove that window. I left it out to keep this change to the resume bug.

The check keys on the ack, not a Muse version, so if Muse fixes resume ordering, T3 still relaunches on a replaying resume until this is removed or gated. I reported the bug to Muse with a standalone repro through /bug.

Scope and approval

No issue. A small fix in the Muse adapter's session open and host release, with three tests. I can open an issue first.

Verification

Linux (Fedora 44), main 9a391eb3f5 plus this branch, vp test run src/server/adapter.test.ts in packages/provider-muse.

  • Main's adapter.ts: 3 failed, 39 passed. The resume test fails at fake.closeCount() (expected 1, got 0).
  • Relaunch with an unbounded close: both stuck-close tests hang to the timeout, 2 failed, 40 passed.
  • This branch: adapter suite 42 passed, package 80 passed, vp run check:changed 9a391eb3f5 passes.

Vitest with main's adapter.ts: the three new tests fail

Vitest on this branch: the adapter suite passes

Real Muse Code 1.4.12, muse-wonder-full-1.0, with production builds of main 66ee628e08 (its packages/provider-muse matches 9a391eb3f5) and this change on it. Two server lifetimes on main left a user message queued behind a background-terminal delivery. The third ran the desktop app of main or this branch. I sent the delegation check from desktop, then opened the thread in web, iOS and Android.

main    resumed host: mcp_startup_audit_fault, no mcp.transport.startup -> NO_DELEGATE
branch WARN Muse replayed queued work without T3 tools; relaunching
branch second host: mcp.transport.startup server="t3-code" outcome="ready"
branch  delegate_task -> CHILD=CHILD_OK

In the standalone repro sent to Muse, run twice per case on 1.4.12 without T3, a queued turn left the resume ack's activeTurnId set to that turn's id, and the new host started no MCP. Idle, it was null and MCP started.

Captures, main then this branch (no client code changes):

Desktop app on main: NO_DELEGATE

Desktop app on this branch: delegate_task and CHILD_OK

Web client on main: NO_DELEGATE

Web client on this branch: CHILD_OK

iOS on main: NO_DELEGATE

iOS on this branch: CHILD_OK

Android on main: NO_DELEGATE

Android on this branch: CHILD_OK

Not checked:

  • macOS and Windows desktop.
  • Physical phones and Release native builds. Simulators ran the Debug native client with production JavaScript.
  • Muse 1.4.13 against this branch. The missing MCP start was confirmed on the 1.4.13 canary without T3.
  • With a real Muse: the five-relaunch limit, or a close that stays pending during a relaunch. Fake-host tests cover one relaunch and the stuck close.
  • The live captures predate the bounded close, which only matters for a slow or stuck close. The live run had none.

Prior PRs: none for this bug. #19195 by @ddonathan added relaunch after a host failure, and this change reuses its launchHost() path.

Muse Code 1.4.12 does not start session MCP servers from session/resume
config when the resumed session replays queued work, so a Muse root loses
T3's tools for that host's life. This test fails until the adapter
relaunches the host on a replaying resume ack.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Muse Code 1.4.12 (and the 1.4.13 canary) never starts the session MCP
servers from session/resume config when the resumed session replays queued
work, a queued T3 turn or a background-terminal delivery. The root then runs
without mcp__t3_code__ tools for that host's life, and MSP has no method to
reload them.

The resume ack says so: session.activeTurnId is set. When T3 resumed with
MCP config and the ack names an active turn, the adapter now drops the
replaying host's events, closes it (Muse cancels the replayed item), and
resumes on a new host, up to five times. Each relaunch drains one queued
item, and an idle resume starts MCP.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
The loop condition read config on every pass, which lint reports as
no-unmodified-loop-condition. Behavior is unchanged.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
The resume relaunch awaited the replaying host's close() with no limit,
inside register under the command permit. A replayed background command
that inherits Muse's pipes keeps that promise pending, so ensureThread
and interruptTurn hung for good.

Each host now owns a child scope, and the relaunch closes that scope.
This runs the host's existing release finalizer: close bounded at 13 s,
then the AgentScope unit reap. The 13 s bound now applies with or
without a unit; the SDK has sent SIGKILL by 12 s, so the session lease
is free. The finalizer ends the session only for the current host epoch.

Walkthrough

The adapter now manages each Muse host in an owned scope and retries MCP-enabled session resumes on fresh hosts when a resumed session reports active work. Tests cover replayed turns and recovery when host shutdown remains pending.

Changes: Muse resume recovery

Layer / File(s) Summary
Host-owned lifecycle and shutdown
packages/provider-muse/src/server/adapter.ts
Each host gets an owned scope for its shutdown finalizer and process-exit observer. Shutdown uses an interruptible 13-second timeout.
Resume retry on fresh hosts
packages/provider-muse/src/server/adapter.ts, packages/provider-muse/src/server/adapter.test.ts
The adapter checks for sessionMcp support during session opening. When MCP-enabled resume reports an active turn, it closes the host and retries on fresh hosts up to five times. Tests cover replayed turns, host reuse, and pending shutdown recovery.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This production Muse adapter change alters resume behavior by closing and relaunching hosts when queued work is replayed, potentially cancelling replayed work and waiting through bounded shutdowns. The lifecycle and concurrency changes are covered by focused tests but warrant human review due to their runtime impact.

Outside diff Recheck sessionMcp for every replacement host.

1828-1903: Minor · Functional Correctness · Quick win

Recheck sessionMcp for every replacement host.

When the initial host reports an active turn, the retry loop launches a replacement and reuses open. The capability check runs only before that loop. A replacement without sessionMcp therefore receives the saved MCP configuration and can resume without T3 tools.

Move the check into open so it runs for the initial host and every replacement.

merged current main without rebasing. the one conflict was the host close path: kept main's typed MuseSdkError and this PR's 13s bounded close.

moved the sessionMcp capability check into open, so every replacement host is checked before it resumes with the saved MCP config (the outside-diff CodeRabbit finding). no other change.
