Problem

Refs #29778.

Completed delegated Codex conversations remain in native active history after T3 delivers their results. The reporter correlated 436 such children with ChatGPT desktop Recents. That affected Recents view remains unverified.

Change

Archive completed, idle, app-owned Codex children after the parent receives their results. Keep their T3 history and resume the same native conversation when continued. Current-run identity, delivery, queued work and native-descendant checks protect unfinished work. Pending delivery retains the session within the existing four-hour limit.

A pending child whose first turn arrives after Stop settles only when its actual completion arrives. A later accepted turn reopens it, while stale completions preserve the newer turn. Failed sweep archives use bounded unload and preserve replacement unload tracking.

Main already supplies archived-source fork recovery from #29618. This PR adds no duplicate recovery implementation.

Scope and approval

This addresses only native Codex history left by completed T3-owned delegation in #29778. It changes default native conversation visibility. No explicit maintainer approval of that lifecycle choice has been given, and no exception under CONTRIBUTING.md is claimed. The reporter's investigation establishes the reported failure, not approval of this approach. Maintainer review remains pending, as #29868 (comment) notes.

Verification

Head 2118005ab17062526d1fb626a88bc8d863bac184, base 8b44f42b14c8e273156474a924ccf29680dd7857. The six earlier lifecycle patches compare equal after rebase; main's five newer commits concern diff metadata, layout, usage rows and release versions.

On physical corex, Linux x86_64, vp run check:changed 8b44f42b14c8e273156474a924ccf29680dd7857 passed format, lint, knip, GraphQL, typecheck of 26 affected packages and 196 tests across three files. Current-head output, red/green regressions and independent review findings are in #29868 (comment). All CI checks passed at this head. CodeRabbit completed review with no actionable code findings. Its description warning prompted the Scope and approval disclosure above. The thread-transfer report shows no wire-size or message-count increase for Codex or Claude against this base.

The extended notification regression failed with pending instead of interrupted before R10, and interrupted instead of running before the resume correction. The sweep regression failed with close instead of unload before its fallback. The existing lifecycle table also caught an overly broad reopening condition; the final condition preserves delayed first-turn completion state. Fresh independent Codex reviews found and prompted the continuation and tracking corrections.

Maria's real Linux T3/Codex integration used head bf43c504d3d73c70131bdd8fea7d270a95b6f25f, Chrome 151, Playwright 1.60.0 and Codex 0.164.0. It showed cleanup after the normal 30-minute idle timeout, retained T3 messages, same-ID continuation and archived-source fork. This is older-build evidence, not a run of the current head.

Standalone isolated Codex 0.166.0 probes on corex Linux and shiv macOS 27.0.1 observed active-list removal, archived-list inclusion and same-ID continuation with earlier history. They used a synthetic local Responses endpoint and no T3 build. They establish the native contract only.

Not shown and limits

Authenticated ChatGPT desktop Recents remains unproven. The macOS retry and alternatives reached the signed app's login screen. The reporter request is unanswered, the laptop was unreachable, POCOs lacked an OpenAI app, and iOS device access failed. The completed Windows attempt in #29868 (comment) left the VM stopped after OSStatus -10004; no guest client was tested. Boat's Linux worker cannot supply this macOS evidence. No test environment was signed into an account.

Packaged T3 desktop/mobile and remote/relay/tunnel modes were not run. Late delivery and descendant protection have focused regression coverage but no real-turn capture for this head. Cleanup covers live sessions, not historical backlogs, crashes before cleanup or delivery beyond the pin limit. Failed native archive can leave a child visible. An explicit fork can reactivate its source. Reverting stops future cleanup; already archived history can be unarchived.

Readiness remains blocked on evidence from the reported app.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — Completed delegated Codex conversations will automatically disappear from other Codex clients’ active thread lists after result delivery and idle checks, while remaining resumable in T3. This changes default product behavior and requires human confirmation of the visibility change.

Merge Risk: Minimal · up to 21180

The change archives finished delegated Codex child sessions after their results reach the parent, and the earlier concerns about sessions staying pinned while a child task was still pending are addressed in the current code and tests. No actionable merge-blocking risk remains. The unverified native archive behavior for descendant subagent threads is not a merge blocker.

Full details: Description check

Explanation

The pull request description is thorough and follows the template's required sections: Problem, Change, Scope and approval, and Verification. The Problem section states the failure and links issue #29778. The Change section explains the archive-after-delivery behavior and its safeguards. The Scope and approval section discloses that no explicit maintainer approval of the lifecycle choice exists and claims no CONTRIBUTING.md exception. The Verification section gives head and base SHAs, the exact check command and its results, the 196 tests across three files, the red/green regression history, and a candid list of what was not tested. The description is detailed. Two points weaken it. The Problem section is mostly evidence context, and it does not state an expected behavior in one clear sentence. The Verification section relies on external links and comments for much of its evidence. Neither gap makes the description incomplete. The author also states that the change lacks maintainer approval, which the template treats as a precondition for scope. The Scope section documents that gap but does not resolve it. Overall, the description is mostly complete and substantive.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Extract the shared archived-session retry into one helper.

The regex /\bsession \S+ is archived\b|\bcodex unarchive\b/i and the unarchive-then-retry step now appear here and in resumeNativeThread (Lines 6733-6738). If Codex changes the error text, a fix in one place leaves the other path broken. Move the predicate and the retry into one helper that both resumeNativeThread and forkThread call.

♻️ Proposed refactor
+ // Codex refuses to resume or fork a session archived elsewhere until it is unarchived.
+        const retryAfterUnarchive = <A, E, R>(threadId: string, request: Effect.Effect<A, E, R>) =>
+          request.pipe(
+            Effect.catchTags({
+              CodexAppServerRequestError: (cause) =>
+                /\bsession \S+ is archived\b|\bcodex unarchive\b/i.test(cause.errorMessage)
+                  ? client.raw
+                      .request("thread/unarchive", { threadId })
+                      .pipe(Effect.andThen(request))
+                  : Effect.fail(cause),
+            }),
+          );

Then call retryAfterUnarchive(threadId, fork) in forkThread and retryAfterUnarchive(threadId, resume) in resumeNativeThread. The catchTags typing may need an adjustment for the generic error channel.

Include pending descendants in the session-wide idle probe.

runtime.hasPendingBackgroundWork counts a subagent only when its status is running. A subagent in pending is not counted. isOrchestrationV2WorkActive treats pending, running, and waiting as active. The per-thread probe at line 7066 already uses it.

If a descendant stays pending after delivery, releaseIfStillIdle can release the session once the idle timeout expires. delegatedChildAwaitsDelivery only matches children in the awaiting_delivery state, so it does not cover this case.

Suggested fix
-              if (subagent.task.status === "running") {
+              if (isOrchestrationV2WorkActive(subagent.task.status)) {

isOrchestrationV2WorkActive is already imported in this file.

Add a regression test. Hold a descendant in pending past a short idle timeout, then assert that the session is not released. Advance time with TestClock, not wall-clock waits.

Tested unchanged head bf43c504d3d73c70131bdd8fea7d270a95b6f25f on Linux with Chrome 151, Playwright 1.60.0 and real Codex gpt-6.1-sol turns through the local proxy (CLI/app-server 0.164.0). Projects, threads and children were created through the app. No mocks, fixtures, seeded data or timer overrides.

  • On github/main (cefb544af3), a completed, delivered child remained in native active history more than 30 minutes after completion.
  • On this head, both delivered children received successful native archive requests at the unchanged 30-minute idle timeout. Both disappeared from active history and appeared in archived history. T3 retained their visible messages. The ordinary parent stayed active.
  • A UI follow-up unarchived and resumed the same native child id, recalled its original phrase, and delivered its new result to the parent. It archived again after the next unchanged 30-minute idle interval.
  • The UI fork action recovered from the native archived-source error, created a different native id, and preserved the other child's phrase in a real follow-up turn. The source returned to active history, matching the limitation disclosed in the description.

Retried the missing ChatGPT desktop check on physical shiv (arm64, macOS 27.0.1). The official signed installer supplied ChatGPT 1.2026.183, notarized by OpenAI OpCo, LLC (2DC432GLL2). I opened its temporary bundle in the background and inspected the actual window. It first offers migration to the newer ChatGPT app with Work; dismissing that notice exposes the login screen below. There is no authenticated session or Recents view available. No sign-in or conversation changes were made. This is evidence of the access limitation, not proof of the reported Recents fix or the current Work client's behavior.

ChatGPT account gate on shiv

The existing reporter request remains unanswered. The Mac laptop did not answer SSH. Both connected POCO phones have no OpenAI app installed; the iOS simulator inventory likewise has no ChatGPT app, and input was refused because another session owns it. I did not reuse or close that session. The Linux Boat job can verify T3's changed logic but cannot provide an authenticated macOS ChatGPT client. The prior native Codex archive/resume probes and Maria's real T3 UI/provider verification remain separate evidence.

Fresh independent review checked main's recovery and settlement changes: held queued runs still prevent archiving, delivery projections are unchanged, and archive eligibility still reads the full thread state. No new integration finding or superseding implementation.

On physical corex, Linux x86_64, vp run check:changed 647bd148b3fbd7c8b57a10fd41ae6d5fc7b35cc1 exited 0, with 195 tests across three files and all format/lint/knip/graphql/typecheck checks passing. Complete output follows. New-head CI is pending; the previous runtime evidence remains scoped to its original build. ChatGPT desktop Recents is still unproven; the existing Windows-access run is queued and has not produced evidence yet.

apps/server/src/orchestration-v2/ProviderSessionManager.ts:1747-1761
Minor · Stability & Availability · Quick win

Preserve cleanup after a failed sweep archive.

When the sweep claims a loaded child, it removes idleThreadUnloads before archiving. If archiveProviderThread fails or times out, the session still proceeds to release and close, but the manager neither calls unloadThread nor restores tracking. No later unload can be scheduled because the tracking entry is gone. Match the ordinary idle path by retaining a cleanup path after archive failure, using unloadThread when available or restoring the tracking state when it is not.

Update the sweep cleanup around `archiveProviderThread` so a failed or timed-out
archive does not leave the claimed child untracked: use `unloadThread` when
available, or restore the relevant `idleThreadUnloads` tracking state when it is
not. Preserve the existing successful-archive behavior and align failure cleanup
with the ordinary idle path.

T3 delegated children run as ordinary Codex threads, so every completed
child stayed in Codex's active thread list and crowded other Codex clients
(#29778). Codex has no supported way to mark a thread as a child.

Sessions now archive an app-owned delegated child natively when it idles
out, once its latest task completed, the parent has the result, and nothing
in the child still runs, queues, or waits. A child whose result reaches the
parent after its own idle unload is archived when the shared session idles
out. Archiving drops the loaded-thread cache, so a follow-up resumes, and
Codex unarchives, the same native thread.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Archiving a Codex delegated child also archives the native subagents it
spawned. The pending-work probe found those only through a root turn still
held in memory, so a grandchild still running under a finished direct child
went unseen and archive would stop it. Replacement checks (includeSubagents)
now follow each live turn's ancestry back to the thread.

A child whose parent takes the result after the child's session idles out
lost its cleanup when the session released first. The idle release now pins
the session, within the existing background-work pin budget, while an
attached app-owned child's completed result awaits delivery, then archives
it once delivered.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Count native descendants between spawn and their first turn, and extend the existing regression through pending, running and completed states. Document cleanup of older completed sessions and provide the crypto service needed by the imported archived-fork regression on current Effect.

Co-authored-by: Alex Southwell <4596216+saphid@users.noreply.github.com>
Count pending and waiting native descendants in the session-wide idle probe, matching the per-thread archive guard. Extend the existing nested-descendant regression to check the session remains busy while pending and clears after completion.
Settle a rejected late child only on its completion, reopen it on a later native turn, and preserve newer task state against stale completions. Fall back to bounded unload after sweep archive failure while preserving replacement tracking.

[GPT-6.1 Sol][Codex] on behalf of shivam.

Rebased onto main 8b44f42b14c8e273156474a924ccf29680dd7857 and fixed the two findings in #29868 (review). Final head 2118005ab17062526d1fb626a88bc8d863bac184.

  • R10: actual late-child completion settles the pending task; interrupt acknowledgement does not. The extended replay proves continuation under a new parent, protection from stale completion, and no extra parent continuation.
  • R11: failed sweep archive uses bounded unload; tracking remains available when unload is unsupported. Captured tracking identity prevents fallback or deletion from consuming a resumed use's newer tracking.

The regressions failed before their fixes. Fresh independent Codex review also caught the resume and tracking cases; the existing lifecycle table caught an overly broad reopen condition. Final vp run check:changed 8b44f42b14c8e273156474a924ccf29680dd7857 passed on physical corex, Linux x86_64: format, lint, knip, GraphQL, typecheck of 26 affected packages, and 196 tests in three files. CI and CodeRabbit review of the pushed head are pending.

The exact check output, red/green regressions and review notes are embedded below.

The existing Windows attempt r_ouhuc4l7v5mejg00 is complete. On physical shiv, macOS 27.0.1/26A434 with UTM 5.0.8, utmctl start --hide emitted OSStatus -10004 twice and timed out. Guest inventory also timed out; the VM remained stopped. Subprocesses were reaped. No Windows build or ChatGPT client was tested. The command transcript is embedded below.

Windows VM stayed stopped

Unproven: authenticated ChatGPT desktop Recents disappearance and continuation on the reported installation. The macOS login-screen retry and previous alternatives and Windows failure do not establish that behavior. No account was signed into for testing. Native protocol checks and older-build T3 integration retain their narrower scope in the PR body.

PR29868 cleanup follow-up review
Base: 8b44f42b14c8e273156474a924ccf29680dd7857
Reviewed tree: c47854a2e5ea42835a0e59e7ce5805324d935c40 + four-file cleanup diff.

Fresh Codex review R10: completion correlation and acknowledgement fences sound; found first later accepted child turn needed reopening even at provider ordinal1. The initial terminal-status condition regressed an existing delayed-first-start case. Final condition reopens only a later observed native turn or later provider ordinal. Extended existing test through new parent/child turn, duplicate old completion and actual new completion. Red expected running, received interrupted. Green passes.
Fresh Codex review R11: bounded unload fallback sound; found post-RPC tracking deletion could discard replacement tracking. Claim captures exact tracking identity; fallback and deletion compare it, preserving newer tracking.
Fresh Codex review of follow-up fixes found no remaining issue before the required check caught the delayed-first-start regression. Root narrowed the reopening condition using the existing native-turn set, then reran the existing lifecycle table and entire adapter file. Read-only reviews, no tests or edits by reviewers.

All six patches before these fixes compare equal after rebase from main647bd148b3fbd7c8b57a10fd41ae6d5fc7b35cc1 to main8b44f42b14c8e273156474a924ccf29680dd7857. Main's five new commits touch diff metadata, Android layout, usage rows and release versions; no cleanup implementation supersedes this PR.

Unproven: actual authenticated ChatGPT desktop Recents removal. Existing native archive/continuation probes establish the provider contract only. Existing Linux integrated capture at bf43c504d3d73c70131bdd8fea7d270a95b6f25f is older-build evidence and does not establish current-head UI behavior.
