Problem

Turning on Resume after usage limits made the usage-limit recovery sweep arm and resume delegated child threads that had stopped on a usage limit while the setting was off, even when their parent thread was settled. When a resumed child finished, its delegated-completion wake could pull the settled parent back to active and start a new run. The candidate query only checked the child's own archivedAt and settledOverride and never looked at its lineage.

Reproduced on main (2b15ba0) with a projection-level test. It builds two parent/subagent pairs whose children failed on a usage limit and have nothing armed; one parent is settled. With autoResume on (and separately with snooze on), both children get a limit-arm: command, including the child of the settled parent.

Change

getLimitRecoveryCandidates gets one extra condition in the branch that freshly arms a recovery (both the auto-resume arm and the snooze arm). A thread whose lineage.relationshipToParent is subagent and whose direct parent has settledOverride = 'settled' is not offered for a new arm. The parent lookup goes through the threads table's primary key and only runs for rows that already passed the usage-limit pre-filter.

The resume branch is unchanged: a recovery that was already armed for the child's latest failed run still resumes at its reset. Unsettling the parent emits a thread.* event, which already makes the sweep due, so the child becomes eligible again on the next tick.

Scope and approval

Fixes #27944

Verification

  • New test ProjectionStoreV2 > does not freshly arm limit recovery for a subagent of a settled parent in apps/server/src/orchestration-v2/ProjectionStore.test.ts. It checks that a child of a settled parent is not armed under autoResume or snooze, that a child of an unsettled parent still is, and that a child armed before its parent settled still produces its limit-resume: dispatch.
  • Red with main's ProjectionStore.ts (2b15ba0):
× does not freshly arm limit recovery for a subagent of a settled parent
AssertionError: only the child of the unsettled parent is armed ({"now":"1970-01-01T00:00:00.000Z","autoResume":true,"snooze":false}): expected [ …(2) ] to deeply equal [ Array(1) ]
      actual: [ 'thread:settled-parent-limit:active:child', 'thread:settled-parent-limit:settled:child' ],
      expected: [ 'thread:settled-parent-limit:active:child' ]
    Tests 1 failed | 44 skipped (45)

  • Green with the fix (003df33): Tests 1 passed | 44 skipped (45).
  • ProjectionStore.test.ts and UsageLimitRecoveryWorker.test.ts: 46/46 pass. runtimeLayer.test.ts -t "limit": 20 pass. vp run check:changed passes (format, lint, knip, typecheck, tests).
  • Server-only change with no visible surface, so there are no captures. Not checked: a live provider hitting a real usage limit, which a VM cannot trigger. The proof is the projection test above.

… for usage-limit resume

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…ed parents

The limit-recovery sweep only checked a thread's own settlement, so turning
on "Resume after usage limits" armed and resumed abandoned subagent threads of
settled parents in a burst, and their completion wakes reactivated those
parents. The fresh-arm branch of the candidate query now skips a subagent whose
direct parent is settled. A recovery already armed for the child's latest run
still resumes at its reset, and unsettling the parent makes the child eligible
again on the next sweep.

Approvability

Verdict: Approved at 5abe8a0

Macroscope's review found this PR approvable — Fresh usage-limit recovery will no longer arm subagents of explicitly settled parents, preventing abandoned work from waking those parents. Already-armed recoveries and product preference defaults remain unchanged.

No code changes detected at 8f9e45d. Prior analysis still applies.

Walkthrough

The limit-recovery candidate query now excludes subagent threads whose parent has settledOverride set to "settled". Tests check fresh recovery selection under two option sets and verify that an already-armed recovery can still produce a resume command after its reset time.

Explanation

The description includes all required sections: Problem, Change, Scope and approval, and Verification. It explains the bug, implementation, linked issue, focused tests, observed results, and untested live-provider scenario.

Full details: Out of Scope Changes check

Explanation

The changes stay within issue #27944. The production change updates usage-limit candidate selection, and the test covers settled and unsettled parents, auto-resume, snooze, and existing recovery behavior.

Full details: Linked Issues check

Explanation

Issue #27944 requires that a child whose direct parent is settled is not resumed by the usage-limit sweep. ProjectionStore.ts excludes the child only when the sweep would create a fresh recovery arm. UsageLimitRecoveryWorker.ts still dispatches limit-resume: when an existing recovery matches the latest failed run, without checking the parent's settlement. The added test confirms this behavior for an already-armed child.

Resolution

Also exclude subagents of settled parents from the already-armed resume path, or cancel their recovery before dispatch. Keep a regression test that settles the parent after arming and verifies that no resume dispatch occurs.

* fix(web): play completion sounds without delay (pingdotgg#29146)

Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.

Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.

This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.

Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)

* fix(server): keep PR watches active after host failures (pingdotgg#29450)

## Problem

PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.

## Change

Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.

The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.

## Scope and approval

This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.

## Verification

`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.

## Summary
Merge upstream 91f66f8 → 61c3794 (149 commits) with a merge
commit. All eight conflicts resolved against fork policy; lockfile
re-derived. Both-parent resolution check and inventory check pass.

## Verification — draft
All nine fast checks pass: duplicate adds, tripwires, both-parent
resolutions, unsupported-method derivation, lockfile, format, lint,
typecheck, production web build.
Initial full verification failed tests after package and single-file
retries. OpenCodeServerLedger.test.ts has five process-group cleanup
failures in isolation; t3 server failures include the registered /tmp
sticky-bit and global Git insteadOf sandbox gaps. No assertions
suppressed. Final full sequential verification is running; results will
follow. Pre-commit knip still has the known 27 unused exports; commit
used --no-verify.

## Usable as-is
- Browser-offline primary-environment connection fix and connection
diagnosis during sign-in (client-runtime).
- Web terminal controls no longer obscure the first line; timezone in
usage retry notices; model picker height, sidebar accessibility,
completion sounds and expanded-work scroll preservation.
- Checkout grouping overrides in ProjectSettingsPanel remain client
settings; project write/delete gates retained.
- Chat images reload after workspace mutations; PDF reload reuses its
worker. Formatting, review-comment blank lines, context-chip emoji and
attachment size improvements.

## Unsupported in Moatless / needs implementation
- orchestration.getNativeChildWork (packages/contracts/src/rpc.ts):
added UnsupportedMethodError from the derivation; deferred
provider-native child work cannot be fetched.
- Thread tags / recording-time hiding (pingdotgg#29053): thread shell tags,
thread.tags-updated and hiddenThreadTags/hideTaggedThreads settings need
backend data. Client filters alone do not implement tagging.
- Model/effort-change annotations (pingdotgg#28995): timeline readers accept
optional modelSwitch; Moatless must emit it.
- Kiro ACP (pingdotgg#14693), Antigravity 1.4.5 (pingdotgg#28629) and secret-provider
source-control tokens (pingdotgg#27756) need Moatless runtime/integration work;
upstream Node implementations are not deployed by this fork.

## Backend behavior to consider reproducing in Moatless
- Imported Claude first-turn resume and nested-subagent completion
persistence (pingdotgg#23219, pingdotgg#23561); preserve delegated replies after parent
settlement (pingdotgg#23456).
- Prevent settled-parent child usage-limit auto-resume (pingdotgg#27951), and
Stop a run whose provider already finished (pingdotgg#28520).
- Preserve user locale for Git hooks (pingdotgg#27778); keep slow forge CLI calls
off checkpoint/start paths (pingdotgg#28431); remove branches left by failed
worktree creation (pingdotgg#28373).
- Upstream provider session/compaction/recovery and shell-read
performance fixes merit parity review, not client transplantation.
Concrete anchors and closure checks are in docs/fork/gaps.md.

## Merge accounting
Initial landed and upstream-range counts are both 790 files; the
intentional swap is inventory.json versus the deliberately deleted
pair.test.ts. Initial fork delta: 772 files. Verification/documentation
follow-ups are recorded in docs/fork/upstream-merge-log.md.
Owned-surface sweep found only Kiro transcript and migration 077 runtime
fixtures/repair; no new workflows.

Do not merge while draft: isolated test failures remain unresolved.

[Bug]: Usage-limit auto-resume restarts delegated children of settled parents, which can reactivate those parents
