fix(server): OpenCode no longer drops a $skill read before its directory scan ends - #29535
With OpenCode 2, a $skill mentioned in a prompt was sometimes dropped before it reached OpenCode. The prompt went out with no skills, so the model got the literal $name and never saw the skill's content. T3 resolves mentions against a single GET /api/skill read for the thread's directory. OpenCode fills that list in the background: a directory it hasn't finished scanning lists no skills or only some, for roughly 100 ms to over a second. Any mention missing from that one read was discarded.
Reproduced on main with the real dev server against OpenCode 2.2.5:
Reporter's steps (new worktree thread, $greet, immediate send): the skill is attached. T3's GET /api/model for the new directory runs first and returns only once the scan has ended.
OpenCode restarted with the thread still open: the next turn, $greet again please, made one GET /api/skill against the cold directory. It then sent POST .../prompt {"text":"$greet again please"} with no skills. OpenCode's idle location eviction leaves a directory just as cold.
Change
A prompt whose mentions are all found on the first skill.list is sent exactly as before. If a mentioned name is missing, the adapter waits for OpenCode to finish scanning the directory, then reads the skills again:
The wait is a call to OpenCode's model list for that directory (the existing readModels, 5 s timeout, logged on failure). OpenCode 2.2.5 answers it only once the scan has ended. This is observed behaviour, not a documented contract.
The second read is merged into the first, so a failed reread never loses a skill already found.
It keeps no state and doesn't depend on the event stream, which is down right after a restart.
A $word that isn't a skill, such as $HOME, now costs one model read and one skill read, about 50 ms in a directory OpenCode has already scanned.
apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts, 88/88 pass after merging main:
sends a $skill that appears once OpenCode finishes scanning the directory: fails on main with expected { …(3) } to have deep property 'skills', passes here. The first read is empty, the barrier runs, and the reread returns greet.
keeps a $skill found on the first read when the reread after the scan comes back empty: guards the merge of the two reads. It fails without that merge.
vp run check:changed passes: format, lint, knip, typecheck of 10 packages, tests.
This is a server-only fix with nothing visible to capture. The runtime check used the real dev server and a real OpenCode 2.2.5 opencode serve, with a logging proxy between them recording T3's requests.
Before: main 0847a44, OpenCode restarted under the open thread
18:46:45.130 PATCH /api/session/ses_…
18:46:45.150 GET /api/skill?location[directory]=<worktree>
18:46:45.276 POST /api/session/ses_…/prompt {"text":"$greet again please","delivery":null,"resume":null} ← no skills
19:16:19.318 GET /api/skill?location[directory]=<worktree>
19:16:19.459 GET /api/model?location[directory]=<worktree> ← returns once the restarted server has scanned the directory

A $word that isn't a skill, in a directory OpenCode has already scanned (9e02a6f, which takes the same path):
19:09:38.373 GET /api/skill → 19:09:38.392 GET /api/model → 19:09:38.406 GET /api/skill → 19:09:38.421 POST …/prompt {"text":"echo $HOME"}
OpenCode 2.2.5 behaviour this relies on (raw API probes)
A directory's first GET /api/skill returns [] (3/3 runs). Later reads return partial sets (opencode, report → +browser → +project skills) over about 100–300 ms. During that window GET /api/command already returns all of its commands.
The end of the scan is one skill.updated event carrying location.directory. Rereading a directory that is already scanned emits nothing.
The barrier: in 5/5 runs on fresh directories with 200 and 2500 project skills (plus 66 global), a skill read sent right after GET /api/model returned listed every skill. GET /api/model took 0.4 s and 1.6 s, growing with the scan.
Not checked: OpenCode versions other than 2.2.5. The probe scripts and logging proxy used above aren't attached, because the upload tool wasn't available on the machine that ran them.

Evidence-only round on @juliusmarminge's PR. No code changes; evidence and verification added below.
Evidence
Server-only: every row is a timestamped transcript of T3's requests to a real opencode serve 2.2.5 through a logging proxy (S = GET /api/skill, M = GET /api/model, P = POST …/prompt and its skills). Injected rows fail or hold one named request in the proxy. Viewport and theme do not apply.
both sides: prompt POSTs ~15 ms after Stop persists, /interrupt follows ~30 ms later, run settles interrupted (pre-existing ordering; head's wait is longer)
Environment: fs-25 Linux 6.1.102 x86_64, Node 26.12.1, OpenCode 2.2.5 release binary (sha256 8d060e1d…a92050), model openai/gpt-5.4 via the hub, fresh databases (T3CODE_DEV_SEED=0), isolated HOME, synthetic fixture project. R16–R19 (native commands, /compact, steering, attachment fallthrough) don't reach the changed resolver and were checked in source only.
First read [] 6/6, partial → full in 187–304 ms. Differs from the description:GET /api/command returned 3 commands during the scan versus 5 after it (3/3 runs).
One directory-scoped skill.updated per scan, 16/16; none across 48 warm rereads.
Barrier: a skill read right after GET /api/model listed every skill 5/5 at 224 and 5/5 at 2524 IDs (21 synthetic globals). Model reads took 0.86–1.05 s and 1.33–1.43 s, versus 0.4 s and 1.6 s in the description, which came from another machine.
What else changed: unknown $words add one M and one S2 read without changing the payload (R06/R07); recovery rows now attach greet; R03 shows background catalog calls interleaving differently. Nothing outside the title's scope.
Proof audit: the fixture matrix (19 rows), then two passes over the captures. Every captured row is sufficient: R03 after an isolated recapture, the barrier mechanism reproduced, and the auditor rebuilt R04, R06 and one probe-(c) run and they matched. R05 stays unverified (conditional: the description claims no native partial-read pair; R04 + R08 + probe (a) cover it). The auditor's open items are the two description numbers that differ above.
Tests: sends a $skill that appears once OpenCode finishes scanning the directory fails on the merge-base (log, prompt has no skills) and passes on the head (log). The reread-empty test's base failure is a replay-order mismatch, not a lost skill (before, after). OpenCode2AdapterV2.test.ts has 87/87 at f23933f; vp run check:changed passes, 88/88 across its 2 test files (log).
…ts directory scan ends
The first skill.list for a directory OpenCode has not finished scanning
returns no project skills. The adapter sends the prompt from that single
read, so `$greet` goes out without `skills`. This test fails on main.

…ing a $skill
OpenCode 2 lists a directory's skills partially until its background scan
ends with `skill.updated`, so a turn sent right after an OpenCode restart
read no project skills and went out without `skills`. When a mentioned
skill is missing from the first read and that read may be mid-scan (empty,
or a directory this runtime has not seen finish scanning), the adapter now
waits for the directory's `skill.updated` from its existing event stream,
bounded by the inventory timeout, and reads once more.
Fixes#29513

…efore resolving $skills
OpenCode answers its model list for a directory only once the directory's
scan has ended, so a missed $name rereads skills after that barrier. It needs
no scan tracking, works while the event stream is down after a restart, and
costs a $word that is no skill one quick model read rather than a 5 s wait.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Macroscope's review found this PR approvable — The adapter retries missing explicitly mentioned skills after waiting for directory discovery, rather than silently omitting them. The additional reads are time-bounded and leave prompts without skill mentions unchanged.

Walkthrough
The OpenCode adapter now refreshes the skill inventory when a prompt mentions a skill absent from the initial listing. Tests cover skill listings that change during a directory scan and verify prompt inclusion and turn completion.

Summary
Skill lookup and prompt validation packages/provider-opencode/src/server/v2/adapter.ts, apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts
The adapter extracts distinct skill names, lists skills with a timeout, and refreshes models before rereading the skill list when a mentioned name is missing. Tests cover a skill appearing after the initial listing and a skill present initially but absent on reread.

Merge Risk: Moderate · up to 0c375
This change makes OpenCode more likely to include a mentioned skill when a directory is still being scanned. On a slow scan, or one that outlasts the five-second bound, the prompt can still be sent without the skill. This is a bounded correctness gap in an edge case, and it should be resolved or explicitly accepted before merge.

Full details: Title check
Explanation
The title names the main change: OpenCode no longer drops a $skill read before its directory scan ends. The adapter now waits on the model-list read and rereads skills when a mentioned skill is missing. The title is specific and follows the conventional commit form used in the repository template. It does not name the wait-and-reread mechanism, but it does not need to.
Full details: Description check
Explanation
The description covers the problem, the change, scope and approval, and verification. The Problem section gives reproduction steps and an environment (OpenCode 2.2.5). The Change section explains the barrier and merge logic. The Scope section links issue #29513. The Verification section gives specific test names, results, and runtime traces. It uses the template's section headings. Two weaknesses remain. The Scope section is a single line and does not explain why no prior approval is needed beyond the issue link. The description also says 88/88 tests pass, while the takeover evidence reports 87/87 in one file at one commit. The author should reconcile those counts. The verification also marks R05 (2,500-skill cold partial read) as unverified, which the description should state clearly in the Verification section. The description also includes a long evidence table with many external links. Those links cannot be checked from the PR itself, so reviewers may need the key results inline. Overall the description is mostly complete and meets the template.
Full details: Linked Issues check
Explanation
Issue #29513 requires submit-time OpenCode skill resolution to preserve a mentioned skill when the first location-scoped read is empty or partial. The current adapter reads skills, uses the existing model-list read as a scan barrier when a mention is missing, rereads skills, and merges both results before sending the prompt. The reported tests cover delayed discovery and preservation after an empty reread. The issue's event-based approach is a suggestion, not a required implementation.
Full details: Out of Scope Changes check
Explanation
The current head also changes rollback history handling in packages/provider-opencode/src/server/v2/adapter.ts and adds rollback and fork message-list tests in apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts. These changes concern rollback and session forking, not submit-time skill resolution for issue #29513.

skillsMentioned can make three sequential inventory requests, but its new Effect.fnUntraced declaration gives no reason to omit a span. Use Effect.fn("OpenCode2Adapter.skillsMentioned"), or document why the enclosing operation’s span is sufficient. The same decision applies to the new listSkills I/O helper. As per coding guidelines, “Default to named spans for service operations and I/O” and explain deliberate untraced work. The matching path instruction also requires named spans for reusable Effect functions.

* fix(web): play completion sounds without delay (pingdotgg#29146)
Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.
Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.
This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.
Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)
* fix(server): keep PR watches active after host failures (pingdotgg#29450)
## Problem
PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.
## Change
Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.
The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.
## Scope and approval
This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.
## Verification
`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.

Problem
With OpenCode 2, a
$skillmentioned in a prompt was sometimes dropped before it reached OpenCode. The prompt went out with noskills, so the model got the literal$nameand never saw the skill's content. T3 resolves mentions against a singleGET /api/skillread for the thread's directory. OpenCode fills that list in the background: a directory it hasn't finished scanning lists no skills or only some, for roughly 100 ms to over a second. Any mention missing from that one read was discarded.Reproduced on main with the real dev server against OpenCode 2.2.5:
$greet, immediate send): the skill is attached. T3'sGET /api/modelfor the new directory runs first and returns only once the scan has ended.$greet again please, made oneGET /api/skillagainst the cold directory. It then sentPOST .../prompt {"text":"$greet again please"}with noskills. OpenCode's idle location eviction leaves a directory just as cold.Change
A prompt whose mentions are all found on the first
skill.listis sent exactly as before. If a mentioned name is missing, the adapter waits for OpenCode to finish scanning the directory, then reads the skills again:readModels, 5 s timeout, logged on failure). OpenCode 2.2.5 answers it only once the scan has ended. This is observed behaviour, not a documented contract.A
$wordthat isn't a skill, such as$HOME, now costs one model read and one skill read, about 50 ms in a directory OpenCode has already scanned.Scope and approval

Verification
apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts, 88/88 pass after merging main:sends a $skill that appears once OpenCode finishes scanning the directory: fails on main withexpected { …(3) } to have deep property 'skills', passes here. The first read is empty, the barrier runs, and the reread returnsgreet.keeps a $skill found on the first read when the reread after the scan comes back empty: guards the merge of the two reads. It fails without that merge.vp run check:changedpasses: format, lint, knip, typecheck of 10 packages, tests.opencode serve, with a logging proxy between them recording T3's requests. Before: main 0847a44, OpenCode restarted under the open thread
After: f82e176, same sequence

GET /api/skillreturns[](3/3 runs). Later reads return partial sets (opencode, report→+browser→+project skills) over about 100–300 ms. During that windowGET /api/commandalready returns all of its commands.skill.updatedevent carryinglocation.directory. Rereading a directory that is already scanned emits nothing.GET /api/modelreturned listed every skill.GET /api/modeltook 0.4 s and 1.6 s, growing with the scan. Not checked: OpenCode versions other than 2.2.5. The probe scripts and logging proxy used above aren't attached, because the upload tool wasn't available on the machine that ran them.
