execFile leaves child stdin open; the stub previously blocked in cat until the proxy's 30-second model-lookup timeout. This delayed every chat request and made the first test take about 60 seconds. Closes #3769.

Evidence

• Before: first focused test passed in 60.5s; fresh and resumed requests each took about 30s in the diagnostic harness. The new 1-second regression test failed before the fix because codex debug models blocked on stdin.
• After: node --test bin/codex-proxy.test.mjs — 25/25 passed in 24.9s; the focused first and test(proxies): PR #2164 cached_tokens emission is unpinned for the codex CLI transport and commandcode-proxy #3114 tests pass in 1.56s and 1.62s.
