fix(server): keep large tool output out of SQLite - #28167
Completed command_execution and dynamic_tool items can persist large inputs and outputs inline in both orchestration_events and orchestration_v2_projection_turn_items. A single result can exceed the 64 KiB budget, and each update duplicates the payload. Deleting a source thread can remove content still referenced by surviving forks, breaking transcript export.
Change
The EventSink now writes large item content to the server attachment store and keeps a bounded preview plus a contentRef in each SQLite row. Turn-item detail, MCP timeline, and command-stream reads restore the full content on demand. Image blocks use the existing attachment route.
Thread deletion retains attachments referenced by surviving forks and removes them after the last fork is deleted. Provider events, imported history, and server-created items use the same persistence path. The change also keeps failure-status metadata when an output is compacted.
Scope and approval
Related issue #26591 describes the served-image storage path. It is related context, not approval for this broader command/MCP output scope. Maintainer approval for this scope remains pending.
Verification
vp test run apps/server/src/orchestration-v2/runtimeLayer.test.ts -t "keeps source content readable through a fork until its last fork is deleted" reproduced content-missing against the original deletion lookup and passed with fork-aware retention. vp run check:changed passed on 39 files since d897d3f3f00f; format, lint, both knip checks, GraphQL, 35 package typechecks, and 603 tests across 32 files passed.
A copied-sample proof reduced SQLite event and projection payloads from 3,387,590 B to 10,360 B. The referenced content totaled 1,548,491 B, and both sampled content hashes matched after readback. The web MCP inspector fetched the stored result. Mobile typechecks passed; no native device pass was run.
Macroscope's review found this PR requires human approval — Large tool results and images move from SQLite into attachment files, with readers restoring full content on demand. The reported cleanup gaps can retain customer content after rejected writes or thread deletion, so attachment lifecycle guarantees need resolution.

Walkthrough
The change stores large command and dynamic-tool content and tool-output images as attachments, persists bounded previews with content references, and adds hydration to read paths. It also updates attachment collection and deletion, fork handling, workspace-preparation results, and failure presentation.

Adds content-reference and image-attachment metadata. The codec creates bounded previews and encodes stored fields. The content store writes and reads attachments and can restore image data.
The ingestor passes original turn items to event persistence. EventSink stores their content before writes and cleans up created attachments when a transaction fails. Wire projections omit content references. Attachment lookup includes turn-item references and supports deletion across eligible fork ancestry. Runtime and test layers provide the content store.
pingdotgg/t3code#26591 — The change moves served tool-output images from event and projection rows to attachments and restores them on demand.
pingdotgg/t3code#9661 — The change bounds persisted command and dynamic-tool payloads, but the issue also tracks other retained output copies that are not established as addressed here.
Deleting a source thread now preserves offloaded output needed by surviving forks. No actionable merge-blocking risk remains in the reviewed changes.

Remove the standalone TurnItemContentStoreShape interface.
The repository rule says: "No standalone FooShape; name the type Foo["Service"]." The rule also asks for the interface to be inline in the Context.Service tag. Declare the method types inline in the tag generic. Then replace TurnItemContentStoreShape["store"] and TurnItemContentStoreShape["read"] with TurnItemContentStore["Service"]["store"] and TurnItemContentStore["Service"]["read"].

Keep each stored output version distinct in the test fixture.
If the same command item stores two large outputs, this attachment ID stays constant and the second write replaces the first. Reading the first event’s contentRef then returns the second output. The production store derives its ID from the content bytes, so this fixture can conceal an event-replay regression. Key the fixture by content, as the production store does. started an isolated dev environment, completed a Replay (dev) showcase turn through the provider/server path, then paired the web client using Playwright and /usr/bin/google-chrome. opened the completed thread and expanded its command card; the recorded command output appeared in the client. this is a smoke check, not a fresh large-output provider reproduction. stopped the dev processes and blacksmith worker.
the earlier cleanup fix remains intact: direct thread deletion and project cascades share one permit for deletion planning and commit, acquired before thread locks. existing lifecycle tests cover sequential deletion, concurrent direct deletions, and a project cascade racing direct deletion.

native ios/android and electron clients, macos/windows, other providers, remote/relay connections, imported history, interrupted preparation, and rejected-write cleanup were not exercised in the running app. attachment-backed tool-image serving was not exercised: the native image-view call used a file path and does not prove that route. focused tests cover image access and ownership, but those are not a real provider image result. no latency or performance claim is made.

Retain source attachments referenced by forks.
A fork exposes inherited source items with their original threadId and contentRef. Deleting the source schedules those attachments for cleanup, while the surviving fork remains readable. Its transcript export can then fail with content-missing.
Copy inherited content to fork-owned attachments, rewrite contentRef, or retain source attachments while any surviving fork references them.
