fix(server): keep PR generation focused on the change - #31819
Agents could carry unrelated context into a new task. PR generation could follow a later babysitting request in the supplied conversation and publish a blocked-status message as the title and body. A new independent worktree could also inherit the current feature branch, accidentally creating a stacked PR.
Change
Clarify that PR writing is a separate task, mark conversation history as reference material, and restate the task after the diff. Honor a requested base. For independent work without one, use the supported remote/branch form for the default branch and fetch it; preserve required unpublished commits and use the local default when no remote exists. Dependent work can retain its parent branch. Both fixes keep a new task from inheriting unrelated conversation or branch context.
Existing Codex developer instructions, configuration, and checkout exploration remain unchanged. The opt-in eval covers task confusion and configured title prefixes. Review fixes also validate case names, preserve reused baseline files, and reject status-only PR bodies.
Scope and approval
This is maintainer-directed work by Yash-Singh1, who is explicitly listed in the repository's contribution-triage exemptions. The maintainer requested both reported problems be fixed together and authorized squash-merging after validation, provided review fixes do not materially expand scope. The changes remain within that request; they add no settings, runtime launch-service behavior, or Codex instruction overrides.
Verification
vp run check:changed passes, including focused tests, formatting, lint, unused-code, GraphQL, and affected-package type checks.
All nine PR-content eval samples pass with Luna, covering historical babysitting requests, ordinary changes, and configured title prefixes.
Actual CLI request captures confirm that custom instructions from both Codex configuration and T3 launch arguments survive PR generation.
Reproduced baseline overwrite when the input is also the output results path, then verified the original baseline is preserved byte-for-byte. Invalid baselines fail before model calls or output writes.
Worktree-launch eval cases build in dry-run mode. Their live run remains unavailable because this machine lacks the eval runner's proxy configuration. The existing selected-remote-base test verifies the remote-qualified checkout path.
The context-budget update records the measured guidance increase and carries the required agent-context-increase label. No static-analysis suppressions are added.
- Treat thread history as reference material during PR writing
- Add an eval for follow-up requests that could redirect generated PR content
- Use the repository default branch for independent worktrees
Macroscope's review found this PR requires human approval — PR generation is refocused on the diff rather than historical follow-up requests. The accompanying worktree guidance changes the default base for independent threads to the remote default branch, requiring human review of that behavior change.

The PR updates worktree base-branch guidance for independent and dependent work. It revises PR-content prompts, expands Codex test checks, and adds an evaluation runner that grades generated content across configured cases.

Summary
Prompt and generation checks packages/provider-core/src/server/textGenerationPrompts.ts, apps/server/src/provider/agentContextBudget.json, apps/server/src/textGeneration/CodexTextGeneration.test.ts
The prompt identifies PR writing as a new task, marks thread content as historical reference, and directs the model not to carry out follow-up requests. Context budgets increase. The Codex test harness checks the working directory and developer instructions, and the PR-content test uses those checks.
The runner generates and grades samples for three scenarios, records results, and can copy a baseline. Tests cover the grading predicate. The documentation describes evaluation commands and run details.

evals/README.md requires a summary or review file for a person. The runner writes only results.json. The README also says a case fails when half or more of its samples fail, but the runner reports only the per-sample count. Add a per-case pass/fail summary. The baseline.json copy provides no comparison.

Reject status-only PR bodies
A response with title “Retire legacy model” and body “All checks passed.” can pass the grader without describing the diff. The status-only test misses this because its title fails first. Reject status-only bodies without rejecting valid PR bodies that include test results, and add a test with a retirement title.

Addressed the outside-diff grader finding from the latest review. The retirement fixture now requires change-related content in the body, so a retirement title with only "All checks passed." fails. A substantive retirement description that also reports passing tests still succeeds. Both cases are in the existing grader test table. The PR description now follows the required template and records the maintainer-directed scope and verified triage exemption.
Remove the expected base-ref choice from the case context.
The context tells the model to use the explicit default-branch ref. The base-ref assertion can therefore pass without the new guidance.
Keep the repository facts, but remove that instruction. Run the case against the old and new guidance to check that it fails before the fix and passes after it.

Addressed the eval-context nit from the latest review. Removed the instruction telling the model which base ref to choose. The context and notes now contain repository facts only: the default branch, stale local main, and obsolete upstream. The expected decision must come from the shipped guidance. Added requested-base and no-remote guards without changing launch-service behavior.
