Send ResetFailed, not Reset, when a failed append changes the journal header.

When journal.append bumps the header and then fails, subscribers keep the old header version. The Err branch sets storage_failed and returns without publishing. No later Append arrives, so subscribers never detect the stale version.

Do not send a Reset snapshot on this path. If sync_data fails after write_all succeeds, the complete entry remains in the file. replication_snapshot accepts a newline-terminated entry, so a Reset would publish an entry that the append reported as failed. Send ResetFailed instead. The receiver ends the subscription on ResetFailed and does not serve a snapshot.

Update the `journal.append` error branch to compare the current
`replication_header_version()` with `version_before`; if it changed, publish
`ReplicationUpdate::ResetFailed` through `replication_sender` before returning
the error. Do not send `Reset` or publish a snapshot on this failure path.

P2 Badge Reject replication snapshots after a storage failure

If FileJournal::append writes a complete entry but its subsequent sync_data fails, persist marks the actor storage-failed and correctly publishes no replication update. This handler nevertheless reads the journal without checking that state, and the newline check in replication_snapshot accepts the fully written but unconfirmed bytes. A newly connected replica therefore receives a signed, terminal-committed snapshot containing the failed mutation, while existing replicas remain at the previous durable cursor; a writer crash can also lose that mutation. Reject new feeds after storage failure, or restrict snapshots to a known-durable boundary.

Refuse replication feeds while storage has failed.

After append fails at sync_data, the written line remains in the journal file. The ReplicationFeed handler still captures that file and signs it as a snapshot. New subscribers then receive an entry that the writer reported as failed. Check storage_failed in the handler and return an error while it is set.

Stop lag disconnects for large snapshots under write load.

The feed buffers 256 updates, but serve_stream sends the full snapshot before it reads any update. More than 256 appends during that transfer return Lagged and end the stream. Requests carry no resume cursor, so a new feed starts from a full snapshot and can fail the same way under the same write load.

Review comment at @pkgs/id/src/world/handle.rs at line 48:
Update the broadcast buffer setup in the `serve_stream` flow so clients can
receive large snapshots without disconnecting when writes exceed the fixed
256-update capacity. Preserve the existing snapshot and live-update behavior,
and establish a safe buffering or delivery strategy that handles updates
generated while the snapshot is being sent without relying on a resume cursor.

Reject replication feeds when the journal cannot report a header version.

A journal that overrides replication_snapshot but keeps the default replication_header_version of None still opens a replication feed. persist sends no Append updates for that journal, so subscribers keep the initial image and miss later entries. The feed should fail when the header version is unavailable.

Review comment at @pkgs/id/src/world/mod.rs around lines 1006 - 1007:
Update replication-feed initialization to reject a journal when
replication_header_version returns None, before accepting its
replication_snapshot. Preserve the existing snapshot flow when a header version
is available.
