fix(server): a git action that fails partway refreshes the Git panel - #29651
Commit & push and Commit, push & create PR run their steps in order: create the feature branch, commit, push, then open the PR. If a later step fails after an earlier one has already landed, the Git panel keeps showing the state from before the action ran. For example, a pre-push hook or a non-fast-forward rejection can fail the push after the commit is made. A commit hook can also fail after the new feature branch has been checked out. The just-committed files still look uncommitted, or the branch toolbar still names the old branch, until the window is refocused or the user refreshes by hand.
The server refreshes local status after git.runStackedAction only when the whole action succeeds. Neither web nor mobile refreshes after a failure, and local status is otherwise republished only on an explicit refresh, a window wake, or the end of a turn.
Change
The gitRunStackedAction handler in ws.ts now also calls the existing refreshGitStatus(cwd) on failure, before it fails the stream. The client still gets the same error. This follows #19686, which made the automatic pull refresh local status after a failed attempt.
Scope and approval
Very small, focused fix for an obvious bug, so there's no prior issue. The production change is 3 lines in one handler, and the success path is unchanged. vcsPull has the same shape, but it is left alone: its pull --ff-only doesn't touch the working tree when it fails.
Overlap search: the local PR index plus GitHub searches for "git action fails status refresh" and "refreshGitStatus". The closest open PRs are #28575 and #28577. They change the vcsCreateRef and vcsSwitchRef handlers, not this one.
Verification
New case in apps/server/src/ws.test.ts drives the real layerWsRpc handler with a failing runStackedAction. It checks that the client still gets a failure exit and that the status broadcaster is refreshed for that cwd.
Red on main: × a git action that fails partway still refreshes the repository status, Error: Test timed out in 15000ms. The refresh is fire-and-forget, so the test waits on a Deferred set by the refresh mock. Without the fix that Deferred is never set.
Green: vp test run src/ws.test.ts (apps/server): 16/16 passed, rerun after rebasing on current main.
vp run check:changed: format, lint, knip, typecheck and tests all pass.
Not checked: a real client against a live repository with a rejecting push hook.

Macroscope's review found this PR approvable — Failed Git actions now refresh the panel so completed intermediate steps are reflected. The change reuses the existing background refresh and its safeguards while preserving the original failure response.

Walkthrough
When gitRunStackedAction fails, the WebSocket handler refreshes Git status for the working directory before forwarding the original cause to the progress queue. A WebSocket RPC test covers this failure path.
Changes: Stacked action failure handling

Summary
Failure status refresh and RPC test apps/server/src/ws.ts, apps/server/src/ws.test.ts
The failure path refreshes Git status before forwarding the original cause. The test verifies that a failed gitRunStackedAction request refreshes /repo and that the RPC exits with failure.
The Git panel is intended to refresh after a stacked action fails, while the client still receives the failure. No actionable merge-blocking risk is identified.

Move failure-path coordination into a service.
The gitRunStackedAction handler now coordinates the workflow and status-refresh services. Put that coordination in a domain service, then let the handler call one service method and forward its result. As per coding guidelines, “A transport handler does three things: decode the request, call one service method, and map the service's typed errors to the transport's error. Nothing else.” The path instructions also place multi-step dispatch in a service.

Review comment at @apps/server/src/ws.ts at line 3036:
Move the workflow and status-refresh coordination in the gitRunStackedAction
handler into an appropriate domain service method, including the
refreshGitStatus failure path. Update the handler to call that single service
method and forward its result, preserving the existing typed-error mapping.

Use a distinct refresh error and assert the action cause.
The action and refresh mocks fail with the same failure object. The "Failure" assertion cannot detect a regression that sends the refresh error instead of the action error. Give refreshStatus a different error and assert that the RPC exit retains the action cause. As per coding guidelines, “Backend behavior changes ship with focused tests for that behavior.”

Update the test around the `Deferred.succeed(refreshed, cwd)` refresh mock to
fail with an error distinct from the action failure, then assert that the RPC
exit preserves the action cause rather than the refresh error.
Verified as-is at 92ad79fd5e39716fd37d420a638d56208295999e on a Linux VM with the actual Vite web client, repository Playwright and /usr/bin/google-chrome, and real Codex gpt-6.1-sol turns. Projects, threads, repository contents and rejecting hooks were created through the app/provider. One dev server ran at a time.
Reproduced on github/main (57cd05904d): clicking Commit, push & PR commits the pending file, then the real pre-push hook rejects the push. Git reports a clean working tree, but the client still offers Commit, push & PR.
Repeated on the exact PR head with the same repository, initial commit and pending file: the original push failure still appears, but the primary action updates to Push & create PR and Commit becomes disabled. No reload, refocus or further provider turn was needed. The Changes count is the comparison against main and can remain +1 after a commit.
Neighboring failure: Commit & push on main, then Check out feature branch & continue, with a rejecting pre-commit hook. The commit fails, while the Git toolbar correctly changes from main to the newly created feature branch. The file remains staged and HEAD is unchanged.
Neighboring success: removed the rejecting hooks through Codex, then used UI Commit and Push. Both succeeded; the local bare remote contains the exact new commit 0ef5e9d and the working tree is clean.
Blacksmith: vp test run apps/server/src/ws.test.ts passed 16/16. vp run check:changed HEAD^ passed formatting, lint, knip, GraphQL, typecheck (3 packages), and tests (2 files). The worker needed the depth-2 contributor head fetched because its source snapshot initially lacked parent history. No source changes were needed.
Native iOS/Android and Electron clients, macOS/Windows, remote/relay/tunnel connections, non-fast-forward remote rejection, and external forge PR creation were not exercised. Push used a real local bare remote and an executable rejecting Git hook; PR creation was never reached during the failing stacked action. Other provider adapters were not exercised.

Move failure reconciliation into a service.
The handler now calls GitWorkflowService.runStackedAction and starts VcsStatusBroadcaster.refreshStatus when that call fails. Put the action and best-effort refresh in a service that owns both operations. Keep the transport responsible for forwarding progress and the original failure. As per path instructions, “WebSocket transport handlers should decode the request, call one service method, and map typed service errors to transport errors; ... multi-step dispatch ... belong in services.” (raw.githubusercontent.com)

Move the failure-triggered best-effort status refresh out of the WebSocket
handler into a service that owns both the stacked action and reconciliation,
reusing GitWorkflowService.runStackedAction and
VcsStatusBroadcaster.refreshStatus. Keep the handler focused on calling one
service method, forwarding progress, and mapping typed service errors to
transport errors while preserving the original action failure.
The test records only response.exit._tag. The refresh mock also fails with the same failure, so this test would pass if the RPC sent the refresh error instead of the action error. Give the refresh a distinct error and assert the failure cause in the Exit response. This protects the unchanged client-error contract. As per path instructions, “Backend behavior changes should have focused tests.” (raw.githubusercontent.com)

Update the RPC test around the Exit response recorder so the refresh mock fails
with an error distinct from the action failure, then assert that the Exit cause
contains the action failure rather than merely checking response.exit._tag.
Preserve the existing client-error contract.

On the follow-up about moving failure reconciliation into a service (ws.ts:3075): leaving it in the handler for this PR. Refreshing Git status after an operation is how ws.ts already works on main. The handlers call the local refreshGitStatus helper, a detached VcsStatusBroadcaster.refreshStatus that ignores its own failures, at about ten sites. Those include this handler's own success path (~3096) and createWorktree, removeWorktree, createRef, switchRef and initRepository (~3106–3117). This PR calls the same helper on the failure path, so a partly applied stacked action is reflected too. Moving refresh into the services would change all of those handlers together, so it belongs in its own refactor rather than this fix.
