Problem

On Windows, Effect omits ino when an NTFS file ID exceeds JavaScript's safe-integer range. A replaced transcript with the same size and timestamps could therefore be treated as already imported.

The original bigint pathname fallback also allowed an ABA replacement race: discovery sees file B, the reader opens file A during a pathname swap, and the pathname is restored to B before the handle checks. The reader could accept A's contents with B's inode. This is now deterministically reproduced, rather than only a source-level concern.

Fixes #14838. The maintainer triage established the intended direction: retain the metadata shortcut, compare full file IDs, keep existing numeric/null sources decodable, and backfill old null sources once.

Change

  • Preserve file IDs as decimal strings; numeric and null stored sources remain decodable. Numeric/string comparisons remain compatible, and old null sources are read and re-recorded with a known ID.
  • If Effect supplies an inode, retain the Effect filesystem path.
  • If Effect omits it, take all pathname metadata from one bigint stat. A failed stat skips this scan instead of inventing an unknown identity.
  • For that fallback, open one scoped Node handle and take both bigint stats and all bytes from that handle. Reject a mismatched handle before reading; retain the post-read identity check.
  • Keep the completed-history shortcut before opening a transcript. Tests explicitly assert no native open for unchanged string or legacy numeric identifiers.

Effect's public File interface exposes neither the descriptor nor an exact bigint inode. The Node fallback and its narrowly scoped import-diagnostic suppression remain necessary here; the correction avoids depending on the adapter's undocumented descriptor property. No global diagnostic setting changes.

Deterministic verification

Regression coverage uses real temporary files and descriptor reads, injects IDs above 2^60, and pins creation milliseconds. It performs pathname swaps directly at the open/stat/read boundaries, without sleeps:

  • Original production source at 1fed345212c95a54328a2bd86e59510bd6c384e4, updated test: fails. The ABA case returns Importable for swappedx-session under the restored original file's ID instead of Skipped.
  • Updated source: passes. The mismatched opened file is skipped and its descriptor closes; retry imports the correct file.
  • Replacement after handle stat or during reading: the snapshot retains the opened file's identity; a subsequent scan imports the replacement with a different ID. Handles close.
  • Existing between-scan replacement, exact large IDs, legacy numeric/null sources, and unchanged-transcript shortcut checks pass.

Verified on the exact pushed commit e072154b2ccaaae04ea4bb2cb83010ad0b63432c, Linux x64, Node 24.19.0, in ChatGPT Work:

Check Result
vp test run src/project/AgentSessionScanner.test.ts src/project/AgentSessionImporter.test.ts (apps/server) 83 passed
tsc-rs --noEmit (apps/server) exit 0
tsc-rs --noEmit (packages/contracts) exit 0
vp lint on the three PR files exit 0
vp fmt --check on the three PR files; git diff --check clean

The earlier Windows source-level verification belongs to the original commit, not this follow-up. Not performed for this head: packaged-Windows end-to-end acceptance with a real NTFS replacement; a production-sized database backfill; local macOS execution. Simulated large IDs on Linux are not native Windows acceptance.

CI and approval

For this exact head, GitHub Actions CI, Mobile Fingerprint Check, Mobile EAS Preview, and Web Preview report action_required. They have not established passing platform CI. CodeRabbit is pending at this update. No human review has been submitted. Automated tests, bot reviews, workflow authorization, and human approval are separate evidence.

An earlier PR for this issue, #14853, was closed for missing model/harness attribution, not on its approach.

…g as already imported

Effect's stat leaves `ino` empty when the file ID is above
Number.MAX_SAFE_INTEGER, which NTFS IDs routinely are. The scanner stored
that as null, two nulls compared equal, and a transcript replaced at the same
path with the same size and times returned AlreadyImported without a read.

Keep the full file ID as a decimal string, reading it from a bigint stat when
Effect drops it. Stored sources may still carry the old numeric form; a stored
number and its decimal string compare equal, and sources stored as null are
read once so the importer re-records them with the full ID.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The PR is a focused Windows transcript identity fix with backward-compatible persisted data handling and targeted regression coverage. It also introduces a line-level suppression for a static-analysis diagnostic in production code, so manual review is warranted.

📝 Walkthrough

The scanner now preserves inode values above JavaScript’s safe-integer range and uses them to distinguish transcript files. The import-source contract accepts string or numeric inode values. Tests cover legacy records and transcript replacement during scanning.

Changes: Transcript identity

Layer / File(s) Summary
Preserve inode values
packages/contracts/src/agentSessions.ts, apps/server/src/project/AgentSessionScanner.ts
The import-source contract accepts string or numeric inode values. When the Effect stat result omits an inode, the scanner uses bigint filesystem stats. It normalizes inode values to strings for identity comparisons.
Check identity during reads and imports

Transcript reads check identity through the selected file abstraction. Recent-thread scanning uses the selected identity and size. Tests cover large inode matching, legacy null IDs, and replacements during open, stat, and read.

Merge Risk: 🔵 Low · up to e0721

The change fixes Windows transcript imports being wrongly skipped when a file is replaced at the same path. It does this by keeping very large file IDs exact, and those IDs persist correctly. The remaining concern is structural: the new code reads the filesystem directly instead of through the injectable filesystem service the project requires. That limits test substitution but does not break imports. This is reasonable to merge after addressing or explicitly accepting that design point.

Security architecture risk: 🔵 Low · up to e0721

The change strengthens transcript identity checks without an identified expansion of access or privileges. Existing numeric records remain compatible. Downgrade compatibility and concurrent or interrupted import recovery remain bounded areas of uncertainty.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed path affects transcript files selected for a workspace and their persisted import-completion metadata. It operates with the server's existing filesystem authority; the inspected change does not add an identity, credential, network endpoint, or privilege grant.

Trust Boundaries and Controls

  • observed — A writer able to replace a transcript pathname cannot make the fallback read an unrelated opened file merely by restoring the expected pathname before validation: statistics and reads use the same acquired handle. Regression assertions cover open-time replacement rejection and retaining the original handle's content during later pathname replacement.

Resilience and Maintainability Implications

  • observed — The native handle has scoped release, read errors are contained, and transcript byte, record, and retained-history limits remain enforced. The semaphore serializes the selected read block within one scanner instance; it does not establish cross-process import serialization.

Explanation

Issue #14838 requires correct replacement detection without re-reading every completed transcript. The current change preserves large file IDs as decimal strings, keeps numeric and null sources decodable, backfills legacy null IDs once, and preserves the completed-history shortcut. The scanner uses one bigint stat and one scoped handle when Effect omits the inode, checks identity before and after reading, and skips mismatched handles. The reported regression tests cover large IDs, legacy sources, pathname swaps, handle closure, and the unchanged-transcript shortcut. The reported test suite passes 83 tests.

Full details: Out of Scope Changes check

Explanation

The change is limited to AgentSessionScanner, its regression tests, and the AgentSessionImportSource inode schema. The schema update supports exact large IDs and legacy compatibility. The scanner changes implement issue #14838. The tests directly verify the required identity and race behavior. No unrelated change is identified in the supplied pull request evidence.

Move the native bigint stat and handle access behind a FileSystem-style service.

readTranscript and prepareRecentThreads call NodeFSP.open and NodeFSP.stat directly on the module import. make does not get this capability from its environment. Its type therefore hides a real dependency on the host filesystem.

This has two consequences:

  • The new path skips any FileSystem.FileSystem layer that callers provide. The existing tests inject such layers to remap paths, for example at test lines 471, 1103 and 2786. These remaps no longer cover inode lookup or reads when ino is Option.none().
  • The test has to use vi.mock("node:fs/promises") and spy on FileHandle methods. This mocks a module global instead of providing a test layer.

Add a small service for the missing capability. It can open a file and return an exact file ID together with readAlloc from the same handle. Wrap NodeFSP in a layer for that service, and yield the service in make. Keep the current single-handle semantics so the ABA protection still holds.

As per coding guidelines: "Dependencies come from the environment (yield* FileSystem.FileSystem), never as parameters to make … Never hide one in a module global".

Taking over this one. I’ll preserve exact Windows file identity and legacy decoding, rebuild against current main as a contract/scanner stack, trim the regression tests, and run independent reviews. Real NTFS replacement and human approval remain explicit verification gates.

Thanks @Zacharon — preserving exact file identity, legacy payload decoding, and the unchanged-history shortcut is the right direction. Rebuilt this against current Effect as the stack #21463 (backward-compatible persisted identity and consumer types) → #21464 (exact bigint-to-string scanner identity, safe legacy-number comparison, and replacement/backfill regressions).

I compared your complete diff against the stack. Current Effect already provides exact bigint stat/fstat and handle-bound reads, so the native fallback and its ABA workaround are unnecessary and were removed. The core identity fix, legacy numeric/null handling, no-open shortcut, and replacement behavior are all retained. Independent review additionally caught an unsafe numeric legacy ID colliding with an exact replacement ID; the top layer now forces a one-time reread for those values. The existing scanner/importer suites pass 84 tests, plus 2 contract tests, with scoped typechecks/lint/format. Both reviewers returned no code findings in round 3.

The replacements are published and unmerged. Configured top-layer review after draft promotion and a focused Windows CI lane are still pending; real production NTFS replacement acceptance remains unverified. Closing this PR in favor of the stack while retaining your attribution in both bodies.
