fix(server): Claude wake runs no longer hang after a prompt joins the wake turn - #27416
A Claude provider continuation run could stay "running" forever with no output, and every run queued behind it waited too. On 2026-10-30 the "PR Takeovers" dispatcher thread on nucbox-1 (server v0.1.4-nightly.20261027.3796) had run 2967 running for over 1.5 h, with 13 runs queued behind it.
The provider event log shows this sequence:
04:55:26.065: turn 2965 ends with its result (num_turns 3).
04:55:27.258: a run_in_background Bash task started in 2965 finishes while the thread is idle. The CLI emits task_updated, task_notification and background_tasks_changed. At 04:55:27.296 it emits system/init and status: requesting, so the CLI has started its own wake turn.
No turn is active, so the adapter buffers these frames in wakeBuffers and requests a provider continuation (run 2967).
04:55:28.229: the adapter offers the next queued user prompt (run 2966, a webhook) to the same query, which is already inside the wake turn.
The CLI answers the notification and the prompt in one turn. Its result at 04:55:59.167 (num_turns 4) settles run 2966.
04:56:00.927: continuation run 2967 starts and drains the wake buffer. The buffer holds init but no result. init is a root turn frame, so nativeWakeStarted becomes true.
The continuation has no buffered result and no opaque-task shortcut. Because the wake had started, the "wake never opened" deadline was not armed either. Nothing finalizes the turn, yet the CLI is idle and the wake's result already went to run 2966.
Fix
Root cause. When a user prompt is offered while the wake buffer holds an opened wake turn (an init with no result after it), the adapter now marks that init as answered by the prompt. When the continuation drains a buffer whose latest init is marked, it replays the buffered frames and then settles as completed. The wake's frames still replay, so the "Command … finished" notification stays visible in the thread. If Claude instead finishes the wake before the prompt (the existing early-echo rebuffer path), that result is in the buffer and settles the continuation as before.
Backstop. The "wake never opened" deadline is now a watchdog that keeps running after the wake opens. An opened wake turn that streams no frames for 10 minutes, with no open tool call and no pending approval or user-input request, settles as completed (orchestration-v2.claude-wake-turn-went-idle). The never-opened behavior is unchanged.
Verification
Two new tests in ClaudeAdapterV2.test.ts (background wake turns) use the wake testkit:
settles the continuation when a queued prompt joins the opened wake turn reproduces the incident: an idle task_notification and init, a continuation request, a queued prompt offered, and a single result. Both runs reach a terminal state.
settles an opened wake run that goes idle without a result: frames re-arm the watchdog, and silence settles the run.
Both tests time out (hang) with the adapter change reverted and pass with it. All 259 tests in the file pass, and vp run check:changed passes. This is a server-only fix with no UI surface, so there are no screenshots.
… wake turn
When a background task finished while the thread was idle, Claude opened its
own wake turn and T3 requested a continuation run. If a queued prompt was
offered into that already-open wake turn, Claude answered both with one result,
which settled the prompt's run. The continuation then drained a buffered init
with no result, so it waited forever and blocked every queued run behind it.
Mark a buffered, still-open wake turn as answered when a prompt is offered into
it, and settle the continuation after replaying the wake frames. As a backstop,
the wake-start deadline now keeps watching an opened wake and settles it once it
streams nothing and holds no tool call or pending request for 10 minutes.

A wake turn with a foreground Agent doing a long, quiet operation is finalized as completed after an idle deadline, before its native result arrives. This guard checks only context.toolCalls and pendingRuntimeRequests, but foreground Agent calls and their child tools are tracked separately; include outstanding foreground subagent work in the activity check.

Macroscope's review found this PR requires human approval — The change prevents joined-prompt wake continuations from hanging and introduces automatic completion after a 10-minute idle window. That new default needs human acceptance of the timeout and completion semantics for quiet, long-running work.

Walkthrough
Claude wake continuations now track received frames and wake-start messages answered by queued prompts. The adapter completes continuations for answered wakes or opaque task notifications without a native wake, and monitors other continuations for wake start and idle timeouts.

The adapter tracks wake frames and identifies buffered wake-start messages answered by prompts. A monitor checks for wake start and idle timeouts, while tests cover a queued prompt joining a wake and completion after an idle timeout.
pingdotgg/t3code#22510 — Describes a queued prompt answering an already-open native wake; this change tracks that answered wake and completes the continuation.
A permission request arriving at the idle deadline could be cut off when its wake turn completes. Coordinate request registration with the watchdog before merging, or accept this narrow risk.

Hold sdkMessageLock only while registering the runtime request.
Holding the lock across awaitClaudeApprovalDecision would block SDK message handling until the user responds. Acquire the lock for the pendingRuntimeRequests insertion, then release it before waiting for the decision. This closes the watchdog race without blocking message processing.
