fix(server): preserve sibling wakeups when interrupting a completion wake - #28204
Interrupting a delegated-completion wake could dispose undelivered sibling results from its original parent run and suppress their later wakeups. Those siblings still owe the parent a completion, whether they finish before or after the interrupt.
Preserve undelivered siblings outside the interrupted wake's task IDs and use the existing delivery planner for their successor wake. Explicit Stop still stops the whole cohort. This PR covers plain interruption.
Scope and approval
Submitted under CONTRIBUTING's very small, focused obvious-bug exception. Plain interruption incorrectly discards completion results from siblings that were never included in the interrupted wake. The fix limits disposal to the affected wake and retains the existing delivery and Stop rules. It adds no new workflow or product setting. The production change and its regression tests address this one interruption defect.
No prior maintainer approval or triaged-issue approval is claimed; this PR remains subject to contribution triage. The independent idle-streaming fix is #28221.
App recordings
Interrupting one assistant's completion turn no longer drops the other assistant's result. The parent receives that result automatically, even if the sibling finished while the interrupted turn was active.

Recorded November 4 on physical corex in T3's Chromium Browser, with the real codex-personal/gpt-6.1-sol parent and children, separate isolated T3 homes and synthetic git projects. Matching steps: launch A and B, release A, wait for its completion turn to block at the controlled gate, release B and confirm it completed, then call t3_thread_interrupt with A's exact wake run ID and stop omitted. The videos remove idle time only. The main recording also opens B's conversation to show its completed result. The composer Stop button intentionally stops the whole cohort and is a different operation.
B finishes after the interruption
On head 945a79c333009a5c02a4d369eb7014f8d6752145, interrupting A's completion turn leaves B running. When B finishes, its result reaches the parent automatically.

Recorded November 4 on physical corex in the real T3 web client, Chromium 156, using actual codex-personal/gpt-6.1-sol parent and child turns in isolated synthetic state. The controller keeps B blocked until persisted state confirms A's wake is interrupted and B is still running, then releases B. The app shows the interruption with one running child, followed by RESULT_B and the parent's response that it received both results. No user message or manual resume follows the interruption. Idle time is cut; the trigger and outcome remain visible. Children appear as Subagent because the provider omitted task titles; their prompts identify A and B.
Persisted events place A's wake interruption at 11:36:02.210Z, sequence 215, and B's completion at 11:36:36.233Z, sequence 251. B then has one generation-2 completion message and one completed successor run, ordinal 3. Its delivery is delivered, and the original cohort stays open with nextGeneration: 3.
A fresh independent GPT-6.1-Sol evidence review checked the recording and matched the canonical export to the isolated database. It resolved the B-after-interruption evidence finding with no objections. This proves one controlled corex web/Codex scenario; it does not prove exactly-once provider execution across crashes.

The web recordings do not exercise crash recovery. The dedicated current-head crash reruns below cover reservation, dispatch and started-provider cuts. Native desktop/mobile, other providers and remote/relay/tunnel remain unverified. Both the historical and current-head started-provider crashes disprove universal exactly-once execution and external effects; no such guarantee is claimed.

Rebased mechanically onto 647bd148b3fbd7c8b57a10fd41ae6d5fc7b35cc1. The implementation is unchanged. The test setup follows main's new NodeCrypto.layer requirement. Final tree 81a70e50045796ec81c43a126cd2597244073e30 matches the independently reviewed and tested integration tree exactly.
Fresh independent GPT-6.1-Sol review inspected main's changed delivery, recovery and Stop paths. No open findings. R2 remains resolved: completed but undelivered siblings outside the interrupted wake's task IDs survive interruption; closed cohorts stay closed.
Five focused suites passed 134/134 tests, exit 0, on corex with Node 26.12.1, Vite+ 1.4.0 and Vitest 5.0.5. These include both sibling-completion orderings, delivery through ProviderContinuationService, repeated recovery, bounded recovery, steering and Stop controls.

Current base/head runtime transcript and persisted identities
Reran the reservation, dispatch and started-provider crashes on physical corex at 945a79c333009a5c02a4d369eb7014f8d6752145, using real codex-personal/gpt-6-luna turns, isolated homes and the unmodified server. Each actual SIGKILL/restart was followed by another actual restart after delivery settled. A fresh independent GPT-6.1-Sol review matched the exported events and projections to the read-only databases and found no objections.

All counts and identities remained unchanged through the second restart. The started-provider case repeated the tool's marker write, so exactly-once provider execution or external effects is disproved on this head. One delivered transition does not mean one execution.
The current-head web recordings now cover B completing both before and after interruption. The earlier recordings at f12367634e2afdd76dcccfcbde24a4ff42731d3a remain historical. Reservation, dispatch and started-provider crash cutpoints have now been rerun at this head as documented above. Other crash cuts, native desktop/mobile, other providers and remote/relay/tunnel modes remain unverified.
Earlier live before/after and restart evidence at f12367634e2afdd76dcccfcbde24a4ff42731d3a: #28204 (comment). Reservation and dispatch crash evidence: #28204 (comment). Started-provider crash evidence: #28204 (comment). That last crash caused two actual provider executions and two B message/run identities, with one completed run and one final delivered transition. A second settled restart was stable. Exactly-once provider execution or external effects is disproved by that fixture and is not a guarantee of this fix. The current tests cover repeated recovery without claiming every real crash boundary.
Implementation: Claude Opus 5.5 through Claude Code on corex. Independent review, rebase and verification: GPT-6.1-Sol through Codex.
Macroscope's review found this PR approvable — Interrupting a completion wake now preserves pending sibling results rather than disposing the entire cohort. The change is narrowly scoped and leaves whole-cohort stopping and hold-queue behavior unchanged.

On corex, both plain-interrupt regression rows fail against the base orchestrator (stopped instead of open); explicit Stop passes. The final head passes 125 tests across DelegatedCompletionDelivery, ProviderContinuationService, SteeringCompletion.integration, ThreadStop, and BackgroundWorkStop.integration.
The synthetic service runtime shows both completion orders: interrupt disposes the wake's included result, the cohort stays open, the sibling is claimed as generation 2, and the real ProviderContinuationService persists its successor message. Two recovery and resume passes leave exactly one successor run in starting, with the next generation still 3.

R12 follow-up on head f12367634e2afdd76dcccfcbde24a4ff42731d3a, base 15fe2832727893cbdd2214383f9f971474849695: an actual SIGKILL on physical corex after B's successor provider turn started and executed a tool, before delivery acknowledgement. Real codex-personal / gpt-6-luna parent and children; isolated synthetic state; no source changes.
Before the crash, B's generation-2 successor was running, native provider turn 01a19369-90cf-7040-a11b-90062d98f746 was running, its provider-start effect had succeeded with attempt count 1, and its tool had appended one B_EXECUTED marker before blocking on a FIFO. B remained claimed, observedByRunId:null, with zero acknowledgement or delivered transitions.
Recovery cancelled that already-started run and provider turn, reserved generation 3, and ran a replacement provider turn 01a19369-ec51-7bc3-a5a3-2399b7daa4fb. The replacement appended the same marker again and completed. This is two actual B provider executions and two marker writes, two persisted B messages/trigger runs, one completed B run and one final delivered transition. A second actual SIGKILL/restart after settlement left both identities, provider-turn counts, marker count, delivered state and generation unchanged. No restart-continuation run was created.
This result rules out an exactly-once provider-execution or external-side-effect claim across this crash. The earlier two cuts in #28204 (comment) each executed one B provider turn because their first attempt had not started. Here it had started, and its synthetic side effect repeated. The interruption fix preserves the sibling result; it does not change the existing cancelled-delivery re-offer policy in finalizeDelegatedCompletionDelivery.
Limits: the fixture deliberately asks the model to write the marker on each B offer. It demonstrates that recovery can repeat an external effect; it does not estimate how often normal model behavior would repeat one. This tests one controlled provider-started/unacknowledged cut and one synthetic side effect with Luna. It does not establish behavior at every instruction/crash point, under every provider driver, or in native clients. The report makes no universal exactly-once claim. The cancelled first provider turn already executed a tool before process loss. The replacement repeated that tool. Exactly-once execution/external-side-effects is disproved by this fixture. Delivery settlement is not the same as execution count.
verified as-is at f12367634e2afdd76dcccfcbde24a4ff42731d3a on a disposable linux boat vm. baseline was github/main at 15fe2832727893cbdd2214383f9f971474849695 in a separate worktree. one dev server ran at a time, with isolated empty state and vp i in each checkout.
i drove the actual web app using repo playwright 1.60.0 and /usr/bin/google-chrome 151.0.7922.71. i added a real git project through the ui and ran real codex gpt-6.1-sol parent and child turns through the local proxy. file gates controlled when the real child commands finished. no mocks, seeded conversations, replay provider, or source patches were used.
main reproduces the bug: plain interruption of a's completion wake stops the original cohort and disposes b. b subsequently completes with RESULT_B, but no b completion message or successor run appears.
head, b finishes after interruption: a is disposed, the cohort stays open, and b reaches one generation-2 message and one completed successor turn. the app shows RESULT_B and RECEIVED_B.
head, b finishes before interruption: b is completed/pending while a's wake runs. interruption preserves b, then one generation-2 message and one completed successor turn deliver it.
neighboring behavior: clicking the actual composer stop button interrupts b, disposes the whole cohort, and produces no b completion wake. sending a normal message afterward completes with NEIGHBOR_OK; the old cohort stays stopped.
plain interruption used the running server's t3_thread_interrupt mcp tool with the exact wake runId, with stop omitted. explicit stop used the web button. canonical events and persisted projections were inspected read-only to confirm ordering and message/run counts.
blacksmith ran the five existing suites DelegatedCompletionDelivery, ProviderContinuationService, SteeringCompletion.integration, ThreadStop, and BackgroundWorkStop.integration: 125 tests passed. current head ci reports 31 successful checks, zero pending or failing. no code changes or pushes were needed.

native ios/android, electron shell behavior, macos/windows, other providers, remote/relay/tunnel connections, active steering, crash/restart boundaries, and exactly-once external provider side effects were not exercised in this pass. recordings of the interrupted-wake and ui-stop interactions were inspected and their uploaded playback and seeking were checked in t3's browser player; the screenshots above provide the inline pr evidence.

Limit interrupted wake disposal to its own completion task IDs. Keep undelivered siblings eligible, including tasks that finish during the wake, and reserve the successor through the existing delivery finalization. Explicit Stop still disposes the entire cohort.

The interrupt table now reads the interrupt's own stored events instead of a
later projection, so the wake's terminal claiming the sibling cannot race it.
It runs with the real ProviderContinuationService and follows the claimed
sibling through the continuation dispatch to one started successor run, then
checks that two recovery passes do not wake the parent again.

Ready check repeated on head 945a79c333009a5c02a4d369eb7014f8d6752145 against current main 8b44f42b14c8e273156474a924ccf29680dd7857. GitHub still reports the earlier base snapshot 647bd148b3fbd7c8b57a10fd41ae6d5fc7b35cc1, so I fetched and inspected all five new main commits locally.
The 28 changed files concern release versions, Android content layout, status-row alignment, file metadata expiry and review diff loading. I inspected the server VCS changes, shared query changes and thread presentation changes. None changes the orchestrator, delegated delivery, interruption, continuation, recovery, provider adapters or their contracts. git merge-tree --write-tree exits 0, merged tree c4f667cba33217af806053cbe8682504b91f4868. The wake patch applies unchanged. The current-head 134 focused tests, passing Boat job and matching app recordings in the PR body still cover the fix. No extra test run or branch update was needed for this base movement.
All eight configured required checks are satisfied, CodeRabbit is successful with no actionable findings, and there are no unresolved review threads. The PR is open, non-draft and mergeable, with no auto-merge or merge-queue entry. No Julius claim or takeover was found. Native clients, other providers and live crash boundaries were not rerun; the existing crash evidence explicitly does not guarantee exactly-once provider execution.
The missing B-after-interruption recording is now embedded in the PR body at head 945a79c333009a5c02a4d369eb7014f8d6752145. At 0:21, A's wake is interrupted while B remains running. At 0:48, the parent has automatically received B's result. No user message or manual resume follows the interruption.
Corex web recording. The body includes persisted events showing interruption before B completes, one generation-2 B message and one completed successor run. A fresh independent GPT-6.1-Sol evidence review matched the export to the isolated database and resolved the finding without objections. I verified uploaded playback and seeking.
The live ready check still passes on this head against current main 8b44f42b14c8e273156474a924ccf29680dd7857. GitHub reports the earlier base snapshot 647bd148b3fbd7c8b57a10fd41ae6d5fc7b35cc1; its five subsequent commits do not change the affected orchestration paths, and the local merge is clean. Required checks pass, CodeRabbit passes, no review threads remain unresolved, and the PR is open, non-draft and mergeable. No auto-merge or queue entry is enabled.
Evidence: both current-head completion orders now have real corex web/Codex recordings; the body retains 134 focused passing tests and exact-head Boat check:changed output with 55 passing tests. Native clients, other providers, remote/relay/tunnel and live crash boundaries were not rerun at this head. Historical started-provider crash evidence demonstrated repeated execution; this patch does not guarantee exactly-once provider execution or external effects.
Reran all three requested crash scenarios on physical corex at 945a79c333009a5c02a4d369eb7014f8d6752145, tree 81a70e50045796ec81c43a126cd2597244073e30. Each used a fresh isolated T3 home, real codex-personal/gpt-6-luna parent and children, synthetic file gates, and the unmodified server. Each crash was an actual SIGKILL followed by a restart of the same home. Every settled case then survived a second actual SIGKILL/restart with unchanged counts and identities.

The first two boundaries use external Node inspector breakpoints at ProviderContinuationService.ts:119 and :180. The third waits for the real provider tool to append its marker and block. Canonical events, read-only projections and outbox attempts establish each boundary independently. No child-reading tool or manual parent message drove recovery.

Reservation and dispatch boundaries use external Node inspector breakpoints on unmodified ProviderContinuationService.ts lines119 and180, conditioned on generation2. Started boundary awaits an actual B_EXECUTED append and persisted running provider turn while delivery remains claimed, before acknowledgement.
Each crash uses actual SIGKILL on the captured isolated-server process group; the same home is restarted. Every settled case gets another actual SIGKILL/restart.
One earlier reservation fixture attempt ended its children before the blocked shell exited, so it was discarded and replaced before any reported scenario. Its state/processes are excluded.

Problem
Interrupting a delegated-completion wake could dispose undelivered sibling results from its original parent run and suppress their later wakeups. Those siblings still owe the parent a completion, whether they finish before or after the interrupt.

Change
Preserve undelivered siblings outside the interrupted wake's task IDs and use the existing delivery planner for their successor wake. Explicit Stop still stops the whole cohort. This PR covers plain interruption.
Scope and approval
Submitted under CONTRIBUTING's very small, focused obvious-bug exception. Plain interruption incorrectly discards completion results from siblings that were never included in the interrupted wake. The fix limits disposal to the affected wake and retains the existing delivery and Stop rules. It adds no new workflow or product setting. The production change and its regression tests address this one interruption defect.
No prior maintainer approval or triaged-issue approval is claimed; this PR remains subject to contribution triage. The independent idle-streaming fix is #28221.
App recordings
Interrupting one assistant's completion turn no longer drops the other assistant's result. The parent receives that result automatically, even if the sibling finished while the interrupted turn was active.

The controller keeps B blocked until persisted state confirms A's wake is interrupted and B is still running, then releases B. The app shows the interruption with one running child, followed byRESULT_Band the parent's response that it received both results. No user message or manual resume follows the interruption. Idle time is cut; the trigger and outcome remain visible. 945a79c333009a5c02a4d369eb7014f8d6752145, using realcodex-personal/gpt-6-lunaturns, isolated homes and the unmodified server. Each actual SIGKILL/restart was followed by another actual restart after delivery settled. A fresh independent GPT-6.1-Sol review matched the exported events and projections to the read-only databases and found no objections.All counts and identities remained unchanged through the second restart.
