Refs #27773

A first-request Codex usage-limit failure can arrive before a canonical quota notification, leaving the terminal and recovery card without a reset time. Read account quota when that reset is missing, share the bounded request across overlapping failures, then publish the terminal. Complete reads replace omitted windows. Concurrent notifications preserve newer usage and reset invalidation through one atomic snapshot/history state; reads for a replaced plan are discarded. Sharing caps bypass ordinary quota recovery. Read failures and non-interruption defects preserve terminal publication.

The adapter also preserves retry metadata during late enrichment and rechecks after registering the failure item. Reconciliation incorporates reasoning discussed by budres in #28291. Maria supplied the first R16 correction; the follow-ups guard reset invalidation and plan changes.

Verified head e06ccb1ac4bab44a45b8a7033ad1d618a95a7d2f, comparison base 7ef841dcf7c9f48b950b6d2b02e48b44e2be3265: fresh native main/head recordings, web recovery recording, persisted milestones and check:changed output. All check:changed stages passed, including 193 tests and 26 package typechecks. Required GitHub checks pass or are conditionally skipped; all eight review threads are resolved. An independent review found no quota/recovery overlap or supersession in newer main c52df24d285a072ad9c3f7ab5b3656c0be1cb0f5.

The new native comparison uses the iPhone 17e simulator on shiv, iOS 27.0, development-client fingerprint 316afd351f76b38d7c29e47460801c335423fdc1, and corex source servers. Main shows the first-request failure without a reset. Head recovers the reset, arms auto-resume before expiry, and completes the automatically submitted continuation. The native worker submitted it 466 ms after the returned reset; web submitted its continuation about 25 seconds after its reset. The clients, server, Codex adapter, persistence and worker are real. The external Codex JSONL process supplies a hand-built usage-limit failure and quota read. These recordings prove the synthetic provider flow.

Earlier Boat red/green and independent review retain their e5c699b attribution. All six PR files remain identical to d5f94b4. The fresh captures cover the recovery UI inherited from newer main.

Unproven: a genuine authenticated upstream first-request Codex 429 and continuation at the account's actual reset. The laptop Mac's existing ChatGPT Pro account reports 100% primary usage and reset 2026-11-11T07:19:40Z, but minimal Luna and GPT-6.1 Sol turns both completed successfully without a quota failure. No test environment was signed into an account and quota was not deliberately exhausted. The POCO Android capture, desktop shell and relay/tunnel transports were not refreshed; the new iOS comparison exercises the shared native recovery flow. Full details and the sanitized real-account outcome are in the linked evidence comment.

Known inherited limit: after a failed bounded read, a notification arriving after settlement can update provider usage without persisting a reset into the settled failure. Recovery for that fallback is not claimed.

Implemented by Opus 5.5 through T3 Code, with review follow-up by GPT-6.1-sol through Codex. Tested and independently reviewed by GPT-6.1-sol through Codex in T3 Code.

Approvability

Verdict: Approved at e5c699b

Macroscope's review found this PR approvable — Codex usage-limit failures now fetch missing reset times and reconcile them with live quota notifications before publishing the terminal event. The bounded, read-only fallback changes usage reporting rather than quotas or entitlements.

Walkthrough

Provider usage-limit updates now support replacing the full set of windows. The Codex adapter reconciles account rate-limit reads with notifications and uses available reset times when publishing usage-limit failures.

Changes: Codex reset-time recovery

Layer / File(s) Summary
Define complete window replacement
packages/provider-core/src/server/snapshot.ts, packages/provider-core/src/server/usageLimits.ts, packages/provider-core/src/server/usageLimits.test.ts
Usage-limit updates accept replaceWindows. When enabled, omitted windows are removed, and an empty update can clear all windows. Tests cover replacement and unchanged results.
Reconcile rate-limit reads and notifications
apps/server/src/provider/codexUsageLimits.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
The adapter merges notifications and applies read snapshots as complete window sets. Reads are reconciled with notifications received while pending, and concurrent callers share one read. Tests cover stale and complete reads, as well as concurrent updates.
Publish usage-limit failures with reset data
apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
When a usage-limit failure lacks reset data, the adapter waits for the shared read before publishing. Available reset times are added to failure items. Tests cover read errors, callback defects, retry metadata, and overlapping failures.

Explanation

Issue #27773 requires a Codex usage_limit failure to carry resetAt when the reset is obtainable, including when a run's first request is rejected with a 429. The diff from merge base 12ec17c to head a18548c implements this in apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts. When a usage_limit terminal has no resetAt and the rate-limit snapshot has no reset, emitRootTerminal sets awaitsReset. It then runs readAccountUsageLimits, which calls account/rateLimits/read with a 10 second timeout, and publishes the terminal after that read. Read failures and non-interrupt defects are logged and the terminal is still published. Interruption propagates because the catch excludes causes with interrupts. Overlapping terminals share one pending Deferred read. publishRootTerminal records the limited-turn failure item with retry metadata and calls enrichLimitedTurnItems after registration, which covers a late reset. In apps/server/src/provider/codexUsageLimits.ts, reconcileCodexRateLimitsRead keeps fields that notifications changed during the read. Covered behavior is tested in CodexAdapterV2.test.ts per the PR description. The issue names the turn error message as a second possible source. The PR does not parse it, but the issue states account/rateLimits/read as a valid source, so this does not leave a coding requirement unmet. Known gap: when the bounded read fails, a notification after run settlement does not persist a reset to that settled run. The PR discloses this as existing consumer behavior, and the primary read path meets the requirement. The live authenticated quota path is unverified, but that is evidence scope, not a missing code requirement.

Full details: Out of Scope Changes check

Explanation

The changed production files support #27773. CodexAdapterV2.ts implements the quota read, terminal ordering, shared read, failure-item registration, and late enrichment. codexUsageLimits.ts adds reconcileCodexRateLimitsRead and laterReport to merge a read with newer notifications. packages/provider-core/src/server/usageLimits.ts and snapshot.ts add an optional replaceWindows flag so a complete read can remove omitted windows from the published provider usage. The flag defaults to the existing merge behavior, so other callers keep current semantics. Tests in provider-core and the adapter test file cover these paths. The PR description states that no client, wire-schema, other provider adapter, shell, or transport code changed. The diff inspections for the adapter and codexUsageLimits.ts show no unrelated changes. Changes to other packages were not inspected in this assessment beyond the AI-generated summaries.

[GPT-6.1-sol][Codex] on behalf of shivam.

Evidence finding 1 is addressed in the new before recording in the PR body. Physical corex ran main 9b1d351ba0f63f527f884ffb3f2e394f50388764; POCO F4 Android 14 used native client fingerprint 8006860c656de3319b4b47d3bab69506af6a0809 and Metro from that checkout. Synthetic Codex protocol responses reproduce the first-request failure, and the video shows Send, the failure, and the recovery card without a reset time.

Evidence finding 2 remains blocked. I launched the real Codex 0.166.0 app-server in an isolated home using corex's mcli provider pinned to the already-exhausted account. No model turn was sent. The actual RPC transcript was:

initialize: ok
account/read: account=null, requiresOpenaiAuth=false
account/rateLimits/read: error -32600, codex account authentication required to read rate limits

The mcli status snapshot at 2026-10-30T20:14:07.998Z reports that account's weekly window has 0% remaining and resets at 2026-11-04T06:35:47.000Z. The supported mcli route provides model access without a Codex account login, so it cannot return the ChatGPT account quota response this change consumes. I did not copy OAuth credentials around mcli, spend quota to force a limit, or substitute synthetic responses for the requested live proof. The existing after recording remains explicitly synthetic.

Ready evidence is incomplete until the live account quota path and actual reset resumption can be exercised. No code changed.

Takeover fixup

  • Fixed the open Macroscope finding: a reset-less quota notification during the pending account/rateLimits/read no longer drops the read's reset (65d61c3, plus one regression row that fails before the fix).
  • Merged current main with a merge commit (9dd10f1). No conflicts.
  • vp run check:changed on Blacksmith: format, lint, knip, graphql, typecheck (26 packages), tests (4 files, 180 tests) pass.
  • Live quota recovery against an exhausted account remains unverified, as below.

Earlier verification (gpt-6.1-sol, head c7cce88)

Checked exact head c7cce8818cc4a1511e13d7abc53dcf616493b50d on this disposable Linux VM. Installed with vp i and ran one background dev server at a time. Baseline was fetched github/main at 15fe283272 in a separate worktree. Both final verification servers used isolated homes.

Drove the real web client using repository Playwright and /usr/bin/google-chrome. Added a project pointing at the real repository through the UI, created threads, selected Codex gpt-6.1-sol, and sent real turns through the local proxy. The identical prompt completed on both main and the PR. On the PR, a follow-up completed, Stop generation interrupted a streaming turn with "Stopped by user", and another turn in that thread completed afterward. Reload preserved the interrupted turn and subsequent reply. No mocks, fixtures, or synthetic provider responses were used for this app verification.

Main normal-turn baseline, not a reproduction of the quota bug:

main real Codex turn completed

PR same prompt completed, not proof of reset recovery:

PR real Codex turn completed

PR streaming cancellation and successful next turn, recorded at normal speed:

PR stop generation and subsequent real Codex turn

Ran existing focused checks through Blacksmith:

vp test run apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts -t "classifies Codex terminal failures"
1 file passed; 11 tests passed; 107 excluded by the name filter.

These replay-based checks support the implementation but do not prove live quota recovery. Observed PR CI: 31 successful, 0 failing, 0 pending. No source changes or pushes.

Not verified

The changed behavior remains unverified. I could not reproduce the first-request usage-limit bug on main or prove reset recovery on the PR. This VM's real Codex proxy configuration has model access without an authenticated Codex account quota endpoint. Direct real app-server RPCs returned:

initialize: ok
account/read: {"accountPresent":false,"requiresOpenaiAuth":false}
account/rateLimits/read: {"code":-32600,"message":"codex account authentication required to read rate limits"}

A live exhausted account, canonical account quota snapshot, failure-card reset time, bounded failed-read behavior during an actual usage-limit failure, and automatic resumption at the live reset were not exercised. Normal successful turns do not enter this PR's recovery branch. I did not exhaust an account or substitute synthetic responses. Verification requires an authenticated account that can return live account limits and exhibit the first-request failure.

Native Android/iOS and desktop shells, Windows/macOS, relay/tunnel connections, and other providers were not exercised. The change is server-side Codex behavior; no client or contract changes are in this PR.

Evidence delivery check: reopened this comment. GitCafe currently renders the external media as links. Both uploaded images load at 1440×1000, and the uploaded video plays and seeks successfully in T3's browser. Both dev servers and the task's Blacksmith worker have been stopped.

Refreshed verification for head 3256f53819ddf04b67fce823d3ebc62bfa4170e1, rebased on main cefb544af3c98224874419198c0359b652ca3c94. Main still lacks the missing-reset recovery read. The refresh also fixes complete-snapshot omissions, newer-notification races, same-window credit exhaustion, and overlapping reads. Independent review found no remaining code findings.

Both recordings use the native Android dev client on POCO M3 Pro, native fingerprint 971743703b097b8870b39ff842d130f50b115eeb, connected to isolated corex source servers. The Codex adapter, event persistence, and recovery worker are real. Codex's JSONL process is a hand-built fixture that returns a first-request HTTP 429 usage-limit failure, a premium-only notification, and a canonical Codex account quota snapshot.

Before, current main. Send triggers the quota failure and the card has no reset time, requiring a manual retry.

before-main.mp4

After, current head. The card recovers 2026-11-03T15:50:01.000Z. Tapping Resume at reset arms auto-resume. The real worker submits Continue where you left off at 15:50:05.277Z; the continuation completes and checkpoints at 15:50:05.538Z. The recording shows the automatically submitted continuation. The fixture's final agentMessage item lacked required completedAtMs during this capture, so its reply is absent from the screen. The attached stored-event transcript identifies the completed run. This recording does not claim a rendered final provider reply.

Unproven: an authenticated, genuinely exhausted Codex account returning a live upstream reset and continuing at that live reset. The current real app-server probe still returns accountPresent=false and -32600 codex account authentication required to read rate limits. Model access through mcli is available; the account quota endpoint is not. The native capture proves the application flow with a fixture, not the upstream service. Maria's earlier successful-turn report at comment 6548003029 describes the old head and is not used as current-head proof.

No client or contract code changed. Other provider adapters are unchanged. Desktop/web shells and remote relay/tunnel transport were not exercised at this head. A Device-panel Pixel 9 follow-up could not install: no cached x86 client and the configured Java path was missing. The physical-phone before/after recordings above are the native-client evidence.

Review regressions: R11's dying usage callback lost the terminal before its fix; the new row failed at the project timeout, then passed after the fix. R12's replacement test failed with 1 failed and 7 passed before the fix, then all 8 passed. The adapter complete-read assertion also failed when replacement was disabled. R13's controlled experiment inserted a temporary scheduler gap between terminal emission and item registration: enrichment was lost without the recheck, and passed with it. That artificial gap was reverted; the committed test gates the late notification with a Deferred and contains no sleep or polling. These experiments ran before the mechanical rebase; the final Boat output above reruns the committed regressions.

Native Android recordings and persisted recovery transcript retain their original commit attribution. They show the successful synthetic 429/read/reset/automatic-continuation path on server 3256f53819ddf04b67fce823d3ebc62bfa4170e1; they are not final-head recordings. Current-head tests cover the later replacement and error/race branches. Final-head iOS screenshot and persisted recovery events show the successful continuation and rendered final reply. The video export failed, so no complete final-head recording is claimed.

Unproven: first-request exhaustion, upstream quota recovery, and reset-time continuation using an authenticated real Codex account. A fresh quota RPC probe still returns -32600: codex account authentication required to read rate limits. This evidence uses a hand-built 429, as authorized, and does not claim live upstream proof. The cancelled j_je6vswvirhnie5i3t3f5 job is not a pass. Earlier comment 6548003029 does not establish current-head quota recovery.

Nitpick Add a span or a reason for Effect.fnUntraced on emitRootTerminal.

5934-5934: 📐 Maintainability & Code Quality | 🔵 Trivial | 💤 Low value

Add a span or a reason for Effect.fnUntraced on emitRootTerminal.

This function runs once per run terminal. It reads the rate-limit snapshot and forks the reset read. It does not run per event or per chunk.

publishRootTerminal now carries the Effect.fn span. Use a named span on emitRootTerminal as well. If you keep fnUntraced, add a comment that gives the reason.

Proposed fix
- const emitRootTerminal = Effect.fnUntraced(function* (
+ const emitRootTerminal = Effect.fn("CodexAdapterV2.emitRootTerminal")(function* (

As per path instructions, {apps,packages,infra}/**/*.ts: "Hold changed code to the rules in docs/internals/effect-services.md". As per coding guidelines: "Default to named spans for service operations and I/O. Use Effect.fnUntraced deliberately... Explain the reason beside the function".

Review comment at @apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts
at line 5934:
Update emitRootTerminal to use a named Effect.fn span, consistent with
publishRootTerminal, since it runs once per terminal rather than per event or
chunk. If retaining Effect.fnUntraced, add a nearby comment explaining why it
must remain untraced.

Current-head native iOS verification, server and Metro 43bb8fd0e2cc1a513cde92220cb8076b5e2ec0d9, base b7ab02e0f6077a72cadfa5c9a40db4db67bc325f. iPhone 18 Pro simulator on shiv, iOS 27.0, cached native development fingerprint 887f84d7e8ee2e1063658bf884c9d17f60d6cbb3. Native sources are unchanged by this PR; the native cache was installed from main cefb544af3c98224874419198c0359b652ca3c94. The simulator connected directly to the isolated corex source server.

I drove Send, observed the first-request synthetic 429, the recovered reset card, and pressed Resume at reset. The server persisted reset 2026-11-03T17:08:47.000Z, armed recovery at 17:08:31.831Z, committed the automatic continuation at 17:08:47.371Z, sent Codex's turn/start at 17:08:48.043Z, and completed the continuation and checkpoint at 17:08:48.136Z. I inspected the app's rendered final reply: “Resumed at the reset time. Quota recovery completed.”

Final-head iOS continuation and final reply

The final-head video export disconnected with ECONNRESET. The completed recording was no longer available after reconnect, so this is a screenshot and runtime transcript, not a complete final-head recording. The existing before/after Android recordings remain linked with their original commits in comment 6608779510.

The protocol process is a hand-built Codex fixture; the native app, adapter, server, persistence, and recovery worker are real. Unproven: authenticated upstream quota exhaustion/read/reset-time continuation. Corex's fresh real quota probe returns -32600, codex account authentication required to read rate limits. No accounts were signed into the test environment.

Final-head runtime follow-up for R12 and R13 on corex. The source server is 43bb8fd0e2cc1a513cde92220cb8076b5e2ec0d9, base b7ab02e0f6077a72cadfa5c9a40db4db67bc325f, with isolated state. A headless client used the actual WebSocket command API and subscribed to actual provider and thread streams. The external Codex JSONL process is a hand-built fixture.

R12 passed through the real publication path: a notification published an exhausted primary window without a reset, then a complete quota read replaced it with only the secondary window and its reset. The first witness missed the primary state because the server coalesced the immediate updates. Giving the synthetic read one second of network latency made both published states observable. No source code was changed for this experiment.

The R13 adapter race is covered by the committed Deferred regression and controlled-gap experiment in the test evidence. A separate full-server experiment with a failed read followed by a notification 1.5 seconds later did not persist late enrichment into the already settled failure item. The terminal persisted as usage_limit with resetAt null, and the provider usage snapshot later received the reset. RunExecutionService stops per-run ingestion after terminal settlement, and ProviderSessionManager directly persists only runless request artifacts. Those consumers are unchanged from main. This PR fixes recovery when the bounded quota read succeeds before terminal publication; it does not fix arbitrary post-settlement enrichment after that read fails. The earlier wording about late enrichment refers only to adapter event emission, not verified persistence or automatic recovery.

Fresh real account probes on both corex and shiv returned accountPresent false and -32600: codex account authentication required to read rate limits. The laptop SSH connection timed out. Phones and simulators are viewing clients of these environments, and Boat supplies model access without a separate authenticated quota account. No live quota account was available, and none was signed into a test environment. The synthetic first-request successful-read recovery and actual automatic continuation were demonstrated in the linked native evidence; live upstream exhaustion remains unproven.

R14 follow-up at head a18548c8ad6266d2c862f9598a7486bad03e1624, rebased mechanically onto main 12ec17ce4c6481a8ea1049dedd78d0e56bc48aec. Main still has no quota recovery read. None of the eight newer main commits touches this PR's quota code. git range-diff reports all nine patch commits unchanged by the rebase.

The existing late-reset table row now starts a provider retry, advances TestClock before terminal publication, then receives the Deferred-gated late reset. Without the metadata fix, its assertion fails because the enriched error item loses retry attempt 1. With the fix, it preserves both retry state and the original retry start time. The named emitRootTerminal span also addresses the tracing follow-up.

These red/green runs preceded the mechanical rebase. The pinned final-head Boat results follow below. The native and runtime evidence in comments 6608779510, 6610669777, and 6610786026 keeps its original commit attribution. The last patch only preserves retry metadata on enrichment and adjusts tracing. The recorded successful-read fixture path does not have a provider retry, so its result is unchanged. The live authenticated quota path and the read-failure post-settlement limit remain as disclosed.

Fresh app recording at live head a18548c8ad6266d2c862f9598a7486bad03e1624, base 12ec17ce4c6481a8ea1049dedd78d0e56bc48aec. Chromium in T3’s Browser panel, connected directly to the isolated source server on corex. The server, Codex adapter, WebSocket client, persistence, and recovery worker are real; the Codex JSONL subprocess is the hand-built 429 fixture. This supplements the native phone recordings and iOS screenshot, which retain their original commit attribution.

The recording begins before Send, shows the first-request usage-limit failure, a recovered reset at 17:51:53Z, Resume at reset being armed (Cancel auto-resume appears), and automatic continuation with the rendered final reply. The server committed “Continue where you left off.” at 17:51:53.498Z; Codex turn/start followed at 17:51:55.433Z; the continuation and checkpoint completed at 17:51:55.480Z. I inspected the flow, final screenshot, recording frames, and persisted events.

The provider response is synthetic; this does not claim an upstream 429 or continuation at the live account’s reset. The authenticated Mac probe is documented separately in comment 6611352751.

Fresh native phone recording at head a18548c8ad6266d2c862f9598a7486bad03e1624, base 12ec17ce4c6481a8ea1049dedd78d0e56bc48aec. This replaces the missing current-head native after capture.

I used the iPhone 17e simulator on shiv, iOS 27.0, with the cached native development client fingerprint 887f84d7e8ee2e1063658bf884c9d17f60d6cbb3, installed through vp run mobile:install ios 7DC03B72-546B-4090-80D3-E225C64AA1E0. The native shell came from unchanged native sources on main cefb544af3c98224874419198c0359b652ca3c94; Metro and the corex source server both ran a18548c. The app connected directly to isolated server state.

The recording shows entering the trigger, Send, the first request of the new run failing, the recovered reset card (23:33:55 Asia/Kolkata, 18:03:55Z), arming Resume at reset, and automatic continuation with its completed reply. The server persisted the failure with that reset at 18:03:20.143Z, automatically committed “Continue where you left off.” at 18:03:57.759Z, and completed the reply/checkpoint at 18:03:58.803Z. I inspected the phone, exported recording frames, screenshot and persisted events. Earlier test messages in the same disposable thread remain visible above this new trigger.

The native app, server, adapter, persistence and recovery worker are real. The Codex response is a hand-built fixture. The before recording on main, showing the same native recovery card without a reset, is linked in comment 6608779510.

The evidence gate accepted current-head tests but rejected the earlier native captures and the synthetic upstream response. This new recording addresses the stale native capture. The separate live-upstream gap remains: existing Codex homes on both corex and shiv return accountPresent:false and quota RPC error -32600 (these fresh probes include the actual existing homes, not only isolated mcli configuration). The laptop Mac’s existing account authenticated and returned its live exhausted window, but a Luna turn completed rather than returning 429 (comment 6611352751). A minimal Sol probe could not run after the Mac’s SSH connection timed out, both directly and through corex. No reset credit was consumed and quota was not deliberately exhausted. The pool iPhone failed to boot; the separate iPhone 17e used for this recording worked. Boat supplied pinned tests, not a live authenticated quota account.

Fresh installed evidence-gate result after this native capture: head, before_after, tests and unverified pass; both_apps is not applicable. The only failure is real_environment, because the recovery provider responses are synthetic. No code or GitHub review finding remains open. Additional configured Codex homes /home/shivam/.codex-p and /home/shivam/.codex-vm do not exist and cannot start the provider.

Live-provider access rechecked on 2026-11-04 at 00:50 UTC. PR head remains a18548c8ad6266d2c862f9598a7486bad03e1624, base 12ec17ce4c6481a8ea1049dedd78d0e56bc48aec; required checks pass and all review threads are resolved. No source changes or new app capture in this recheck.

Mac SSH recovered. Its existing authenticated Codex CLI 0.166.0, using the upstream OpenAI provider and existing default account, still reports a 100% used weekly window resetting at 2026-11-04T07:19:38Z. A single ephemeral, read-only gpt-6.1-sol turn with “Reply OK. Do not use tools.” completed successfully. This completes the Sol probe previously blocked by SSH, but does not reproduce a 429. Earlier Luna also succeeded. I did not sign in, copy credentials, deliberately exhaust quota, or request reset-credit redemption.

Sanitized results, obtained directly through the real Codex app-server RPC rather than the T3 adapter:

{"host":"Mac","accountPresent":true,"rateLimits":{"limitId":"codex","primary":{"usedPercent":100,"windowDurationMins":10080,"resetsAt":1793776778},"secondary":null,"rateLimitReachedType":"rate_limit_reached"},"model":"gpt-6.1-sol","turnStatus":"completed","turnError":null}
{"host":"corex","accountPresent":false,"quotaReadErrorCode":-32600}
{"host":"shiv","accountPresent":false,"quotaReadErrorCode":-32600}

Unverified: a genuine first-request upstream Codex 429 through main and this head, followed by upstream-reset recovery and automatic continuation at that reset. The current-head native fixture recording and evidence-gate verdict remain in #27791 (comment). Boat and working native clients cannot supply a quota-account failure that these real requests do not reproduce.

…eset time

When the first request of a run hit Codex's usage limit, Codex had only pushed a
model-specific rate-limit snapshot, so the failure had no reset time and could not
auto-resume. A usage-limit terminal with no reset now waits on its own fiber for one
shared, bounded account/rateLimits/read and publishes with the codex reset. A failed,
timed-out or defective read still publishes the original failure; interruption ends it.

The read replaces the snapshot as of its request, so windows and resets it omits are
cleared in the published usage cache (replaceWindows), while fields a notification
changed during the read stay; a lower same-reset report is a late tick and loses to the
read, following budres's same-window reasoning in #28291. Late resets after the
terminal still enrich its failure item, keeping retry metadata.

ChatGPT sharing caps never take this path: their reset is not in the ordinary allowance
snapshot, so they publish at once with no reset and never read or inherit one.

Fixes #27773

Rebased quota recovery onto main 935818e422f9b58c46e14d70f4ef386fa3575105, preserving sharing-cap handling. Current head ff265e82d4cab6d511585b1f8ee2334b8545ebf0 also fixes R15: an identical notification during a quota read no longer discards its recovered reset. The new primary-window row fails on the prior integration with resetAt: null; the final regression rows cover primary, secondary, and individual windows. Two independent review rounds are clear.

Boat job j_w5tbmbafsng65d74pblh completed with exit 0 on this exact head. The full adapter, quota helper, managed-error, and provider-core usage tests passed 183/183. vp run check:changed 935818e422f9b58c46e14d70f4ef386fa3575105 passed format, lint, knip, GraphQL, 26 package typechecks, and 179 tests. No repo-wide checks were run.

Unproven: a genuine upstream first-request 429 and automatic continuation at its actual reset. The Mac laptop is reachable and its existing authenticated Codex account reports ordinaryUsageAllowed:true and 59% weekly usage, so it currently supplies no quota failure. No account was signed into the test environment and no quota exhaustion or reset credit was requested. The phones and simulator can exercise the clients but cannot supply an exhausted provider account. Boat supplies Linux verification, not an authenticated exhausted subscription.

Native comparison on iPhone 17e, iOS 27, cached development fingerprint e8f78d5e730353790c0a572575c387141109cfa4. Metro uses the integrated native sources, which are identical at 5520920 and ff265e8. Servers run from main 935818e and final ff265e8 on corex, each with isolated state and the same hand-built Codex usage-limit response. Only test data is visible.

Before: typed first request and missing-reset/manual-retry card on main.
Current main native trigger and missing reset

After: typed first request, adapter account/rateLimits/read, persisted reset, and native recovery card with the returned reset on ff265e8. This clip does not show arming or automatic continuation. The device transport failed with "Remote daemon is unavailable"; a same-session refresh restored it, but a fresh full-flow attempt then hit "Runner command deadline exceeded". Its advised diagnostic retry reported an expired ref frame. Verification stopped, without switching UI automation systems.
Final-head native trigger and recovered reset, without auto-resume

The earlier complete auto-resume recordings remain attributed to a18548c in comments 6611431750 and 6611862443. They are not fresh verification of this head. Web/desktop shells, relay/tunnel transport and a complete native auto-resume recording were not refreshed at ff265e8. Main later advanced to 3470164 only through the unrelated web Working-shelf revert #31269; it did not change the adapter, provider-core or native code tested here.

Known inherited limit: when the bounded read fails and the quota notification arrives after run settlement, provider usage changes but the settled failure item does not acquire a persisted reset. Recovery through that fallback is not claimed.

The current-head sharing-cap runtime probe uses an exact managed code followed by an ordinary quota notification. It publishes subscription_sharing_usage_limit_exceeded with resetAt: null and makes zero account/rateLimits/read requests on its turn driver. The 183-test run also asserts this distinction.

Fresh corex and shiv probes also returned accountPresent:false and quota JSON-RPC -32600, authentication required. No login or credential copying was attempted.

A quota notification that arrived while the first account read was pending
could report an exhausted window with no reset. Reconciliation kept the
notification, dropped the read's reset, and the usage-limit terminal lost
its auto-resume time.

The read now fills a reset the notification left unknown, but only when
the request snapshot never had a reset for that window, the window
duration and plan match, and usage has not dropped. A notification that
cleared a known reset, resized the window, changed plan, or rolled over
keeps its own state.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
When a quota notification gave a window a reset and a later one dropped it
while the first account read was pending, the read's reset was filled back
in. The request snapshot never had that reset, so reconciliation could not
tell a cleared reset from one Codex had not yet reported.

The adapter now records which windows a notification gave a reset since the
pending read's request. The read fills a missing reset only for windows
neither the request nor any notification during it had a reset for; the
duration, plan, and rollover guards are unchanged.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
The snapshot and the resets notified during a pending read lived in separate refs, so a
notification could land between a read taking its notified set and applying it, letting the
read restore a reset that notification had just cleared. Both now change in one Ref.modify.

Final source verification at head 9d81fba410cad184e00a2ec8f369af78efaaccd4, base 67eebaca23f8f7bc46e8817434087fc9c5acf8e3, on 2026-11-05. This retains Maria’s R16 correction and adds guards against borrowing a stale reset after a notification clears it, including when values return to the request snapshot. Snapshot and notification history now change atomically in one Ref. Fresh independent Codex review found no remaining R16–R18 issues.

Boat job j_hz2ubchjfqlteqmbhulb completed with exit 0 on this exact head: 192 focused tests in the complete adapter, quota helper, managed error and provider-core usage files. vp run check:changed 67eebaca23f8f7bc46e8817434087fc9c5acf8e3 passed format, lint, knip, GraphQL, 26 package typechecks and the changed-file tests. No repo-wide checks were run.

The real web client connects to the corex source server at this head with isolated state. Its external Codex process is a hand-built protocol fixture, never an upstream service. The fixture sends an empty premium notification, fails the first request, sends a canonical window without a reset during account/rateLimits/read, then returns a reset. The recording shows the trigger, recovered card, arming auto-resume and the automatic continuation. The final rendered reply is captured separately below; the video stops while that reply is being painted. The fixture reset was 04:43:05Z; the server committed its continuation at 04:43:06.972Z and completed it at 04:43:07.762Z.

web-atomic.mp4

Final-head completed reply

Unproven: a genuine upstream first-request Codex 429 and continuation at the real account’s reset. The existing Mac account authenticated but allowed ordinary usage (61% weekly usage); corex’s existing account probe returned no account and quota RPC -32600. No account was signed into a test environment and no quota was deliberately exhausted. Boat cannot supply an authenticated exhausted subscription.

Native refresh: POCO F4 serial c39d2814 is not exposed by the available Device tool ("No device c39d2814"). The alternate iPhone 17e on shiv has matching native fingerprint e8f78d5e730353790c0a572575c387141109cfa4, confirmed by mobile:install from the final-head checkout. Native capture is not complete: opening its Metro link failed and the transport reported "Remote daemon is unavailable". Its advised debug retry also failed: xcrun simctl openurl did not answer within 20000ms; verification stopped. Earlier native before/after captures retain their original commits in comment 6648982589. They are not current-head native evidence. Desktop shell and relay/tunnel transport were not refreshed.

Known inherited limit: after a failed bounded read, a notification arriving after settlement can update provider usage without persisting a reset into the settled failure. This PR does not claim recovery for that fallback.

R19/R20 are fixed at head e5c699b2c93347df560452dc28a13fef74f80aa9, base 67eebaca23f8f7bc46e8817434087fc9c5acf8e3. If a notification changes the plan while a quota read is pending and that read reports a different plan, reconciliation now retains the newer snapshot as a whole. It cannot import the old plan's reset, higher usage, new windows or individual limits. A read already reporting the new plan still reconciles normally.

Boat job j_whepx6afntrarc7gxbkn completed with exit 0 on this exact head. Using the committed tests with the helper from prior head 9d81fba410cad184e00a2ec8f369af78efaaccd4 produces five expected failures (primary, secondary, higher usage, newly present window and individual limit), each exposing the stale reset; the matching-new-plan case passes. Restoring the final helper produces 197 passing tests across the complete adapter, quota helper, managed-error and provider-core usage files. vp run check:changed 67eebaca23f8f7bc46e8817434087fc9c5acf8e3 passed all stages including 26 affected package typechecks. The pre-commit format/lint/knip hook passed, and the fresh second Codex review cleared R19/R20 with no new findings. No repo-wide checks were run.

The final delta changes only the plan-mismatch branch. The real-client fixture recording, screenshot and persisted recovery milestones in #27791 (comment) keep their 9d81fba attribution; their unchanged-plan path is unchanged by this commit, and the final-head table tests the new branch and matching-plan recovery. This is not a fresh native capture or a live-account 429 demonstration.

Unverified: a genuine upstream first-request 429 and continuation at its actual reset; final-head native recovery; desktop shell and relay/tunnel transport. The authenticated Mac account permits ordinary usage, corex has no authenticated quota account, POCO F4 is not exposed by the Device tool, and the alternate matching-client iPhone 17e failed its device transport and advised retry. Boat supplies the isolated tests, not an exhausted subscription. No sign-in or deliberate quota exhaustion was attempted. The settled-item fallback limit described in the preceding evidence comment also remains.

Expected red on the prior helper

[GPT-6.1-sol][Codex] on behalf of shivam.

Updated the branch to current main without conflicts. Head d5f94b4fb4865a28dff8b7f3dcd71b4cccbd859c, base 337c5858c33bc7f8f6dfd391a2c279fa0f7e4d17. Main changed mobile sharing-limit presentation and composer behavior plus VCS status handling; it did not add the missing account quota read. All six files in this PR's diff are byte-for-byte unchanged by the merge, so the prior independent code reviews and red regressions still cover that implementation.

On corex, Node 26.12.1 and Vite+ 1.4.0, the complete adapter, Codex quota helper, managed-error and provider-core usage test files pass 197/197 at this exact head. vp run check:changed 337c5858c33bc7f8f6dfd391a2c279fa0f7e4d17 also passes format, lint, both knip stages, GraphQL, 26 affected package typechecks and 193 changed-file tests. No repo-wide checks were run.

Fresh native comparison at this head and base: iPhone 17e, iOS 27.0, T3 Code Dev, matching native fingerprint e8f78d5e730353790c0a572575c387141109cfa4. Metro runs the final-head source on shiv. The two isolated source servers run on corex, with no copied credentials or injected UI/database failures. The external hand-built Codex JSONL fixture returns a first-turn usageLimitExceeded, then supplies a canonical quota reset while a concurrent notification leaves that reset unknown. The source Codex driver, orchestration, persistence and native client are real; the provider responses are synthetic.

Main: submit the first request, then see a failure with no reset and only manual Resume. The fixture receives no recovery quota read for that provider thread. This clip trims idle time before the trigger; its action sequence is unchanged.

before-main-short.mp4

Head: submit the same first request, see the recovered reset, tap Resume at reset, then watch T3 submit its automatic continuation and show the completed reply. The recording includes the full wait at normal speed.

after-head.mp4

Head: auto-resume armed
Head: automatic continuation completed

Unproven: genuine authenticated upstream first-request 429 and continuation at the account's actual reset. Today's read-only Codex probe on shiv returned accountType: null and -32600: codex account authentication required to read rate limits. It started no turn and changed no login. The synthetic captures do not prove that upstream path. The originally reported Android phone was not recaptured at this head; the attempted Android pool run failed its transport. Desktop shell and relay/tunnel transport were not refreshed. Earlier web and Android captures retain their original commit attribution. The inherited settled-item fallback limitation described above is unchanged.

Follow-up at head d5f94b4fb4865a28dff8b7f3dcd71b4cccbd859c, base 337c5858c33bc7f8f6dfd391a2c279fa0f7e4d17. The fresh native main/head captures and current-head local test output show synthetic first-request quota recovery through the real iOS client and corex server, including the persisted automatic continuation. I inspected both recordings' frames, the resulting reply and the test logs. All required GitHub checks pass or are conditionally skipped, no review threads are open, and the PR is mergeable. The body now points to these current-head results.

I retried the existing laptop Mac account without starting a turn, changing its login or copying credentials. The real Codex account and quota RPCs succeeded:

{"accountType":"chatgpt","quotaError":null,"quota":{"limitId":"codex","primary":{"usedPercent":78,"windowDurationMins":10080,"resetsAt":1794381580},"secondary":null,"planType":"pro"}}

The real account is not exhausted. Shiv's existing account probe is unauthenticated. The iOS alternative succeeded synthetically; the Android pool alternative failed device transport; the pinned Boat tests supply code-path proof without a live exhausted account. No job remains queued or running. I did not deliberately consume the remaining quota. A genuine upstream first-request 429 and continuation at its actual reset remain unverified, which is the remaining evidence-gate gap. The synthetic recordings do not claim otherwise.

Merged current main without conflicts. New head 29068de389e8bb824a864ebab94b4f49a0d4aa0e, base 09eb65344f6a2198039d17af653390475d653a1a.

The seven new main commits address side-chat permissions, worktree branch synchronization, Pi usage paths, and mobile outbox/remount, toolbar fade and markdown/SVG performance. They do not add Codex quota recovery. All six PR files are byte-for-byte identical to d5f94b4fb4865a28dff8b7f3dcd71b4cccbd859c. The existing independent reviews and 197 focused tests plus native main/head recovery recording retain their original head attribution. The native recording shows an unchanged-plan, plain-text flow in one mounted thread; the new main changes do not alter its quota read, failure card, arming or automatic continuation.

On corex, vp run check:changed 09eb65344f6a2198039d17af653390475d653a1a exited 0 at the new head. It passed all stages, including 26 package typechecks and 193 tests:

pass format 0.6s
  pass lint 1.1s
  pass knip 1/2 15.5s
  pass knip 2/2 18.6s
  pass graphql (apps/server) 0.8s
  pass typecheck (26 packages) 19.6s
  pass test (4 files) 24.1s
CI also runs: full test suites, build
unused patch check skipped: no dependency files changed

A fresh read-only check of the existing laptop account succeeded. It reports ChatGPT Pro, canonical primary usage 86%, duration 10080 minutes and reset 1794381580. No turn was started and no credentials were copied. The account is still not exhausted, so genuine upstream first-request 429 recovery and continuation at its actual reset remain unverified. The synthetic iOS comparison supplies the app evidence; it does not claim that live upstream path. CI for the pushed head will be checked when it finishes.

Fresh verification at head e06ccb1ac4bab44a45b8a7033ad1d618a95a7d2f, compared with main 7ef841dcf7c9f48b950b6d2b02e48b44e2be3265.

The real native and web clients, corex source server, Codex adapter, event persistence and recovery worker were exercised with a hand-built external Codex JSONL process. It reports usageLimitExceeded on the first request without a reset and with only an empty premium bucket. The recovery read emits a concurrent canonical notification without a reset, then returns a canonical exhausted window with a future reset. The fixture never contacts OpenAI or reads credentials. This proves the synthetic quota path, not authenticated upstream enforcement.

Native comparison: iPhone 17e simulator on shiv, iOS 27.0, development-client fingerprint 316afd351f76b38d7c29e47460801c335423fdc1. Mobile source is pinned to the comparison base and identical to this head. Main shows the first-request failure and a card saying the provider did not report a reset. Head recovers the reset, offers Resume at reset, records the armed state, then automatically submits the continuation and renders its completed reply. These recordings were inspected after export. The earlier attempt armed after expiry is excluded.

Main, first request has no reset
Main recovery card
Head, first request through automatic continuation
Head, auto-resume armed before reset
Head, completed continuation

Web compatibility on Chromium at 1280 x 800 uses the same head and a separately restarted isolated provider session. It records the first request, recovered reset, arming before reset, and completed automatic continuation. The web worker submitted the continuation about 25 seconds after the reset. This is not evidence of exact-deadline scheduling.

Web first request through continuation
Web armed recovery
Web completed continuation

Read-only inspection of events written by the app, all times UTC:

Client Armed Returned reset Server-created continuation Second run completed
Native 02:26:34.793 02:28:24.000 02:28:24.466 02:28:24.832
Web 02:33:52.400 02:35:38.000 02:36:03.226 02:36:07.047

Both continuations have createdBy: agent, creationSource: server, and text Continue where you left off. Native recovery was created from mobile, web recovery from web. No event or projection was injected.

vp run check:changed 7ef841dcf7c9f48b950b6d2b02e48b44e2be3265 exited 0 on this head. Actual output:

check:changed: 6 files since 7ef841dcf7c9
Test Files 4 passed (4)
Tests 193 passed (193)
check:changed summary
  pass format 0.7s
  pass lint 1.5s
  pass knip 1/2 22.8s
  pass knip 2/2 49.9s
  pass graphql (apps/server) 9.9s
  pass typecheck (26 packages) 112.7s
  pass test (4 files) 201.9s

The runner reported files changed during execution, but the tracked tree is clean. Current-head required GitHub checks pass or are conditionally skipped. All eight review threads are resolved; Macroscope retains approval. A separate read-only review of newer main c52df24d285a072ad9c3f7ab5b3656c0be1cb0f5 found four unrelated PR-management commits and no Codex, recovery, or chat overlap, supersession, or Julius takeover.

Unproven: a genuine authenticated upstream first-request Codex 429 and continuation at its actual upstream reset. The laptop Mac's existing ChatGPT Pro account now reports primary usage 100%, duration 10080 minutes, and reset 2026-11-11T07:19:40Z. Despite that report, minimal Luna and GPT-6.1 Sol turns completed without a quota failure. Codex CLI is 0.166.0. The sanitized Sol transcript at 01:35 UTC ends with turn/completed, status completed, error null, and a canonical 100% notification. No account was signed into a test environment and quota was not deliberately consumed. Existing shiv authentication was unavailable. Boat does not run providers or device apps. The native simulator proves the fixture flow but cannot supply the missing upstream rejection. The POCO Android recording, desktop shell and relay/tunnel transports were not refreshed.

The known inherited fallback remains unclaimed: when the bounded read fails and a notification arrives after settlement, it can update provider usage without persisting a reset into the settled failure.
