fix(server): a steer during a delegate_task wait no longer drops the child's result - #27387
A parent that calls delegate_task with mode: "wait" loses the child's result if the user steers it during the wait and its turn is still running when the child finishes. The steer cancels the tool call, which ends the wait without a taskId. The task stays at completionWake: "settled_only". The orchestrator then skips delivery while the parent run is live, and nothing delivers the result after that run settles. The task row ends as completed | settled_only | completionDelivery: null, the same row the issue reports.
Reproduced on main a773dc38ac in the web dev server with a Claude Sonnet 5.5 parent:
The parent calls delegate_task (mode wait). The child is Claude Haiku 5.5 and sleeps 25 s.
While the row shows "Delegating", steer the parent to run a 60 s command.
The child completes while the steered turn is still running. That turn ends with STEER-DONE, and CHILD-DONE-27362 never reaches the parent.
Change
The blocking wait now lives in one place, awaitTask, which the local and remote (linked environment) wait paths both call. The switch to completionWake: "always" used to run only when the wait returned without a terminal result (a timeout). It now also runs from an Effect.onExit finalizer when the wait fails or its fiber is interrupted, which is what a cancelled MCP tools/call does. The finalizer runs uninterruptibly. It uses the same delegated_task.wake-policy command and the same command ids as before.
That command already plans delivery for a task that finished while the parent run is live. So once the switch runs, the result is delivered whether the child finishes before or after the steer. No contract, schema or client changes.
Triaged via the automated issue flow. Out of scope: the work-log label ("Failed to delegate a child task" after the steer), which the issue also mentions.
Verification
Red, then green:
The red commit f74aa42449 adds a blocking local delegation cancelled mid-wait wakes its parent like an async task and its remote variant to apps/server/src/mcp/OrchestratorMcpService.test.ts. Each test forks delegateTask({ mode: "wait" }), waits until the wait has subscribed, interrupts the fiber the way a cancelled MCP request does, and reads the task row.
At f74aa42449 (main plus the tests), both fail: expected 'settled_only' to equal 'always'.
At the fix, OrchestratorMcpService.test.ts and DelegatedCompletionDelivery.test.ts pass, 71/71.
vp run check:changed passes at the merged head: format, lint, knip, typecheck (3 packages), tests (3 files, 43/43).
Runtime transcript. Run in the real web client with T3CODE_DEV_SEED=0 vp run dev, a fresh database per side, a real Claude Sonnet 5.5 parent and a Claude Haiku 5.5 child, steered from the composer. Columns: time | status | completionWake | completionDelivery.
Variant: a 75 s child and a steer that ends the parent turn first. Main is unaffected there, because the existing post-settle wake delivers. The fix gives the same outcome, with the row switching to always right after the steer.
Not checked: the remote (linked environment) wait path in a real app. Its unit test covers the switch. Captures exist locally but are not attached, because the upload tool was unavailable. The fix has no UI surface.

…ver wakes its parent
When a wait-mode delegate_task call is interrupted mid-wait (a steered Claude
parent cancels the MCP tools/call), the task stays completionWake
"settled_only", so its result is never delivered to the still-live parent run.
These tests reproduce #27362 for the local delegateTask path and the remote
awaitTask path; both fail on main.

…child's result
A wait-mode delegate_task upgraded the task's completionWake to "always"
only after waitForTask returned, so an interrupted tools/call (a Claude
steer mid-wait) left the task settled_only and its result never reached
the still-live parent run.
awaitTask now owns the blocking wait for both local and remote tasks and
upgrades the wake on any non-terminal end, including interruption and
failure, through Effect.onExit. Command ids and wake-policy logging are
unchanged.

Macroscope's review found this PR approvable — Interrupted delegated-task waits now preserve later child-result delivery through the existing parent wake mechanism. Normal completion and task creation settings remain unchanged, keeping the change narrowly scoped.

Wait-mode delegations now upgrade a task’s completion wake policy when waiting ends without a terminal result. Local and remote delegations use the shared wait path. A test covers interrupted waits.

Summary
Wait outcome and wake-policy handling apps/server/src/mcp/OrchestratorMcpService.ts
awaitTask returns a waited result directly only when it is terminal. Otherwise, it upgrades the completion wake policy and rereads the task. It marks a timeout only when the wait returned no result. Wake-policy dispatch failures are logged and do not replace the wait outcome.
Delegation callers and interruption coverage apps/server/src/mcp/OrchestratorMcpService.ts, apps/server/src/mcp/OrchestratorMcpService.test.ts
Remote and local wait-mode delegations pass a request key to awaitTask. The test interrupts local and remote waits and checks that the task remains running with completionWake set to always.
pingdotgg/t3code#27379 — Both changes handle interrupted blocking delegate_task waits by upgrading completionWake.
pingdotgg/t3code#25662 — Its transaction merge changes the wake-policy dispatch path used by this PR.

Steering a blocking delegate_task wait now delivers the child result in the normal cases. In a rare case, a cancellation arriving just as the wait times out could still leave the task on the settled-only policy and skip delivery. This is a small, localized fix that can be made before or soon after merge.

Explanation
The whole-PR diff against the merge base changes apps/server/src/mcp/OrchestratorMcpService.ts and OrchestratorMcpService.test.ts. Every change traces to #27362. The changes are: (1) awaitTask now owns the blocking wait for local and remote delegate_task; (2) the wake upgrade runs on failure and interruption, and on a wait that ends without a terminal result; (3) the remote wait path reuses the same upgrade; (4) the wake-policy command ids are unchanged (local keeps key, remote keeps taskId); (5) updated comments; (6) regression tests. No unrelated change appears in the reviewed diff. The PR does not change contracts, schemas, or client code. The diff shows no label or presentation change, so no unrelated label change is present.
Full details: Linked Issues check
Explanation
Linked issue #27362 has two coding objectives.
(1) Deliver the child result when a steer cancels a delegate_task wait. Implemented. In apps/server/src/mcp/OrchestratorMcpService.ts, awaitTask now dispatches delegated_task.wake-policy with completionWake "always" in an Effect.onExit finalizer when waitForTask fails or is interrupted. It also runs when the wait ends without a terminal result (timeout). Both the local and remote wait paths call awaitTask. The regression tests in OrchestratorMcpService.test.ts cover local and remote interrupted waits and assert completionWake "always" after cancellation. The PR reports a red run at the base and a green run at the head (71/71). Those test results were not re-run here. The PR transcript shows delivery after a steer on the local path. The remote path has no real-app check. This objective is met on the evidence available.
(2) Work-log labels. Not implemented. The issue asks for a launched wait to read "Waiting for <child>" and for an interrupted wait not to render "Failed" (Diagnosis and Suggested fix; title). The whole-PR diff does not touch packages/shared/src/t3McpToolPresentation.ts or packages/client-runtime/src/work-log/presentation.ts. The PR description marks this work as out of scope. The issue text lists it as a requirement, so the requirement is unmet in this PR.
The linked issue is therefore only partly resolved.
Resolution
Either (a) add the label changes for #27362 in this PR: a mode-aware label for a launched wait ("Waiting for <child>") in t3McpToolPresentation.ts, and no "Failed to …" label for an interrupted or cut-short wait in work-log/presentation.ts, with tests for both; or (b) get maintainers to agree that the label work is split to a separate issue, and record that change on #27362 so this PR no longer carries that requirement.

Make the Effect.onExit finalizer decide every upgrade.
The upgradeWake decision is split across two places. The Effect.onExit finalizer handles failure and interruption. The explicit yield* upgradeWake at Line 1654 handles a successful wait with no terminal result.
Assume waitForTask succeeds with Option.none() (timeout) or with a non-terminal result. The finalizer then runs Effect.void.
Assume the MCP tools/call is cancelled at that moment, after waitForTask returns but before Line 1654 starts. The fiber is interrupted at the generator step boundary, so the explicit upgrade never runs.
The task then keeps completionWake: "settled_only". The child result does not reach the live parent run, which is the bug this PR fixes.
The trigger is narrow: the timeout and the cancellation must happen at almost the same time. The fix is small. Put the full decision in the finalizer. The finalizer runs uninterruptibly, so the upgrade happens exactly once, and the explicit call can go.
