A stopped Codex app-server leaves new threads waiting indefinitely on thread/start. When the transport exits, the shared session can remain ready and later sends reuse its dead transport.
Change
Expose transport termination to the adapter and retire the shared session through the existing event-stream failure path. Bound thread setup, resume and turn acceptance to two minutes; a timed-out setup fails the run instead of retrying or falling back to a new native thread. Escalate app-server shutdown after two seconds so a stopped process cannot hold release open.
Julius explicitly requested triage and authorized a focused bug-fix PR. Server-only Codex liveness behavior; no client or wire-schema changes.
Verification
vp run check:changed at 4a840995da: passed format, lint, both knip checks, GraphQL, affected-package typechecks and 140 tests in four files. Exact-head output.
Six focused regressions cover unanswered thread/turn starts, transport death, deadline retirement, terminal setup failure and skipping resume fallback after an unknown outcome. The original three fail on main at their bug assertions; the resume guard removal fails directly on fallback invocation. Original red assertions, resume red assertion.
Independent Codex and Claude review round 3: no confirmed code blockers. Review summary.
Unusually slow setup or resuming a very large rollout can exceed the fixed cutoff. The two-minute deadline applies to request acceptance and setup, not subsequent model/tool execution. Retiring the shared app-server interrupts sibling turns using that process. Linux process cleanup was exercised; Windows and macOS process cleanup were not run. The original hours-long natural notification-backlog trigger was not established.

Unanswered thread/start and turn/start never fail, and a terminated
app-server transport never fails the adapter session event stream.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
A stopped app-server left thread/start and turn/start pending forever, and
a killed one stayed "ready" because nothing failed the session's events.
The protocol now exposes a replayable termination, and the adapter fails
its event stream when the transport ends or when thread/start,
thread/resume or turn/start go unanswered for 2 minutes. The session
manager already releases a runtime whose events fail, so the next request
opens a fresh app-server. Unanswered requests fail rather than replay.
Release escalates to SIGKILL so a stopped process cannot block it.

A thread/start or thread/resume the Codex app-server never answered has an
unknown outcome, but the start service re-raised it for the effect worker to
retry on a fresh app-server, which could run the turn twice. Settle the run as
failed instead.

…d resume
A thread/resume that times out may still have resumed the native thread, and
the adapter has already retired the shared runtime, so the fresh-thread
fallback hit a closing transport and the worker retried. Settle the unknown
resume outcome as a failed run before attempting the fallback.

Macroscope's review found this PR approvable — This is a localized Codex resilience bug fix that bounds setup/turn requests, surfaces transport death, and retires wedged shared app-servers. Added tests cover timeout, retirement, transport failure, and prevention of unsafe retries, while normal successful paths remain unchanged.

The Codex app-server client exposes transport termination. The adapter applies deadlines to selected requests and fails its event stream on timeout or transport exit. Provider turn startup treats unresponsive thread operations as unknown outcomes and settles affected runs as failed.
Changes: Codex app-server failure handling

Summary
Expose transport termination packages/effect-codex-app-server/src/protocol.ts, packages/effect-codex-app-server/src/client.ts
The protocol and client expose a termination effect that fails with the app-server transport’s terminal error.
Bound requests and propagate adapter failures apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
The adapter applies two-minute deadlines to turn/start, thread/start, and thread/resume. Timeouts and transport termination fail the event stream. Process termination escalates to SIGKILL after two seconds. Tests cover unanswered requests and transport exit.
Settle runs with unknown provider outcomes apps/server/src/orchestration-v2/ProviderTurnStartService.ts, apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts
Unresponsive errors, including nested causes, use the run failure path instead of retry or fresh-thread fallback. Tests cover unknown outcomes from thread/start and thread/resume.

Explanation
The title "fix(server): retire unresponsive shared Codex app-servers" follows conventional commit format and names the main change. The diff adds transport termination exposure, retires shared sessions when the transport ends or setup stalls, and bounds thread/turn requests with deadlines, so the title matches the changeset's primary purpose.
Full details: Description check
Explanation
The description follows the template's sections (Problem, Change, Scope and approval, Verification). It states the problem, explains the fix, links issue #24300, cites explicit maintainer authorization, and gives concrete verification with commit-level results, a before/after runtime table, and a note that no visual change occurred. It also lists limitations, including the two-minute deadline scope, the sibling-turn interruption, and untested Windows and macOS process cleanup. The Verification section is thorough. The template's Problem and Change headings are present with content.
Full details: Linked Issues check
Explanation
The changes address the coding requirements in [#24300]. CodexAppServerClient exposes transport termination. CodexAdapterV2 fails the event stream on transport termination and on unanswered thread/start, thread/resume, and turn/start requests. This retires the shared session through the existing release path and bounds setup and turn acceptance. ProviderTurnStartService treats timeout outcomes as unknown, so it does not retry or create a fallback native thread. The focused tests cover these failure paths. The optional isolation goal is not implemented, and the PR documents that sibling turns can be interrupted.
Full details: Out of Scope Changes check
Explanation
The changes stay within the linked issue scope. They modify Codex app-server transport termination, request deadlines, process shutdown escalation, session retirement, and unknown-outcome handling. The added tests cover these server-side behaviors. No client, wire-schema, or unrelated product changes are shown.

Apply the deadline to rollback’s thread/resume request.
When rollbackThread finds an unloaded paginated thread, it sends thread/resume here without withAppServerDeadline. If the transport stays connected but Codex does not answer, rollback waits indefinitely and this request does not retire the shared app-server. Apply the deadline to this request as well.
