Compare the merged snapshot before publishing.

If a Free plan has a duration-less primary window, a plan-only update to Plus changes rateLimitSnapshot but publishes no window. The next notification with the same primary value is then filtered out, so the UI keeps showing a monthly window until usage changes. Compare the merged snapshot with previous so plan changes publish affected windows. Add a regression test for this sequence.

As per path instructions: “For a logic, data, state, or protocol bug fix, provide a failing test first.”

Update the comparison in codexRateLimitsToUpdate to compare the merged
rate-limit snapshot with previous, so plan-only changes publish affected windows
even when primary values are unchanged. Add a regression test for a Free-to-Plus
plan update followed by a notification with the same primary value, verifying
the UI receives the updated window.

Add an adapter-level regression test for repeated rate-limit notifications.

The helper test passes previous directly, so it cannot detect if CodexAdapterV2 omits that argument. The existing adapter case emits two notifications with different reset times and checks only terminal state. Add a replay test beside those cases in apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts that captures onUsageLimits, emits two identical notifications, and asserts that only one update is published.
