fix(usage): keep subscription limits visible across failed refreshes - #22218
Accounts can disappear from Usage → Limits when a CLI Proxy quota router disables an exhausted account or when the next usage refresh fails. A successful snapshot is replaced with an empty account list on a source failure; individual account failures are also filtered out without an account-specific notice.
Reproduction: configure a CLI Proxy usage source, obtain a successful account snapshot, then either mark that account routing-disabled or fail the next account/list request. The subscription should remain identifiable, with its last reading clearly marked stale when refreshing fails.
Change
Keep supported Claude/Codex accounts in read-only usage discovery regardless of their routing-disabled flag. Preserve last-known windows and their original timestamp across transient failures, scoped to the same source configuration and account identity, and surface account-specific failure/stale notices in the existing usage presentation.
Successful empty lists, account removal, source disable/removal, and source reconfiguration still clear prior data. Healthy duplicate reports take precedence over stale reports. Stale reset credits are stripped. Disabled accounts omit reset-credit metadata so they cannot displace an enabled redemption target. Mobile widget snapshots keep their existing unavailable-state behavior rather than presenting retained stale windows as fresh quotas; unsupported accounts do not create stray notices. Provider pickers, provider configuration, routing, and wire contracts are unchanged.
Scope and approval
This is a focused reliability fix for an established usage-reporting path: routing eligibility and failed refreshes should not erase a known subscription's usage. The server and shared presentation changes address the same disappearing-account defect. It introduces no setting, component, or new workflow and is submitted under the focused obvious-bug exception in CONTRIBUTING.md. No prior maintainer approval is claimed.
Acceptance criteria
Supported routing-disabled Claude/Codex subscriptions remain discoverable through the existing usage endpoints; redemption remains blocked for disabled accounts.
Transient source/account failures retain same-identity last-good readings with an explicit stale timestamp; first-sample failures have account-specific notices and no invented percentages.
Authoritative removal, empty results, or source reconfiguration clear cached data; settings-read failures do not erase it; recovery replaces stale readings.
Stale credits cannot create redemption actions, and healthy duplicate readings win over stale readings.
Provider pickers, routing, provider-instance configuration, and credentials are unchanged.
Verification
verified on 64ab57b2745cd1023aa7fb984d9361d3097c05fb: blacksmith vp run check:changed github/main passes format, lint, knip, graphql, 34 package typechecks, 113 tests, and react compiler. both regressions fail with their respective guard removed and pass with it present. two independent reviewers approve this head with no findings. all 28 reported ci checks pass; github reports clean and mergeable; no unresolved review threads.
playwright opened the app and usage dialog. live usage returned codex json-rpc -32600, so live hub retention and native-client acceptance remain unverified. no integration acceptance is claimed. task dev server and blacksmith worker are stopped.
Implementation and verification: gpt-6.1-sol through the Codex harness in T3 Code.
Macroscope's review found this PR approvable — This is a focused, tested reliability fix for an existing usage-reporting path: stale limits remain visible with explicit notices, stale redemption data is removed, and source/account reconfiguration clears retained state. A separate high-severity finding identifies a remaining disabled-account redemption risk, which is reported independently.

Usage-limit reads now include disabled Codex and Claude accounts. Source snapshots retain matching account data when probes fail. Shared aggregation displays stale windows with failure notices and excludes unavailable reset-credit data and redemption actions. Mobile snapshots exclude unavailable accounts from limit pools.

Summary
Report usage for disabled accounts apps/server/src/usage/cliproxyApi.ts, apps/server/src/usage/cliproxyApi.test.ts
Cliproxy reads include disabled Codex and Claude accounts. Tests cover usage, failed reads, credit selection, and blocked redemption.
Retain source snapshots across probe failures apps/server/src/usage/UsageLimitSources.ts, apps/server/src/usage/UsageLimitSources.test.ts
Source snapshots retain matching account data when reads fail. Cache entries are removed when sources are disabled, removed, or reconfigured. Tests cover recovery, identity changes, settings failures, and configuration changes.
Aggregate unavailable and stale limits packages/shared/src/usageLimits.ts, packages/shared/src/usageLimits.test.ts, apps/mobile/src/widgets/subscriptionUsageSnapshot.ts, apps/mobile/src/widgets/subscriptionUsageSnapshot.test.ts
Aggregation prefers healthy reports, retains stale windows with failure notices, and removes unavailable reset-credit data and redemption actions. Mobile snapshots exclude unavailable accounts from limit pools.
The test may miss stale cache restoration after a source is disabled and restored. Synchronizing both events would make the protection reliable.
Security architecture risk: Low · up to adb38
Usage remains visible during failures without carrying forward stale credit actions. Configuration and account matching limit snapshot reuse, and disabled accounts retain a separate redemption restriction. Remaining uncertainty concerns concurrent configuration changes and completeness of downstream coverage.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
observed — The expanded read scope includes supported routing-disabled accounts within each configured hub. Reads use the existing management bearer credential and fixed provider usage destinations; the PR does not add a caller-selected upstream destination through account discovery.
Trust Boundaries and Controls
observed — Usage visibility does not itself grant hub redemption eligibility. Disabled accounts skip credit retrieval, and the consume operation independently reloads the account list and rejects missing, disabled, or non-Codex accounts. Source redemption also requires a currently enabled source with a management key.
observed — The existing redemption request contains source, account, and credit identifiers without a configuration version. The refresh semaphore does not make settings updates atomic with an already-started provider mutation. This limitation predates the PR; the inspected diff does not change that mutation path or expand its input authority.
Resilience and Maintainability Implications
observed — The unchanged redemption flow derives a repeatable request identifier from account and credit identity. Reset and already-redeemed outcomes attempt hub cooldown cleanup; cleanup failure returns a warning rather than representing the credit redemption as undone.

Full details: Description check
Explanation
The description follows the template's main sections (Problem, Change, Scope and approval, Verification) and gives specific reproduction steps, the scope rationale, acceptance criteria, and a detailed verification report. The author explicitly states that live hub retention and native-client acceptance were not verified, and claims no prior maintainer approval, relying on the 'obvious-bug' exception in CONTRIBUTING.md. The template requires either a linked triaged issue or maintainer-approved discussion, or a justification for the small-focused-fix exception. The author provides a justification, but the change touches server, shared, and mobile code across several files (about 400+ lines) and adds retention behavior, so whether it qualifies as a 'very small, focused fix of an obvious bug' is debatable. The PR also does not include a conventional-commit-style title in the description itself (the title is not provided in the description). The objectives note that a later comment references a commit and changes after the description was written, which is not reflected in the description. These gaps are mostly non-critical, so the description is mostly complete.

Full review and local verification for adb38d0fc7705d7ec359290f5ec33a581abb12fd: two independent Codex/GPT-6-Luna medium lanes (spec and standards) inspected all eight changed paths and returned no findings; both native inspection records verified.
The disabled-account redemption issue is fixed in 68ce889, with actual API results tested through both shared selectors. The mobile widget CI regression and unsupported-source notices are fixed in adb38d0; the original mobile assertion is preserved. Parent independently reran all 126 focused tests (104 server/shared + 22 mobile). Targeted lint and shared/server/mobile typechecks pass.
This is source/local verification, not upstream CI or installed-runtime acceptance. Fresh CI is required on this head. No provider/picker settings or running sessions changed.
merged current main (2756da402b) without rebasing. the only conflict was in apps/server/src/usage/UsageLimitSources.test.ts: this branch moved the SecretResolver test layer, and main added a label field to that mock (#31670). kept the move and added label to all three SecretResolver mocks in the file. no production code changed.
verified on c72189b2a5: blacksmith vp run check:changed github/main passes (format, lint, knip, typecheck, 123 tests), and vp test run src/usage passes 172 tests in apps/server. all review threads were already resolved. ci is green on this head and github reports it mergeable. live hub retention and native-client acceptance remain unverified.
