fix(server): a rate-limited Claude usage read no longer clears the limit bars - #27583
claudeUsageResponseToLimits treats that as a successful read with no windows. resolveUsageLimitsAfterProbe then publishes the empty snapshot, so Usage → Limits drops the last good Session/Weekly bars until a later probe succeeds.
Reproduced on Windows with Claude Code (native OAuth, Max) by calling q.usage_EXPERIMENTAL_MAY_CHANGE_DO_NOT_RELY_ON_THIS_API_YET({ skipBehaviors: true }) the way the capabilities probe does; it returned exactly the payload above.
Change
A rate_limits object carrying error now maps to probeFailed, the same result as a timed-out or thrown get_usage. The existing pipeline already keeps the last published snapshot for probeFailed and still accepts streamed rate_limit_event updates. rate_limits_available: false still maps to unsupported. I also updated the probe type's doc comment so the failure case stays documented.
Scope and approval
This is a small fix for an obvious bug, so it uses the no-prior-issue exception. It is a one-condition change in the shared Claude usage mapper. It is related to #19807, which covers the same 429 on the CLIProxyAPI hub path. It is separate from #14441 (unifiedWindows stream events) and does not overlap with it.
Verification
New regression test in claudeUsageLimits.test.ts: fails on main (1 failed / 8 passed), passes with the fix (9/9).
Adjacent suites (ClaudeCapabilitiesProbe, Drivers/claudeUsage, ProviderInstanceRegistry, ProviderRegistry, usage/cliproxyApi) pass. tsc --noEmit in apps/server, vp fmt --check and vp lint are clean.
Not checked: the live UI after a real 429. I confirmed the payload live, but the end-to-end behavior is covered only through the mapper and the resolver logic.
Implemented with Claude Opus 5.5 in Claude Code (via T3 Code); reviewed with DeepSeek V4.1 Flash in OpenCode.
Macroscope's review found this PR approvable — Claude usage reads containing an error now follow the existing failed-probe path, preserving previously known limit bars rather than clearing them. The change is limited to usage reporting and does not alter quota enforcement or billing.

Walkthrough
The usage-limit conversion now treats a response containing a rate-limit error as unavailable. A test checks the result, and a provider comment clarifies how to interpret the response.
The conversion treats a response with a rate_limits.error property as unavailable. The test checks that the result has no windows or overage-included name and uses the probeFailed reason. The provider comment describes the error condition.
pingdotgg/t3code#28376: Both changes map Claude usage responses without usable limits to probeFailed, using different response signals.
pingdotgg/t3code#24119: Both changes relate to preserving usage-limit state across failed probes.
pingdotgg/t3code#22280: Both changes handle recovery or state preservation after failed or rate-limited Claude usage refreshes.

Problem
When the claude.ai usage endpoint refuses a read (HTTP 429), the Claude Agent SDK's
