fix(cursor): avoid reporting unfinished tool calls as successful - #21448
When a Cursor run finishes normally, finalizeTurn closes every tool that never got tool-call-completed as completed. A read denied by .cursorignore is stored as a successful Read with no output and a completion time equal to the run end, while the assistant says the read failed. The issue lists this happening for permission-denied calls, chunked reads and parallel calls.
Change
On a normally completed turn, leftover tools now close as interrupted instead of completed. ClaudeAdapterV2 uses the same status for calls still open when a workflow member settles, since they will never get their result. I did not use failed, which the Claude adapter gives top-level leftovers: as the triage notes, chunked reads that succeeded but never got a completion event would then show as failures. Interrupted, failed and cancelled turns still pass their own status to leftover tools, unchanged.
I tried idle first, the status Cursor Task subagents get for an unknown outcome. The web timeline hides tool rows with idle status (workEntryIsVisibleInGroup), so the denied read vanished from the thread. interrupted keeps the row visible on web and mobile without recording a success.
This does not fill in the real result. Reading leftover calls back from the agent store, as the triage suggests, could turn a denied read into failed with its error. That is a larger change for a later PR.
Scope and approval
Fixes#16150. The triage comment (Grok on behalf of Julius) confirms that finalizeTurn closes leftover tools as completed on a normally completed turn, and suggests closing calls with no known result "with an unknown outcome rather than as a success". It leaves the fix direction to a maintainer; no maintainer agreement is recorded yet. The change is 4 lines in packages/provider-cursor/src/server/adapter.ts plus a test.
Verification
From packages/provider-cursor, on Linux x64, Node 24:
vp test run --config ../../vite.config.ts --dir . src/server/adapter.test.ts
The new test replays the issue's event order through a fake SDK runner: secret.txt starts and never completes, open.txt starts and completes with content, then the run ends finished, cancelled or error. It checks the final turn item and execution node for both reads.
This branch: 19 passed.
Same test against main's adapter.ts: only the finished case fails, with secret.txt reported completed instead of interrupted. The cancelled and error cases pass on main too, so those branches are unchanged.
vp fmt --check and vp lint on both files, and tsc-rs --noEmit for the package, exit 0.
I also ran the server and web app from this branch with vp run dev, and pointed the mobile dev client at the same server on an Android 16 emulator and an iOS 26.5 simulator. The Cursor SDK was swapped locally (not in this PR) for a runner that replays the issue's event log, because I had no Cursor API key for a test server. I sent the issue's prompt from each client, once against main's adapter and once against this branch:
On main, the secret.txt row is stored completed with no output.
On this branch it is stored interrupted, and both Read rows stay visible on web, Android and iOS.
The clients draw a finished completed and interrupted Read row the same way, so there is no visual before/after to show. Mobile now classes the row as neutral instead of success. I did not test against a live Cursor account.
When a Cursor run finished normally, tools that never received
tool-call-completed were closed as completed, with no output and a
completion time equal to the run end. A read denied by .cursorignore
looked like a successful read.
Close them as interrupted instead, as the Claude adapter does for calls
that will never get their result. The row stays visible in the timeline
and is no longer recorded as a success. Interrupted, failed and
cancelled turns keep passing their own status to leftover tools.

Macroscope's review found this PR approvable — The production diff is a small, isolated Cursor finalization fix that changes only the status recorded for tools lacking a completion event; existing completed, cancelled, and failed paths remain unchanged. The accompanying regression test covers the affected and preserved behaviors without introducing schema, configuration, deployment, or new user capability changes.

Walkthrough
Cursor now marks unfinished tool calls as interrupted when a turn completes. For other turn outcomes, unfinished calls retain the turn’s status. Tests check these statuses and confirm that a completed sibling call retains its output.
When a turn completes, unfinished tool calls receive interrupted status. For error or cancelled turns, they retain the turn’s status. Tests check these outcomes and verify the completed sibling call’s status and output.
Unfinished reads are now stored as interrupted, but their display still resembles a completed read. That existing presentation limitation warrants follow-up.
Security architecture risk: Minimal · up to 7bc67
Unfinished operations no longer appear successful merely because their turn finished. The change adds no execution or data-access capability and uses an existing supported status.

No architecture-level concerns identified.
Security review details
Security Blast Radius
observed — The status change applies to every tool still tracked at Cursor turn finalization, not only reads. It changes existing outcome projections; the finalizer does not invoke those tools again. Existing partial shell output may still accompany an interrupted outcome.
Trust Boundaries and Controls
inferred — Provider-supplied arguments and results continue through the existing callback and projection path. The changed branch selects a terminal status rather than adding file access or a permission override. Interrupted records an unknown tool outcome; it does not establish that underlying side effects were cancelled or rolled back.
Resilience and Maintainability Implications
observed — The existing finalized guard prevents repeated finalization and rejects subsequent interaction callbacks. Interruption validates the active provider-turn identity, requests cancellation, and uses the same finalizer after its completion timeout. These recovery controls are unchanged by the status adjustment.

thanks to only21mil for the original status repair. i kept the contributor's commit and fix, merged current main, and moved the regression into the existing cursor lifecycle cases. this removes the duplicate session setup and keeps assertions for the unfinished read, the completed sibling's output, and both execution-node statuses. the diff against main is now 65 additions and 2 deletions across two files.
this qualifies as the focused obvious-bug exemption: a tool with no completion event must not be recorded as successful. it adds no capability, schema, or client behavior.

blacksmith red/green: main fails the three finished-run variants because the unfinished read is completed; cancelled and error variants pass. all 21 adapter cases pass with the fix.
blacksmith vp run check:changed github/main passes format, lint, both knip checks, typechecks for 10 packages, and 22 tests across two files.
a direct adapter runtime replay independently produces the transcript below. it uses an injected sdk runner, not the live cursor service.

unverified: live cursor service, fresh web/desktop/mobile rendering, and persistence through the full orchestration path. the browser app loaded, but its replay provider was disabled for the project, so no browser capture is presented as cursor integration proof. the original replay evidence remains historical; i did not rerun its ios or android checks. no client source changes are included.

installed github/main (e5909e53c3) in a separate worktree with vp i, ran its dev server, then stopped it before starting the exact pr head. drove the real web client with repo playwright and /usr/bin/google-chrome. created a project pointing at a real git repo and submitted real codex gpt-6.1-sol turns through the composer. no mocks or replay provider.
both branches completed a tool read of open.txt and secret.txt and displayed the returned contents.
on the pr, a follow-up read of nonexistent missing.txt showed exit 1, while the sibling read of open.txt returned ok. read-only inspection of persisted turn items confirmed failed and completed, respectively.
reloading the pr client preserved both answers and tool history. the verification repo remained unchanged.
blacksmith vp run check:changed 80a2351fcb909034e65a512dfabeb3cdfd367762 passed format, lint, both knip checks, typechecks for 10 packages, and 22 tests. the existing cursor adapter suite passed all 21 cases. those tests use an injected sdk runner and do not establish live cursor integration.
the actual cursor fix remains unverified. cursor-agent status reports Not logged in, no CURSOR_API_KEY is configured, and adding cursor through provider settings reports Not authenticated with no available models. main also had no cursor models or authenticated account. i could not reproduce the original .cursorignore unfinished-read bug on main or exercise the changed cursor finalization path on the pr.
codex uses a different adapter. its live turns above cover neighboring client/server behavior only and do not prove this cursor fix. live cursor completed/cancelled/error runs, native ios/android, desktop shells, and remote/tunnel connections were not exercised. no before/after cursor-success comparison is claimed.
