Problem

When the SSH key lives in an agent that can't sign right now, T3 Code asks for a password. 1Password and Secretive both refuse to sign while the Mac is locked. Every background reconnect then fails with:

sign_and_send_pubkey: signing failed for ED25519 "~/.ssh/id_ed25519" from agent: communication with agent failed
<user>@<host>: Permission denied (publickey,password).

isSshAuthFailure matches the Permission denied line, and handleSshAuthFailure opens SSH Password Required. Nobody is at a locked machine to answer it, so it expires, and the next reconnect opens another one. The reporter's desktop traces show 172 prompts for one host, one every 4 minutes while the screen was locked. No password exists for that account.

Change

  • isSshAgentSigningFailure in packages/ssh/src/auth.ts recognizes OpenSSH's agent signing failure. It covers the 8.2+ form (signing failed for <type> "<key>" from agent: …) and the older form (signing failed: agent refused operation / communication with agent failed). Signing failures for keys loaded from a file don't match.
  • handleSshAuthFailure returns the original SSH error for that case instead of prompting.

OpenSSH asks the agent to sign only after the server has accepted the offered key. This failure therefore means the key is authorized and only the local agent couldn't sign. A password is the wrong remedy, and the key works again as soon as the agent can sign. The error is the same SshCommandError as any other non-prompting failure, so the connection supervisor's existing backoff (up to 5 minutes) handles retries.

Unchanged: a cached password is still cleared after a failed attempt, and hosts that reject every key still get the password prompt (covered by a test).

One deliberate tradeoff: if the agent can't sign but the host also accepts a password, the user now sees the SSH agent error instead of a password prompt. The key the server accepted works again once the agent is unlocked.

Scope and approval

Fixes #23413. That issue isn't triaged yet. I'm submitting this under the small, focused fix exception:

  • T3 asks for a password when the server already accepted the key and only the local agent failed. The prompt can't succeed while nobody is there, and keeps reappearing.
  • The fix is one classifier and one early return in the existing auth-failure path. No contract, client or settings changes.

Related work, kept out of this PR:

Verification

Failing test first (main @ 3b95e7831b):

  • New tunnel.test.ts case "does not ask for a password when the agent cannot sign an accepted key" drives SshEnvironmentManager.ensureEnvironment with the 1Password stderr. On main it fails with expected [ 1 ] to deeply equal [] (one password prompt requested).
  • The control case "asks for a password when the server rejects every key" passes before and after the fix.
  • auth.test.ts covers the current and pre-8.2 OpenSSH formats, a non-agent signing failure, and a plain Permission denied.

With the fix:

vp test run packages/ssh/src/tunnel.test.ts packages/ssh/src/auth.test.ts
 Test Files 2 passed (2)
      Tests 12 passed (12)

vp run check:changed
  pass format
  pass lint
  pass knip
  pass typecheck (4 packages)
  pass test (2 files)

Runtime transcript (macOS 26.6.2 arm64, OpenSSH_10.3p1):

Setup, all throwaway:

  • sshd on 127.0.0.1:22222 accepting one key and advertising publickey, password
  • ssh-agent holding that key, added with ssh-add -c and no usable askpass, so it refuses to sign
  • an ssh wrapper on PATH pointing at a config whose IdentityAgent is that agent

A small driver called the real SshEnvironmentManager.ensureEnvironment with NodeServices.layer (real process spawning) and a password-prompt service that records requests and returns null, like a prompt nobody answers. Paths are shortened.

Before (fix stashed):

ssh stderr: sign_and_send_pubkey: signing failed for ED25519 ".../user_key" from agent: agent refused operation
            <user>@127.0.0.1: Permission denied (publickey,password).
>>> PASSWORD PROMPT OPENED (attempt 1): Enter the SSH password or verification code for <user>@t3-transcript.
ensureEnvironment failed with SshPasswordPromptError
message: SSH authentication cancelled for <user>@t3-transcript.
password prompts opened: 1

After:

ensureEnvironment failed with SshCommandError
message: sign_and_send_pubkey: signing failed for ED25519 ".../user_key" from agent: agent refused operation
         <user>@127.0.0.1: Permission denied (publickey,password).
password prompts opened: 0

Recovery: I re-added the same key to the agent without -c, standing in for unlocking 1Password. Then ssh -o BatchMode=yes t3-transcript through the same config logged in with exit 0 and no input.

Not checked:

  • The packaged desktop app with the real 1Password agent and a locked screen. The reporter's 1Password 8.12.38 stderr (communication with agent failed) has the same from agent form, and the unit test uses it verbatim.
  • Windows OpenSSH.
  • The full launch past authentication on the fixture, which would have started a T3 server on the test machine.

Implemented with Claude Opus 5.5 (claude-opus-5-5) in Claude Code, running in T3 Code.

When an agent such as 1Password or Secretive can't sign (for example while
the screen is locked), OpenSSH reports "signing failed ... from agent"
followed by "Permission denied (publickey, password)". T3 treated that as a
password requirement, so every background reconnect opened a password
prompt nobody could answer.

OpenSSH only asks the agent to sign after the server accepts the key, so a
password is the wrong remedy. Return the original SSH error instead; the
key works again once the agent can sign.

Preserve signer provenance before classifying ambiguous diagnostics.

The SSH handler receives only the spawned process error and captured stderr. The sign_and_send_pubkey: signing failed for …: agent refused operation form is ambiguous: OpenSSH can emit it for both an agent-backed private key used by a file-backed certificate and a file-backed private key. Do not broaden isSshAgentSigningFailure to match this text alone, because that would suppress the password prompt for file-key signing failures.

The current SSH process boundary does not expose selected-signer provenance. Add a provenance-preserving launch mechanism or error contract, then classify the no-suffix diagnostic as an agent failure only when the selected signer is agent-backed. Keep file-key failures on the existing password-prompt path.

updated the original branch at 741f71d0f820b9c89075fa40ae386a871ee92ec9, preserving the contributor's commit and merging current main without rebasing.

thanks to thepriceisright: the original classifier and the decision to preserve the ssh error are retained. the handler now shares the existing early return, with shorter commentary. added coverage for legacy communication errors, ambiguous certificate signing errors retaining the password prompt, and preservation of the original error and stderr.

verification:

  • final ci: all 29 checks green, including build/typecheck, lint, web tests, eight server shards, and coderabbit. the pr is mergeable. coderabbit's final review generated no actionable comments; its summary repeats the certificate limitation below.
  • current main d92c437baa19 reproduced the regression: one password prompt instead of zero; the rejected-key control passed.
  • blacksmith: focused auth/tunnel suites passed 16 tests. vp run check:changed passed format, lint, both knip checks, five package typechecks, and 17 tests across three files.
  • real linux openssh and SshEnvironmentManager.ensureEnvironment: an approval-required agent key produced one prompt on main, zero on this head, and preserved SshCommandError. re-adding the same key without approval restored ssh login, exit 0. runtime transcript.
  • two independent gpt-6.1-sol reviews approved this exact head with no findings.

unverified: packaged desktop rendering, macos with locked 1password/secretive, and windows openssh. the web client has no desktop ssh entry point; no client rendering code changed, so no synthetic browser evidence is included.

on the existing certificate review: the classifier deliberately leaves ambiguous no-from agent certificate/file diagnostics on the password path. obtaining signer provenance would require a separate ssh launch/protocol change. the new control proves that conservative behavior. this remains a known limit rather than expanding this focused fix.

Verified in a real dev environment

verified as-is at 741f71d0f820b9c89075fa40ae386a871ee92ec9 on a linux boat vm. no source changes or pushes.

  • reproduced on github/main at 80a2351fcb909034e65a512dfabeb3cdfd367762 in a separate worktree, then installed and ran the exact pr head. only one dev server ran at a time.
  • used the actual electron renderer and preload, driven with repository playwright: settings > environments > add environment > ssh. local backend startup was disabled so desktop used the existing dev web server without spawning another backend.
  • real sshd on loopback accepted an ed25519 key held by a real ssh-agent with confirmation required. OpenSSH emitted sign_and_send_pubkey: signing failed ... from agent: agent refused operation, followed by Permission denied (publickey, password).
  • main opened the ssh verification/password dialog. the pr returned the original signing error without that dialog. repeated attempts behaved the same way.
  • neighboring behavior: an unauthorized key still opened the verification/password dialog, and cancellation worked. re-adding the accepted key without confirmation restored ssh authentication. the desktop retry got through authentication, confirmed by sshd's Accepted publickey and command-session logs.
  • in /usr/bin/google-chrome, added a project through the app pointing to a real git repository and completed two real codex gpt-6.1-sol turns. one read the readme and ran git status; the next ran ssh through the same recovered agent and returned SSH_AGENT_RECOVERY_PROVIDER_OK, exit 0.
  • blacksmith: both focused ssh files passed, 16 tests. vp run check:changed d92c437baa19409f99416a44f748f148f4317c65 passed format, lint, knip, typecheck for 5 packages, and 17 tests across 3 files. the worker needed depth-3 head history before it could find the comparison base. the pr reports 29 successful checks.

short runtime transcript

main: accepted key with refusing agent opens the password dialog

pr: same host and refusing agent return the signing error without a password dialog

main: ssh retry opens the password dialog

pr: ssh retry returns the signing error without a password dialog

pr: rejected key still opens the password dialog

real codex turns read the project and confirm recovered ssh authentication

Not verified

  • full remote environment bootstrap after recovery: desktop authentication succeeded, then the dev version's remote runtime archive download returned HTTP 404. the codex turns above ran on the local dev environment, not a bootstrapped ssh environment.
  • actual macos screen locking with 1password or secretive, windows openssh, packaged release desktop, and native ios/android clients. this vm is linux; the real linux agent refusal exercises the changed diagnostic and actual desktop prompt path.
  • cached-password invalidation, automatic background reconnect/backoff, and the pre-8.2 OpenSSH diagnostic were not exercised live. the focused existing tests cover diagnostic variants; no claim of live coverage for those paths.

captures were inspected; uploaded videos played and sought successfully in t3's player.
