fix(server): Claude shows the new account after a re-login instead of the cached old one - #29715
After switching Claude accounts (claude, then /login), T3 kept showing the old account and its exhausted limits until the cached Claude probe expired, up to 5 minutes after the last fresh probe and longer when no check ran. The Claude driver cached its capabilities probe (account and usage) on a key built once from the binary, Claude home and cwd. Nothing about the account was in it, so every check after the login (the background refresh, Usage → Limits refresh, a turn-end refresh) got the previous account's cached probe back. Only Settings → "Refresh provider status" re-probed, because it clears the cache.
Reproduced on main with a dev server on a fresh database, the Claude instance pointed at a stub CLI. The stub answers initialize/get_usage with the account in its home's .claude.json and logs every probe. After rewriting oauthAccount to account B, the way /login does, Usage → Limits → Refresh limits ran no probe and still showed account A at 0% left.
Change
The capabilities cache key now includes the logged-in account (oauthAccount.accountUuid and organizationUuid), read from the Claude global config the driver already uses ($CLAUDE_CONFIG_DIR/.claude.json or ~/.claude.json) on every lookup. A login therefore misses the cache on the next check of any kind, and an unchanged account keeps the cache. /login writes oauthAccount on every platform, including macOS where the token is in the Keychain. API-key or signed-out setups have no oauthAccount, so their key stays constant.
The one-probe fallback after a failed probe now reuses the last good probe only when it was taken under the same key, and the account read after the failure still matches. A login that lands during a failed probe therefore no longer brings back the previous account.
apps/server/src/provider/ProviderInstanceRegistry.test.ts, run against the real ClaudeDriver:
New test "re-probes Claude after a login switches the account" fails on main with account A's email and 100% usage where account B is expected, and passes here.
The existing fallback test gains a login-during-failed-probe step. That step fails without the post-failure account check and passes here.

Not checked: real Claude Code logins, and native Windows or macOS. The new registry tests return early on Windows, like the existing fallback test. With no user action, a new login is detected at the next scheduled provider check (default 5 minutes) rather than instantly; any refresh picks it up at once.

This test reproduces #29676: after a login rewrites the Claude config
to another account, the next provider check still serves the previous
account's cached capabilities and usage.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
The Claude capabilities cache was keyed on binary, home and cwd only, so a
check right after `claude auth login` switched accounts served the previous
account's auth and usage for up to five minutes.
Read the logged-in account (oauthAccount accountUuid and organizationUuid)
from the account config on every resolve and append it to the cache key. The
capacity-1 cache drops the old account's entry. The one-probe fallback now
reuses a probe only for the same key, which also covers a user switch within
one organization. Missing or unreadable config keeps a stable key.

Macroscope's review found this PR approvable — Claude capability metadata is now cached by logged-in account, preventing reuse of the previous account’s metadata after re-login. The change is limited to cache selection and fallback handling, without changing credentials, permissions, or product defaults.

Claude capability probes now use the logged-in account key as part of cache lookup. Failed probes reuse stored capabilities without usage only when the account key matches. Tests cover account changes during failed and successful probes.

Explanation
Issue #29676 requires T3 to re-check the Claude account and usage limits within seconds after re-login, without a restart. The cache key now includes accountUuid and organizationUuid, and the tests prove that the next refresh uses account B. However, the PR states that passive detection waits for the scheduled provider check, which defaults to five minutes. The implementation does not add a watcher or another seconds-level trigger.
Resolution
Add a config or credential change trigger that invalidates or refreshes the affected Claude provider instance within seconds after login. Alternatively, provide another equivalent automatic mechanism that meets the issue's seconds-level requirement. Keep the account-key check for cache correctness.

Discard a successful probe when the account changes during the probe.
The cache key is read before the asynchronous probe starts. The probe captures the email and usage from Claude's query, but the driver reads workspaceId from .claude.json after that query completes. If login changes during this interval, checkClaudeProviderStatus can publish account A's email and usage with account B's workspace ID. makeManagedServerProvider treats the changed workspace as a valid account switch and publishes the result. Re-read the complete account key after a successful probe and discard or retry the result when it differs from the key used to start the probe.
