fix(claude): steers sent during compaction no longer abort it - #25848
While Claude Code is compacting context automatically, a steer offered at priority "now" aborts the compaction (compact_result: failed, compact_error: aborted, result aborted_streaming), and Claude starts compacting again. A coordinator thread that receives frequent t3_thread_send steers can keep restarting its compaction. To reproduce, send a steer to a Claude thread while its timeline shows "Compacting context".
Change
ClaudeAdapterV2 holds steers on the active turn while its compaction runs. The hold check runs under the existing sdkMessageLock. Once the compaction ends (its compact_result status, or a bare compact_boundary on CLI versions that send only that), the held steers are offered in order at priority "now", so the same turn answers them.
steerTurn waits until its steer is actually offered, so a steer reports success only once Claude has it. If the turn ends while the steer is held, steerTurn fails with the existing "ended before the steer was sent" error, and the existing steer-follow-up path re-dispatches the message.
Stop cancels held steers instead of offering them, and releases their PDF reservations.
dispatchRunInterrupt now also cancels unsettled provider-turn.steer effects, the same way it already cancels provider-turn.start/restart. The effect outbox runs one effect per thread at a time, so without this a steer waiting out a compaction would hold Stop until the compaction ended. This applies to every provider: a steer still waiting to reach the provider when Stop arrives is cancelled, where before it was delivered and then interrupted.
"Stop cancels a steer still waiting to reach the provider"
Red on main: the first version of the hold test, run against main's adapter, fails with a steer sent while compacting reached Claude before the compaction ended: expected [...] to have a length of 1 but got 3 (red-main.log). With ClaudeAdapterV2.ts and Orchestrator.ts reverted to main, all four tests above fail.
Green on the branch: both files pass 260 tests, 28 of them matching "steer" (green-branch.log). The EffectWorker, ProviderTurnControlService, ThreadStop, DelegatedCompletionDelivery, ClaudeAutomaticDelivery and BackgroundWorkStop test files also pass. vp run check:changed passes.
Not checked: a live Claude Code compaction. The tests feed the adapter the SDK frames from the report (status: compacting, compact_result, compact_boundary).
Evidence: none, because nothing visible changes.

This test reproduces #25796: a steer sent while Claude is
compacting is offered to the SDK query at priority "now" right away, which
aborts the compaction. The test expects the adapter to hold steers until
the compact_result status arrives, then offer them in order.

Claude Code aborts automatic compaction when a priority "now" user message
arrives while it compacts, then restarts it. The adapter now holds steers
on the active turn while its compaction runs and offers them, in order and
still with priority "now", once the compaction result status (or a bare
compact_boundary) arrives, so the steer is answered in the same turn.
steerTurn returns immediately, so Stop is never queued behind compaction.
Stop offers held steers before interrupting, and a turn that ends with
steers still held drops them and releases their PDF reservations.

A steer held for compaction made steerTurn succeed at once, so a delegated
message was accepted as delivered and then dropped if the turn ended before
the compaction did. Stop also offered held steers mid-compaction instead of
cancelling them.
steerTurn now waits until its held steer is offered, and fails with the
existing "ended before the steer was sent" error when the turn ends first,
so the follow-up re-dispatch applies. Stop drops held steers without
offering them. Because a waiting steer blocks the thread's effect queue, a
run interrupt on a live provider turn now also cancels unsettled steer
effects, which interrupts the waiting steerTurn instead of making Stop wait
for the compaction.

Macroscope's review found this PR requires human approval — This fix changes asynchronous steer delivery and Stop cancellation semantics, including a cross-provider outbox behavior change. The added lock/deferred state and turn-lifecycle coordination are sufficiently nontrivial to warrant human review despite focused regression tests.

Claude turns now hold steers received during compaction and offer them in order after compaction ends. Held steers fail if the turn stops or ends first. Run interruption also cancels pending steer effects.

ClaudeAdapterV2 queues steers during compaction or behind earlier held steers. It releases them in order when compaction ends. Held steers fail if the turn stops or ends first. Tests cover ordered release and both failure cases.

dispatchRunInterrupt requests cancellation of unsettled steer effects. The integration test verifies that stopping during a pending steer interrupts the provider turn.

Full details: Linked Issues check
Explanation
Issue [#25796] requires held steers during Claude compaction, ordered delivery on completion, same-turn handling, and cancellation of undelivered input on Stop or turn end. ClaudeAdapterV2 adds a lock-protected hold queue, releases it after compact_result or compact_boundary, waits for actual offer completion, and fails held steers when the turn ends or stops. Orchestrator cancels pending steer effects on interruption. The PR adds regression coverage for ordered release, Stop, turn end, and pending-steer interruption. These changes also preserve the existing follow-up path through the provider error for an unsent steer.
Full details: Out of Scope Changes check
Explanation
The changed adapter, orchestrator, and tests support the linked issue [#25796]. The generic interruption change applies pending steer cancellation across providers, but it directly supports the required Stop behavior and prevents Stop from waiting behind an undelivered steer. No unrelated change is established.

dispatchRunInterrupt cancels unsettled provider-turn.steer effects. EffectOutbox.cancelUnsettled updates only the effect row. When the worker wins the cancellation race, it returns before executing the steer and its outcome handling. A steer held for compaction or pending in the outbox can therefore remain without an undelivered or failed message outcome.
Record an undelivered or failed state for each cancelled steer message before committing the cancellation. Emit an error turn item for each cancelled provider-turn.steer effect.
