fix(server): orchestrator records command count and duration again - #23966
t3_orchestration_commands_total and t3_orchestration_command_duration disappeared when the v2 orchestrator replaced the old engine, so command throughput, failure rate, and latency could no longer be charted or alerted on. Project commands commit through ProjectService, outside the orchestrator, and were never counted.
Change
Both metrics are back, labeled by commandType (and outcome on the counter: success, failure, interrupt).
One withCommandMetrics combinator in Metrics.ts, attached as the second argument of the existing Effect.fn definitions:
dispatchWithReceiptEffect in the orchestrator. It runs inside the thread dispatch lock, so lock wait is excluded. Limit recovery calls it directly and is covered by the same point.
ProjectService.commit. Its duration includes project/workspace lock wait, which is uncontended in practice, and the metric description says only thread lock wait is excluded.
The two metrics are listed again in docs/operations/observability.md.
Verification
One test per path (Orchestrator.control-reads.test.ts, ProjectService.test.ts) reads the exact labeled counter and timer before and after a single dispatch and expects +1. Both fail on main.
Runtime: an isolated server (scratch --base-dir) exporting OTLP to a local capture endpoint, after project add, project rename, and one thread start:
Macroscope's review found this PR approvable — This restores orchestration command counters and durations around existing server handlers, with focused tests for both orchestrator and project commands. The runtime impact is limited to observability bookkeeping and does not alter command semantics, product defaults, or deployment behavior.

Walkthrough
The change adds orchestration command-count and duration metrics. It records these metrics for commands dispatched through Orchestrator and for project commands handled by ProjectService. Tests check metric counts for thread.create and project.create.

Explanation
The title "fix(server): orchestrator records command count and duration again" matches the main change: restoring the t3_orchestration_commands_total counter and t3_orchestration_command_duration timer via a withCommandMetrics combinator applied to dispatchWithReceiptEffect and ProjectService.commit. It is a conventional-commit title, specific, and concise. Minor note: the change also covers ProjectService.commit, which the title does not mention, but that is an expected omission.
Full details: Description check
Explanation
The description follows the template. It has a Problem section that explains the missing metrics and why project commands were never counted. It has a Change section that explains the withCommandMetrics combinator, the two instrumented paths, and the docs update. It has a Verification section with two focused tests that fail on main and an isolated-server OTLP run with captured output. The Scope and approval section is missing. The description does not link a triaged bug issue or maintainer discussion, and it does not explain why the fix qualifies without one. The template requires that section, so the author should add it.
Resolution
Add the Scope and approval section. Link the triaged bug issue or the discussion with explicit maintainer approval of direction and scope, including the approval comment. If no prior issue exists, explain why this focused fix of an obvious bug qualifies without one. Also consider noting the Claude Code model and harness used, as the template requests for agent-assisted work.

Assert before/after deltas of the exact labeled counter and timer so the
process-global metric registry cannot satisfy them from other tests.

Addressed in the takeover: the metrics are attached with a withCommandMetrics combinator passed as the second argument of the existing Effect.fn definitions (dispatchWithReceiptEffect and ProjectService.commit), so there is no wrapper. autoArmLimitRecovery calls dispatchWithReceiptEffect directly, so the same instrumentation point covers it. Resolving both threads.
