Correct the memory-mappings log table and widen the fix guidance - #536

- Replace the log table with the eight verified log sites and swap the version-pinned 'where the phrase is' column for a search-the-phrase invariant. - Lead the Kubernetes tab with the Helm chart's built-in sysctl init container; note the DaemonSet must be privileged. - Add a Docker tab: host sysctl on Linux, the --sysctl refusal, and Docker Desktop VM steps for Windows and macOS. - Align known-issues to the same 4194304 recommendation and defer its platform steps to the errors page.
