fix(claude): tools running at Stop show as stopped, not failed - #29801
If you press Stop while a Claude tool is still running, such as a long foreground Bash command, the work log shows that tool as failed ("Failed to …"). The tool did not fail. You stopped it, so it should show as stopped, like the turn does.
When Claude Code is interrupted, it sends an error tool_result for each running tool (is_error: true, tool_result_meta.non_execution_kind: "user-rejected"), followed by a result with terminal_reason: "aborted_tools". ClaudeAdapterV2.settleToolCall maps every error result to failed and takes the tool out of the open set. Because of that, the turn finalizer, which marks still-open tools interrupted, never sees it. Clients show interrupted and cancelled as stopped, but they show failed as a red failure.
Change
When the turn has already been interrupted (its provider turn id is in interruptedTurns, which interruptTurn sets before it calls the SDK's interrupt), settleToolCall now records a tool error result as interrupted. The status is the same one endToolCalls already gives tools that are still open when an interrupted turn ends. It also matches the OpenCode fix in #29762.
Workflow members get the same treatment. When Stop cancels a workflow, the adapter settles member tools from each member's transcript, without a turn context. If the member was cancelled and the error line has Claude's denial marker (toolDenialKind, set to "user-rejected" in Claude's Stop reply), that tool is now recorded as interrupted. A genuine member tool failure that is first read after Stop has no denial marker, so it stays failed.
I used the interrupt flag rather than non_execution_kind: "user-rejected". That kind may also describe a real permission rejection, and the existing non-execution table test treats every kind other than cancelled as a failure.
Scope and approval
This is a very small, focused fix for an obvious bug, so there is no prior issue. The adapter already models "interrupted" for tools that are open when Stop ends a turn. This change only stops the CLI's own Stop reply from turning those tools into failures. It is about 16 non-test lines in ClaudeAdapterV2.ts.
Overlap search: I searched the local PR index and GitHub for Claude Stop/interrupt and tool status changes. The closest PRs are #21158 and #15868 (a steer cancelling or interrupting Claude tools) and #28356 and #27487 (subagent rows stuck on Running). None of them touch settleToolCall or how a tool's error is classified after Stop.
Verification
Real CLI behavior: with Claude Agent SDK 0.3.276 and Claude Code 2.1.314 on Haiku, I ran ping -c 25 127.0.0.1 in Bash and called interrupt() after 4 s. The output was a tool_result with is_error: true and tool_result_meta: [{ non_execution_kind: "user-rejected" }], then [Request interrupted by user for tool use], then a result with subtype: "error_during_execution" and terminal_reason: "aborted_tools".
I extended the existing test lets the native SDK acknowledge Stop before closing its own query in ClaudeAdapterV2.test.ts. A Bash tool is running, and the Stop acknowledgement sends the real-shaped error tool_result followed by the aborted_tools result. On main it fails with AssertionError: expected 'failed' to equal 'interrupted'.
Workflow members: I added the test shows a workflow member's tool as stopped when Stop cancels the workflow. In it, a member's first tool fails for real, its second gets the Stop reply, and then the workflow is stopped. Without the member change it fails with - "command interrupted: Rejected" + "command failed: Rejected". The transcript shape comes from real local Claude Code transcripts, subagent sidechains included: the Stop reply is an error tool_result with toolUseResult: "User rejected tool use" and toolDenialKind: "user-rejected", and genuine tool failures have no toolDenialKind.
With the fix, vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts passes 289/289, and ClaudeReplayFixtures.integration.test.ts passes 38/38.
CI=true vp run check:changed passes: format, lint, knip, typecheck (3 packages), and tests (290).
Not checked: a real client capture. The visible change is the existing "Stopped …" label in place of "Failed to …" for that row.
Reviewed with sol-loop: 1 round with GPT-6.1 Sol on high for the main change and 2 rounds for the workflow-member follow-up. Sol verdict: approved. Round 1 of the follow-up found that a genuine failure read late was being relabeled; the toolDenialKind check fixed it.

Macroscope's review found this PR approvable — Stopped Claude tools now report an interrupted status rather than a failure, including cancellation-marked subagent transcript results. The change is limited to outcome reporting and leaves tool execution unchanged.

Walkthrough
ClaudeAdapterV2 now passes cancelled workflow-member state to tool settlement. Rejected tool results for stopped targets are marked interrupted; other error results remain failed. Tests cover graceful Stop and workflow-stop projection.
Workflow-member projection sets a stopped flag when the member is cancelled. Tool settlement uses that flag for rejected tool results and marks them interrupted; other error results remain failed. Tests verify the status for graceful Stop and workflow stop, including preservation of a previously failed command.

The title "fix(claude): tools running at Stop show as stopped, not failed" matches the main change. Explanation
The description follows the template well. It has Problem, Change, Scope and approval, and Verification sections. The Problem section gives the expected behavior and the mechanism. The Change section explains the interrupt-flag approach and the workflow-member handling. The Scope section argues why the fix qualifies as a small obvious bug fix without a prior issue, and it lists an overlap search. The Verification section gives specific tests, observed results, and items not checked. The description does not link a triaged issue or maintainer approval, which the template asks for unless the fix qualifies as a small obvious bug. The author argues that it does qualify. The Verification section also reports a count of 289/289 tests, while the PR objectives report 288/288. This is a minor inconsistency that the author should reconcile. The template's optional trailing note about the agent and model is present.

Not verified
The original red-failure regression did not reproduce live on current github/main (61c3794c32). Baseline attempts with Claude CLI 2.1.316 and the report's 2.1.314 both persisted the stopped tool as interrupted. These runs verify expected live behavior, but do not establish a live failing-before/passing-after comparison.
The real workflow stop transcript lacked toolDenialKind, so it did not prove the newly added workflow denial-classification branch ran. That exact case, including preservation of a genuine earlier failure, passed the regression test. A genuine permission rejection, native desktop/iOS/Android clients, and relay/tunnel connections were not exercised. The proxy rejected Haiku's normalized alias, so Claude integration checks used Sonnet 4.6.
