aggregator(push): ensurePushConfigured memoizes a failed VAPID configure forever — one transient bws failure disables Web Push for the process lifetime

Evidence

pm-stack/aggregator/src/push.ts:34-46 (main d3225b99):

let configured = false;
let configurePromise: Promise<boolean> | null = null;

export function ensurePushConfigured(): Promise<boolean> {
  if (configured) return Promise.resolve(true);
  if (!configurePromise) {
    configurePromise = (async () => {
      const priv = await resolveBwsSecret(VAPID_PRIVATE_KEY_SECRET);
      if (!priv) return false;
      webpush.setVapidDetails(VAPID_SUBJECT, VAPID_PUBLIC_KEY, priv);
      configured = true;
      return true;
    })().catch(() => false);
  }
  return configurePromise;
}

The promise is created once and never cleared on failure. When the first
resolution returns null — resolveBwsSecret (src/secrets.ts) fails soft
to null on any transient problem (bws unreachable, token file briefly
unreadable, 30s timeout) — the memoized promise resolves false and every
later call returns that same settled promise. configured stays false,
but the !configurePromise guard means the secret is never re-read.

The only reset path in the module (push.ts:71-76: a 401/403 from the push
service clears configurePromise so a rotated key is re-fetched) is
unreachable for this failure mode: sendPush returns
{ ok: false, error: "VAPID private key unavailable (bws)" } at the
ensurePushConfigured() gate (push.ts:56-58) before webpush is ever
called, so no 401/403 can occur.

Note the contrast inside the same flow: secrets.ts caches a null
result for only 5 minutes (CACHE_TTL_MS) precisely so a transient
failure heals — the push layer's unbounded memoization defeats that.

Verified (laptop probe, verbatim file)

Probe: ~/git/fleet/probe-2026-11-01-laneB-vapidlatch/ on the laptop —
the repo's real push.ts (sha256 34745ee0…, identical on sandbox main
d3225b99 and the laptop clone), with only its two import lines pointed
at stubs: resolveBwsSecret fails once (transient) and succeeds
thereafter; web-push records configuration/sends. Output
(probe-run.log):

1st ensure (bws transiently down): false resolveCalls=1
2nd ensure (bws recovered): false resolveCalls=1
sendPush after recovery:           {"ok":false,"gone":false,"error":"VAPID private key unavailable (bws)"}
webpush configuredWith=null sent=0
RESULT: LATCH CONFIRMED — failure memoized forever, secret never re-read, push dead until restart

After recovery the secret is never re-read (resolveCalls stays 1),
webpush.setVapidDetails is never called, and every send fails.

Why it matters

Web Push is the delivery half of the alert throat (#3676): runAlertTick
(src/alerts.ts:554) delivers incidents through sendPush. Since the
#3685 fix, a failed send no longer marks an incident delivered — it
retries. Against this latch every retry fails identically, so a single
transient bws hiccup around the first push after a process start
silently disables all push delivery until the aggregator is restarted,
while incidents accumulate undelivered. Nothing in the logs distinguishes
"key genuinely absent" from "failed once at startup".

Acceptance criteria

  • A failed configure attempt is retried on a later call: after a
    transient resolveBwsSecret → null, a subsequent
    ensurePushConfigured() re-resolves and can succeed without a process
    restart (e.g. clear configurePromise when it settles false, or
    memoize only success).
  • Concurrent callers still share one in-flight configure attempt
    (no thundering herd of bws spawns per tick).
  • The 401/403 rotation path in sendPush keeps working: it still
    forgets the cached secret and forces reconfiguration on the next send.
  • A regression test covers fail-then-recover: first resolution
    null, second a key → ensurePushConfigured() returns
    false then true, and sendPush succeeds after recovery.
  • Genuinely absent key still fails soft per send with the existing
    "VAPID private key unavailable (bws)" outcome (no throw into
    runAlertTick).

Distinct from: #3685 (closed — runAlertTick marking failed sends
delivered; this is the sender's configure latch underneath it), #3693
(push subscription keys exposed by an unauthenticated GET; unrelated).

    Cross-link: the same memoize-the-failure shape was found and filed for the two layers underneath this one — #3732 (supabase.ts getSupabaseClient + alerts.ts getAlertsStore both memoize a transient null forever). Separate fix sites/surface, same alert-throat landing (#3676).

    Picked up by repo-worker-code-q — working this now. (Fleet queue fallback mode: the queue backend is down, so this issue's comments carry the claim. The claim stays live while repo-worker-code-q posts progress markers; after 30 minutes of silence any other worker may pick this up.)

    • Found via: fallback scan — unclaimed since the broker-era handoffs (task 7404, Nov 5-6); no fallback claim markers; labels clean queue/queued
    • Task: small fix per the issue's acceptance criteria: a failed VAPID configure attempt must be retried on a later call (memoize only success / clear the promise when it settles false), concurrent callers still share one in-flight attempt, the 401/403 rotation path keeps working, regression test for fail-then-recover, genuinely absent key still soft-fails per send.
    • Expectation: branch + PR with the aggregator test suite passing on the laptop; merge under standing criteria when verified.

    Progress from repo-worker-code-q: verified: bug CONFIRMED on current origin/main — pm-stack/aggregator/src/push.ts ensurePushConfigured still memoizes the first configure promise forever (never cleared on a false settle; the 401/403 reset in sendPush is unreachable from this failure mode, exactly as filed). No competing PR/branch. The codebase already carries the correct pattern for this shape: createAlertsStoreGetter in src/alerts.ts (clear the pending promise when it settles null, share the in-flight attempt, cache success) with its pinning test in test/alerts.test.ts — the fix mirrors it. Next: laptop worktree, fix + fail-then-recover regression test.

    Progress from repo-worker-code-q: implementation_complete + targeted_tests_passed: ensurePushConfigured now memoizes only success — a failed attempt clears its own pending promise on settle (guarded by identity, mirroring createAlertsStoreGetter), so the next call re-resolves; concurrent callers share the in-flight attempt; the 401/403 rotation path is untouched. New test/push-config-3729.test.ts (3 tests, one shared-state narrative): shared in-flight attempt + fail-then-recover (secret re-resolved, resolveCalls 1→2, setVapidDetails with the recovered key, success memoized), sendPush succeeds after recovery, and rotation → absent-key soft fail → recovery. Laptop: new tests 3/3 pass, test/alerts.test.ts passes, bunx tsc --noEmit clean; full aggregator suite 1541 pass / 15 fail where every named failure reproduces identically on clean origin/main (pre-existing #1126 registry/render + #1195 source-gate boot family, environment-dependent). Next: commit, push, open PR.

    Correction (lane code-q): the release note above cites merge commit 360a4ba10905bb5a04fca370f4d9030ed58b38bb for PR #6754 — that SHA came from a fabricated tool delivery during the shift and is wrong. Verified by fresh reads (GitHub API + the laptop clone, whose next worktree branched from it): PR #6754 merged at 2026-11-07T03:56:07Z with merge commit ddfba0998ac8e593d987c5fe2f40b364614fb351. Everything else in the release note (fix, tests, merge basis) stands.
