fix(server): refreshing providers re-reads Claude usage older than a minute - #28720
Refreshing providers (server.refreshProviders, as sent by the Usage page, the usage and provider settings and the command palette) returns Claude usage from the capabilities probe cache, which keeps a fresh probe for five minutes (CAPABILITIES_PROBE_TTL). An explicit refresh can therefore show Claude usage up to five minutes old while other providers come back freshly probed.
Change
ManagedServerProvider.refresh now runs invalidateUsageCache inside the refresh permit before the forced check, as refreshUsage already does, but only when the published usage reading is at least a minute old.
The floor exists because of #27772: Claude's /api/oauth/usage is rate-limited per account, so unbounded re-reads on every refresh would add 429s. With the floor, repeated explicit refreshes read the endpoint at most once a minute per instance; a reading kept fresh by live session updates is not re-read at all. Interval checks call applySnapshotBase directly and still reuse the cache, so background probing is unchanged. Only Claude supplies invalidateUsageCache, so other drivers are unaffected. The reset redemption keeps its own unconditional invalidation, since it must confirm the new limits even within the floor.
Scope and approval
A small, focused fix for an obvious bug, reported in #28721: an explicit refresh should not answer with usage up to five minutes old. It changes no contract, setting, or UI, and stays inside the existing refresh path. It does not attempt #27772's broader back-off; it only avoids making that problem worse.
Verification
New managedProvider.test.ts case "re-reads cached usage on an explicit refresh once the reading passes the floor" (TestClock): a refresh within a minute reuses the cached reading; after one minute it re-reads. On main the second assertion fails (expected 1 to equal 2, cache hit).
vp test run for packages/provider-core (188 passed) and for apps/server/src/provider, ProviderEventIngestor.test.ts and OrchestratorMcpService.test.ts (846 passed). The one failure, ClaudeSkills.test.ts > merges plugin enablement…, fails identically on unmodified main on my machine and is unrelated.
tsc --noEmit for apps/server and packages/provider-core; vp fmt, vp lint and knip:check pass.
Manual, macOS, vp run dev:server --home-dir <scratch> --no-seed with a real Claude Pro account, calling server.refreshProviders repeatedly. The startup check read usage at 16:06:36.481Z.
Refreshes started 16:07:19, 16:07:24 and 16:07:27 (within a minute): checkedAt stayed 16:06:36.481Z.
Refresh started 16:07:37 (past the floor): checkedAt advanced to 16:07:41.183Z.
Refresh started 16:07:43: checkedAt stayed 16:07:41.183Z.
On main, refreshes at 15:58:43 and 15:59:06 both kept the 15:58:40.443Z startup reading.
Not checked: Windows and Linux, and the web Usage page UI itself (the change is server-side; the page calls the same RPC).
Macroscope's review found this PR approvable — Explicit provider refresh now re-reads Claude usage instead of returning cached values. The change preserves background caching and existing refresh serialization, with no change to charges, permissions, or product defaults.

…minute
An explicit provider refresh (`server.refreshProviders`, as sent by the
Usage page, the usage and provider settings and the command palette)
re-ran the Claude check but read usage from the capabilities probe
cache, which keeps a fresh probe for five minutes. Refreshing could
therefore show usage up to five minutes old.
Invalidate the usage cache on an explicit refresh, inside the refresh
permit as `refreshUsage` does, but only when the published reading is
at least a minute old. Claude's usage endpoint is rate-limited per
account (pingdotgg#27772), so repeated refreshes still read it at most once a
minute. Interval checks keep reusing the cache, so background probing
is unchanged.

Walkthrough
Explicit provider refreshes now use a one-minute floor between usage-cache invalidations. The refresh checks the cached reading timestamp, conditionally invalidates the cache, and then forces a provider refresh. A test checks usage readings before and after the floor.

Summary
Usage cache invalidation on refresh packages/provider-core/src/server/managedProvider.ts, packages/provider-core/src/server/managedProvider.test.ts
refreshSnapshot acquires the refresh semaphore and invalidates the usage cache only when checkedAt is invalid or at least one minute old. The test checks that a refresh within the floor reuses the 20% reading, then reads the 100% value after one minute.
Issue 28721 reports cached usage remaining at the initial probe time; this change adds conditional cache invalidation during explicit refresh.
Issue 27772 concerns repeated native Claude usage probes and rate limiting; the one-minute invalidation floor is relevant to limiting those repeated probes.

Some Claude usage windows can remain stale after an explicit refresh until the cache expires. This is a bounded refresh gap that warrants a fix or explicit acceptance before merge.

Use the capabilities-cache age for explicit refresh invalidation.
Claude rate-limit events update one window and stamp usageLimits.checkedAt, but they do not invalidate the capabilities cache. If the cached probe is old and an explicit refresh occurs within the one-minute floor, the refresh can reuse that probe. The published event window remains current, but other windows remain stale until the cache expires.
Track the capabilities-probe read or invalidation age separately from live usage updates. Extend the existing managed-provider test with an old cached probe, a single-window rate-limit update, and an explicit refresh.
