fix(server): a settings save no longer reads an unreadable secret again - #31226
A provider instance whose secret could not be read (for example, a locked vault) is rebuilt on every settings save, even when the save does not touch it. Each save reads the vault again, so a locked vault is prompted for on every unrelated change.
Change
ProviderInstanceRegistry remembers which instance configs are parked on an unreadable secret. A settings save that leaves a parked config unchanged keeps it parked instead of rebuilding it. A refresh (reloadSecrets) still retries every unavailable instance, so unlocking the vault and refreshing brings it back.
Scope and approval
Small, focused fix of an obvious bug in the registry's reconcile path, with no new setting or behavior beyond not repeating a read that already failed. It touches one source file and its test.
Verification
The new test, "does not read an unreadable secret again when a settings save leaves it alone", fails on main (secret reads go from 3 at startup to 5 after one unrelated save) and passes with the fix.
A second test covers the reverse path: after the vault is unlocked, reloadSecrets starts the parked instance.
vp test run src/provider/ProviderInstanceRegistry.test.ts src/provider/ProviderRegistry.test.ts passes (78 tests), and vp run typecheck passes for apps/server.
Not checked: a real locked 1Password vault. The tests use a stub resolver.
Macroscope's review found this PR approvable — Unrelated settings saves stop retrying unreadable secrets, while explicit refreshes and relevant configuration changes still allow recovery. The change is limited to in-memory provider reconciliation and preserves working instances when secret rereads fail.

The registry tracks configurations whose secrets could not be read. It reuses unavailable snapshots for unchanged configurations without repeating vault reads. The settings watcher reloads secrets when secretProviders changes and can rebuild enabled targets without live instances.

Park and rebuild unreadable instances apps/server/src/provider/ProviderInstanceRegistry.ts, apps/server/src/provider/ProviderInstanceRegistry.test.ts
The registry marks secret-read failures and reuses unavailable snapshots when the configuration is unchanged. It skips repeat vault reads for those configurations. reloadSecrets marks enabled targets without live instances for rebuild. The test checks unchanged reconciliation and confirms that the instance remains unavailable without another vault read.
Reload secrets when provider settings change apps/server/src/provider/ProviderInstanceRegistryHydration.ts, apps/server/src/provider/ProviderInstanceRegistryHydration.test.ts
layerSettingsWatcher is now exported. It reloads secrets when secretProviders changes. A test verifies the call sequence reconcile, reloadSecrets, then reconcile.
A secret-provider change saved during startup can leave a provider unavailable until another retry. The change is mergeable with awareness of this narrow race.

Reload secrets when a secret provider changes.
The watcher calls mutator.reconcile on every settings change and never calls reloadSecrets. A change to secretProviders alone leaves the derived instance configuration unchanged. The registry can therefore keep the instance parked on its unreadable secret, and the corrected secret is not retried.
The new test retries unreadable secrets when a secret provider changes expects ["reconcile", "reloadSecrets", "reconcile"]. The current watcher produces only reconcile calls, so this assertion fails.
Track the last applied secretProviders value. After reconciliation, call mutator.reloadSecrets() when that value changes. Leave saves that do not change secretProviders without a reload.
